What the care home does well

The home provides a homely atmosphere and the opportunity for service users to enjoy the sea views from the comfort of the conservatory at the home.

A requirement and recommendations from the previous report have been completed.

What the care home could do better:

A requirement from the previous report to ensure thorough recruitment procedures are completed has not been complied with. The inspection of records showed a current and serious deficiency in procedures designed to protect service users. An immediate requirement was made for the registered person to rectify this situation.

To include accommodation for two named persons only outside of the age range and other category of the home. 3rd March 2005 Date of last inspection Brief Description of the Service: Fistral House is a care home providing personal care and accommodation for up to thirteen older people. The home is located in a residential area of Newquay directly opposite Fistral beach. The home provides a conservatory for service users to enjoy the views. The home has all en suite (toilet and wash hand basin) bedrooms on the ground and first floor. There is a stair lift facility to all the bedrooms on the first floor. Communal accommodation is on the ground floor and consists of a lounge, dining room and conservatory. The home has a spacious level back garden and car parking is available at the front of the home. This was an announced inspection of care standards at Fistral House. I arrived at 10.30 am and the inspection finished at 4.30 pm. During the inspection I was able to discuss what it is like to live at the home with service users. Mrs Dowling and staff assisted me in reviewing records kept to monitor care at the home and discussing issues regarding the welfare of service users. I accepted an invitation to have lunch with service users who expressed approval of services received at the home. Mrs Dowling also completed a detailed pre inspection questionnaire to assist in the inspection process. What the service does well: What has improved since the last inspection? Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 1, 3 and 4. The registered person has provided a statement of purpose and service users guide that details the facilities and services available within the home. The registered provider will assess all prospective service users prior to admission to ensure that the home can meet their care needs. EVIDENCE: A statement of purpose is available within the home and a service users guide has been issued to each service user. These documents explain the services and facilities available within the home as part of the contract. The registered provider visits prospective service users prior to admission to complete an assessment process. These records include information received from other agencies to assist in appropriate admissions to the home. Prospective service users are invited to see the home and are involved in the completion of the dependency assessment documents. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 7, 8 and 9. The health care needs of service users are met with evidence of multi disciplinary working taking place on a regular basis. EVIDENCE: 3 plans of care were inspected and these appropriately referred to the various needs of service users. The care plans identify goals and actions required to meet individual care needs. In addition records are in place for hourly checks of the service users during the night. Monthly reviews are taking place to ensure that care needs are regularly reviewed and assessed. The homes accident book and procedures to report incidents in reference to regulation 37 are appropriately maintained. A range of pressure relieving equipment is provided for service users and liaison occurs with the community nursing service to provide for the welfare of service users. Service users are registered with local GP’s at Narrowcliffe and St. Thomas Road Surgeries. Appropriate medication procedures are maintained and staff training has been provided. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 12, 13 and 15. Service users exercise their choice to enjoy the privacy of their own rooms or join other service users in communal areas for company or meals. Service users maintain contact with visiting relatives and friends at the home. The meals provided in the home are good with special diets catered for. EVIDENCE: Menus inspected indicate that service users are offered a wide and varied choice of meals to include special diets. Teas offered in the home are basically what ever the service user would like. Home baking takes place regularly and service users spoke positively about the standard of the meals provided in the home. Service users bring various items of furniture and possessions to personalise their rooms. Service users expressed approval that visits are received from their relatives. The opportunity to meet visiting relatives did not occur but records of visits are maintained. Social care needs are identified in each service user care plan and the daily records evidence how individual social care needs are being met. Service users and their relatives and friends are confident that their complaints will be listened to, taken seriously and acted upon. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 16 and 18. The registered provider uses the policy and procedures at the home to assist in the protection of service users. EVIDENCE: The home has an adult protection policy and whistle blowing policy and procedure available. A recommendation that staff receive training in adult protection has been achieved. The commission has received a complaint from the relative of a service user that has now transferred to another care home. The registered person assisted in the investigation of issues raised and the relative has been informed that the complaint is not substantiated. Service users live in a safe, well-maintained environment. Service users have access to safe and comfortable indoor and outdoor communal facilities. Service users have sufficient and suitable lavatories and washing facilities. Service users have the specialist equipment they require to maximise their independence. Service users’ own rooms suit their needs. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 19, 20, 22, 23 and 26. The registered provider is providing a good standard of décor and maintenance to ensure service users live in a safe and comfortable environment. EVIDENCE: The home generally offers level access, stair lifts, pressure relieving equipment, call bell system, moving and handling aids and a variety of grab rails are provided. Various bedrooms have been redecorated and the gardens are well maintained. The heating is provided through radiators in each room with individual controls and protective guards. Appropriate ventilation and window restrictors in bedrooms are provided. The baths have mixing valves to control the supply of hot water. The home was observed to be clean and organised for the welfare of service users. There is adequate laundry equipment and staff are instructed to use disposable gloves, aprons and hand washing facilities provided. Service users’ needs are met by the numbers and skill mix of staff. Service users are in safe hands at all times. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 27, 28, 29 and 30. The registered person has failed to undertake appropriate recruitment or vetting procedures for the majority of the staff. This has potentially placed service users at risk. EVIDENCE: The registered provider has not taken appropriate employment references or completed Criminal Record Bureau checks on all staff employed. An Immediate Requirement Notice was issued to the registered provider regarding the action required and timescales for completion. Staffing rotas were inspected and evidence that two care staff members are on duty at all times. During the night one waking staff member is employed with the registered provider available on the premises. Staff training has been completed in Food Hygiene, Medication, Health and Safety, First Aid, Fire Precautions and other associated issues regarding the care of older persons. 2 senior carers are registered with the Registered Managers Award. A senior carer is an NVQ Assessor and other staff have completed NVQ training at level 2 and 3. A senior carer is to attend a course in October 2005 as a trainer for moving and handling instruction. During the inspection I observed staff exhibit appropriate skills and attitudes in their interactions with service users. The service users also expressed approval of the services received at the home. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 31, 32 and 38. The registered provider is qualified and experienced in care settings. The inspection showed that the registered provider is making progress in complying with requirements and recommendations from previous reports. EVIDENCE: The registered person is a qualified nurse and experienced in providing care. The registered persons son is the Head of Care, is registered to complete the RMA and was able to refer to appropriate policy and procedure regarding various topics. Risk assessments and the appropriate maintenance of health and safety precautions, training and equipment is occurring. The health, safety and welfare of service users and staff are promoted and protected. The provider is advised to continue to review standard 38 to ensure as far as is reasonably practicable the health, safety and welfare of service users and staff is maintained.
