Conditions of registration: Date of last inspection 26/10/04 Brief Description of the Service: Forder Lane House is a single storey building built in the 1980’s, and set within a complex of sheltered bungalows, also owned by Dartington Housing Association. It provides care for up to 14 older persons in the category of old age only. The accommodation is in single rooms, mainly en-suite, all with exterior doors to the gardens. There are adapted bathing facilities.A central area provides dining space and a small lounge area, with a conservatory leading off. There are level gardens surrounding the premises.There is a large external laundry, shared by the bungalows. The office is situated on the first floor, with the organisations administration. This is an overview of what the inspector found during the inspection. The inspection was announced and took place over one day on the 30th June 2005. A tour of the premises took place, and records were inspected. Eight service users were spoken with during the inspection. Staff on duty were observed in the course of their daily duties and 2 care staff were spoken with. Discussions took place with Mr Steven Prime, Chief Executive, Mrs Elizabeth Shinner, Registered Manager and Mrs Barbara Bevan, Deputy Care Manager. The intended outcomes for Standards 1 – 6 are: 1. 2. 3. 4. 5. 6. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 3,4,5 Prospective service users are provided information to assist them in choosing to live at Forder Lane House. The admission procedure ensures that service users needs are assessed and can be met EVIDENCE: Forder Lane House has a statement of purpose and service user guide which has been revised since the last inspection. The statement of purpose should include the number and sizes of rooms, including communal rooms. When new service users are admitted their needs are assessed prior to admission. The date of the pre-admission assessment should be recorded. The management is clear regarding the needs that they are able to meet. This is also included in the statement of purpose and service user guide. Several service users spoken with knew the Home prior to their admission through their own previous contact or via their relatives. The intended outcomes for Standards 7 – 11 are: 7. 8. 9. 10. 11. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 7,8,9, Service users have care plans, which are reviewed and enable their needs to be met. The systems for the administration of medication are satisfactory with arrangements in place to ensure that service users’ medication needs are met. EVIDENCE: Forder Lane House D54-D07 S3703 Forder Lane House V221810 300605 Stage 4.doc Version 1.20 Page 10 Evidence was seen of risk assessments and care plans, which are reviewed regularly. Not all care plans had been signed by the service user or their representative. A new care plan format has been introduced. While this states service users needs and how they are to be met, the format lacks detail, for example regarding service users interests and activities. Daily records showed that any concerns are recorded and addressed. Service users said that they are well are cared for and confirmed that their needs are met by staff who are accessible and attentive. Discussions with staff demonstrated their awareness of service users needs, including those of new service users. The systems for the administration of medication are satisfactory with arrangements in place to ensure that service users’ medication needs are met. Those staff who administer medicines have received accredited training. No service users currently administer their own medication, but should anyone wish to do so this is subject to risk assessment. Medicines are stored in a locked cupboard, but the controlled drugs cabinet is not appropriately secured to the wall. The Home does not have a medicines refrigerator. Forder Lane House maintains good working relationships with the G.P practices and the District Nursing Service, which was seen demonstrated during the inspection. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 12,13,15 Forder Lane House provides a resource for local older people who require care. Service users contact with the local community is encouraged and visitors are welcomed. A balanced and varied selection of food is available that meets service users tastes and choices. EVIDENCE: Routines within the home are flexible to ensure that residents can choose how they spend their time. Many service users spend time in their own rooms enjoying their interests ,but like informal time spent talking with staff. Activities are provided such as whist, bingo and an exercise class, to which to residents of the neighbouring sheltered bungalows are invited. There is an open visiting policy and it was confirmed that visitors are made welcome. There is a varied rotating menu. All service users spoken with praised the quality and quantity of the meals. Every effort is made to meet service users dietary requirements and choices. .

Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 16,18 Service users can be confident that procedures are in place to enable them to complain and to protect them from abuse. EVIDENCE: The complaints procedure is displayed so that it is accessible to service users. No complaints have been received since the last inspection. There is an adult protection policy and procedure which is accessible to staff. All staff should receive training in the protection of vulnerable adults. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 19.23,26 Service users are provided with accommodation that is safe, warm, comfortable, attractive and clean. EVIDENCE: Forder Lane House is very well maintained and furnished to a good standard. There is an ongoing programme of redecoration and renewal that ensures that the environment remains clean and attractive. Since the last inspection new carpets have been fitted in 3 service users rooms and new furniture has been provided in the conservatory. All service users rooms are situated on the ground floor and have individual access to level gardens and have an attractive outlook. Service users rooms are personalised to their choice and have en suite facilities or a designated toilet. There are adapted bathing facilities and ramps and environmental adaptations to meet service users needs. There is an infection control policy and all areas of the home were hygienic and odour free. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission considers Standards 27, 29, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 27,30 Staff are employed in sufficient numbers to meet the current service users needs. Staff are provided with training opportunities to enable them to develop their knowledge and skills in caring for service users. EVIDENCE: There is a good range of experience within the staff group. Several staff have worked at Forder Lane House for a number of years. In the past year all staff have received mandatory training in fire safety, manual handling and infection control. In addition some staff have received first aid training and training in the safe handling of medicines. While only one of the 13 care staff currently has NVQ 2, four are currently undertaking the training. Staff confirmed that they are encouraged to undertake training. They felt well supported by the management and said new staff are provided with an induction to enable them to do their work. The registered manager said that there are sufficient staff employed to meet service users needs by day and by night. Staffing levels are kept under review dependent on the needs of the service users. The staff rota was available for inspection. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 32,35,38 Forder Lane House is well managed with efficient systems in place. Service users live in an environment where health and safety standards are met and maintained. EVIDENCE: The management ensure that there is an open, positive and inclusive atmosphere within Forder Lane House. This was evident during the inspection from the interaction of service users with the managers and with staff. Service users spoke highly of the quality of care they receive and their confidence in the management. The registered manager intends completing the registered managers award in the near future. The deputy manager is undertaking NVQ 4 in Care. The Home’s quality monitoring systems are to be reviewed and revised. Routine health and safety issues are managed satisfactorily and records are maintained up to date and accurate. Fire and accident records were in order.
