One named person aged under 65 years of age To accommodate a maximum of two services users aged between 55 and 65 years in the ten immediate care bed unit. 6th October 2005 Date of last inspection Brief Description of the Service: Fieldhead Park is a care home providing accommodation and personal care for 54 persons who are experiencing issues relating to the ageing process. Ten of the places are reserved for persons requiring intermediate care. The accommodation is an extended and adapted former Victorian detached property in its own grounds. It is owned by a private limited company that own other homes in the West Yorkshire area. All but two of the bedrooms are for single occupancy; all the rooms have ensuite facilities. The facilities are based over two floors that are joined by two passenger lifts. The home has a number of lounges and dining facilities. The home is sited some distance from the town of Mirfield, within a short distance of some local facilities. Fieldhead Park Care Home DS0000001083.V272662.R01.S.doc Version 5.1 Page 5 SUMMARY
This is an overview of what the inspector found during the inspection. This was an unannounced inspection and was undertaken by two inspectors over a four-hour period. . The inspectors made a tour of the building and looked at a sample of records kept by the home. The inspectors also spoke to some of the residents and staff. The inspection was conducted with help from the managers of Fieldhead Park Care Home. What the service does well: What has improved since the last inspection? Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. The intended outcomes for Standards 7 – 11 are: 7. 8. 9. 10. 11. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 7,9. Improvements are required with the care planning system operated by the home, to ensure they include sufficient detail to meet the needs of the service user. Greater care must be taken to ensure the policy and procedure for medication is consistently followed in order to protect the service users. EVIDENCE: Four care records were looked at during this inspection. There has been some improvement since the last inspection, however the care plans looked at do require greater attention to detail. Where a service user has a wound or sore, there should be a record of the initial size of the affected area with ongoing records kept to allow its progress to be monitored. A service user who had been recently admitted to the home did not have a working care plan in place. Fieldhead Park Care Home DS0000001083.V272662.R01.S.doc Version 5.1 Page 9 Greater care needs to be taken when developing care plans to ensure that words such as “regular” and “sufficient” are avoided. Specific details should be recorded in the care records kept. Generally the daily records kept for service users were vague and did not reflect how the service user had spent their day. The medication records were looked at for three service users. The PRN (When required) medication could not be reconciled with two of the records kept. Where there is a variable dose of medication to be administered the amount given should be recorded clearly on the MAR sheet. There was no policy and procedure available for medication during this inspection and a requirement is made in this report. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): EVIDENCE: These standards were not assessed during this inspection however; the manager has attempted to address the requirement made following the last inspection regarding adult protection training. Many staff has either received this training or they are to attend a training date planned for later in February 2006. The recommendation made at the last inspection that staff should undertake training to ensure that complaints are taken seriously is now addressed. The manager said that this issue is now included in the adult protection training. As the requirement and the recommendation from the last inspection have not yet been met for all the staff, these will be carried forward until all staff have undergone adult protection and complaints training. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 19. The service users live in a homely environment that generally safe and generally well maintained EVIDENCE: On the day of this unannounced inspection the home was generally clean and odour free. The recommendation from the previous inspection has been addressed and the identified carpets have now been thoroughly cleaned. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 28,29,30. The recruitment process has generally improved and generally protects the service users. The home is working well in addressing NVQ training for staff however; further improvements are required to ensure that all staff are sufficiently trained to do their jobs. EVIDENCE: The home has a training officer and training assessors. 41 of the carers who work at the home have achieved the NVQ level 2 in care. The training records kept by the home were audited as part of this inspection; the inspector noted that some of the records kept were not up to date. The evidence available showed that not all the staff has received the training to ensure that they are sufficiently trained to do their jobs. Some staff have not undertaken fire lectures, fire drills or movement and handling training. Three staff files were audited. Generally the recruitment practices at the home are sufficient to protect the residents. However more care needs to be taken to ensure that any gaps in the employment histories are explored. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 33. The quality monitoring system must be developed in order to ensure the home is run in the best interests of the service users. EVIDENCE: Little has changed since the last inspection in developing a quality assurance system. The requirement is carried forward from the last inspection and a new timescale is given. See standard 30 for requirements made at the last inspection relating to fire safety training and movement and handling training for staff.
