What the care home does well

The staff at the home evidently work hard to meet the needs of the service users. Comments received from service users about the staff was positive. It was evident that the management team at the home have worked hard to build good relationships with the service users and relatives. The meals provided at the home are of a good standard. Feedback from service users was positive regarding the meals provision. Activities are provided to meet the needs of the service users living at the home. Recruitment processes protect the service users living at the home. Monthly management reviews are carried out.The views of service users are sought regarding the service provided at the home.

What has improved since the last inspection?

Staff recruitment processes are thorough. Monthly management visits are carried out to a good standard; this has benefited the service. The standard of cleanliness in the bathrooms has improved. Mandatory training has been delivered within recommended timescales. The mealtimes have been reviewed to meet the needs of the service users. Service users` monies are safeguarded.

What the care home could do better:

Although the management of medication at the home has improved, there are still some administrative errors occurring. These must be addressed.

One named person aged under 65 years of age To accommodate a maximum of two services users aged between 55 and 65 years in the ten intermediate care bed unit. 23rd August 2006 Date of last inspection Brief Description of the Service: Fieldhead Park is a care home providing accommodation and personal care for 54 persons who are experiencing issues relating to the ageing process. Ten of the places are reserved for persons requiring intermediate care. The accommodation is an extended and adapted former Victorian detached property in its own grounds. It is owned by a private limited company that own other homes in the West Yorkshire area. All but two of the bedrooms are for single occupancy; all the rooms have ensuite facilities. The facilities are based over two floors that are joined by two passenger lifts. The home has a number of lounges and dining facilities. The home is sited some distance from the town of Mirfield, within a short distance of some local facilities. The provider informed the Commission for Social Care Inspection on the 14.03.07 that the fees range from £332.98 to £575.00 per week. There are additional charges for hairdressing, newspapers, magazines, private chiropody and specialist therapy (e.g. aromatherapy). Fieldhead Park Care Home DS0000001083.V328298.R01.S.doc Version 5.2 Page 5 SUMMARY This is an overview of what the inspector found during the inspection. As part of this key inspection a visit to the home took place. The inspector visited the home unannounced from 9.10am to 3.45pm. Whilst at the home, key documents such as care assessments, care plans, daily records, medication, training records and the staff records were looked at, and so were some of the rooms. 4 service users, and 3 members of staff were spoken with, along with the management team. The last key inspection was carried out on the 29th June 2006, where there were major issues identified regarding the health and welfare of service users on the intermediate care unit. Since then, three further unannounced random inspections have been carried out at Fieldhead Park to monitor the health and welfare of service users and check on the progress being made at the home to improve standards. These visits were carried out on 12.7.06, 21.8.06, and 20.11.06. The intermediate care unit closed 14th July 2006 for a short time so that the issues could be addressed in full. The unit reopened in October 2006 when it was able to meet the needs of the service users referred to the unit. The staff and management team have worked hard to improve the standards at the home, and have addressed any issues in a positive and constructive manner in order to move the service forward. Service users have their needs assessed prior to admission to the care home, those assessed and referred for intermediate care are helped to maximise their independence and return home. Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. EVIDENCE: The pre-admission information held for four service users were examined. These included a detailed assessment and a copy of a Community Care Assessment. Those admitted for intermediate care had clear information on their needs on admission, the reasons for the admission and the objectives set prior to returning home. There is a marked improvement since the last key inspection visit. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 7, 8, 9, 10. Service users’ health, personal and social care needs are set out in the individual plan of care. Risk assessments are carried out and monitored regularly. The service users are not always protected by the home’s medication policy and procedure. Service users are treated with dignity, respect and privacy. Quality in this outcome area is adequate. This judgement has been made using available evidence including a visit to this service. EVIDENCE: The care plans for four service users were examined. Two were for service users on the intermediate care unit. All four care plans were found to contain satisfactory assessments and plans of care relating to the action required by staff to meet these needs. Monthly weights are recorded and assessments related to mobility, tissue viability, falls, continence, dental health and nutritional state were included. Information relating to wound care was clear and gave a clear picture of the condition of the wound. There was clear Fieldhead Park Care Home DS0000001083.V328298.R01.S.doc Version 5.2 Page 10 evidence of intensive involvement from other health care professionals including district nurse, occupational therapist, physiotherapist, dentist, optician and GP. This is a marked improvement since the last key inspection and staff should be commended on the hard work to improve the care records. The management of medication, policies and procedures relating to medication have improved, however they are still not always protecting the service users. The medications of five service users were checked. Administrative errors on the medication administration record sheets were seen, where a small number of signature omissions were noted. All medication stored in blister packs tallied with the records held, however there were some discrepancies with medication stored in boxes. The manager of the home was aware of this and demonstrated that action is being taken to address this issue. Insulin and eye drops were labelled with the date of opening and the temperature of the medication fridge is recorded as required. It was evident that the staff know the needs of the service users and actually provide the care to meet those needs. This was observed on the day of the visit and confirmed by speaking to staff and service users. Service users were seen to be treated with dignity and their privacy maintained on the day of the visit. Service users spoken to said that they are treated well by the staff. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 12, 13, 14, 15. Generally, the service users’ cultural, religious, social and recreational needs are being met, and they are supported to maintain contact with their family and friends. The service users are able to exercise choice and control over their lives. The home provides the service users with a varied and nutritious diet. Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. EVIDENCE: Various activities are organised at the home. Activities residents said they enjoyed were bingo, playing cards, discussions, “play your cards right”, reading and watching TV. One service user stated that there is something to do on most days. Another service user was waiting to go out on the morning of this visit and was enthusiastic in speaking about where she was going and what she planned to do. This is a regular occurrence for this lady. A minibus is available for residents at the home. A church service is held at the home every month. The dates for these services were displayed near the Fieldhead Park Care Home DS0000001083.V328298.R01.S.doc Version 5.2 Page 12 entrance of the home and this was also confirmed by residents. One resident stated that they go out to their own church every week. Residents were seen to be sat in the various lounges and in their bedrooms. Residents confirmed that they are able to have visitors and see them in private whenever they want. One service user stated that a member of their family rings every day after lunch, on the phone fitted in their room. The mealtime provision has been reviewed since the last key inspection and the times of meals altered to suit the needs of the service users. Evidence was seen regarding the consultation of service users in minutes of service user meetings. This was also confirmed by speaking to service users. One service user stated that following the service user meeting service users have had tripe and curry after requesting these two meals, and that they had been very nice. Service users stated that meals are generally good at the home. Service users were observed to be having breakfast at the beginning of this visit. Those who required assistance were seen to be helped by staff in a discreet and professional manner. Residents are able to exercise choice over their lives. This was confirmed by a resident stating how and when they like to get up, go to bed, stating that they don’t like to be disturbed from their chosen seating during the day, and how they spend their day. This judgement has been made using available evidence including a visit to this service. EVIDENCE: The complaints procedure is displayed near the main entrance to the home. This contains the required information. Records of complaints received by the home were seen and the responses were seen to be made within the given timescale. The responses were open and appropriate action had been taken where needed. Communication within the home has improved and this has had a positive impact on the service. Service users spoken to stated that they felt able to speak to the manager if they had any concerns. One service user gave an example of where an issue was raised and had been dealt with appropriately by the manager. Training records indicated that all staff have had adult protection training. Issues raised in the past at the home regarding adult protection have been dealt with appropriately and policies and procedures followed. The service users live in a safe and overall well maintained environment that is generally clean and hygienic. Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. EVIDENCE: Generally, the home is well maintained and is clean. There were no unpleasant odours noted during this visit. The issues raised in the last key inspection regarding the bathroom opposite the kitchen, and another bathroom on the first floor have all been rectified. Some of the bedrooms were seen during this visit and were clean, personalised with ornaments, pictures and photographs and had good furniture and fittings. Staff are employed in sufficient numbers and receive induction and ongoing training. The recruitment process ensures the service users are sufficiently protected. Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. EVIDENCE: Rotas examined indicate that there are sufficient numbers of staff in place to meet the needs of the residents at the home. This was confirmed by service users spoken to who said that staff are available to meet their needs. Staff were observed to be carrying out care duties in a competent manner. It was reported that all staff have received adult protection training, movement and handling training and fire training. All staff have completed health and safety training. Sixty two percent of care staff have achieved the NVQ level 2 in care award. Other training has included first aid, medication and food hygiene. Training records confirmed that this training has taken place. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 31, 33, 35, 38. The home is run and managed by two managers who are fit to be in charge. Generally, the home is run in the best interests of the service users. The financial interests of the service users are adequately safeguarded. The health and welfare of service users and the staff is promoted and protected. Quality in this outcome area is adequate. This judgement has been made using available evidence including a visit to this service. Monthly management visits are now carried out to a good standard and quality assurance systems have been implemented. These have been instrumental in improving standards at the home since the last key inspection. The staff and management should be commended on their efforts, however this must continue to ensure that standards are maintained. The personal money held by the home for three residents was checked during this visit and tallied with the records held. There were no health and safety hazards observed during this visit. Weekly tests of the fire alarm and emergency lighting systems were recorded as being carried out. Monthly checks of the hot water temperatures are also recorded.
