Registered for 5 Service Users with Learning Disabilities some or all of whom may be over 65 27th April 2005 Date of last inspection Brief Description of the Service: Greenacres is a care home registered by St Annes Community services to provide personal care and accommodation to up to five adults with learning disabilities, some or all of who may be aged over 65 years. The home consists of a detached dormer bungalow located on a busy road on the outskirts of the market town of Ripon, community facilities including shops and cafes are within walking distance. Each of the five bedrooms is for single accommodation, two of which have en-suite facilities. Four of these are situated on the ground floor, the fifth is in the first floor. Whilst the home does not have a passenger or stair lift, all areas are accessible to those residents currently living there. There are well-maintained garden areas to the front and rear of the home with level access to the rear and hard standing for parking to the front. This is an overview of what the inspector found during the inspection. This report relates to an unannounced inspection carried out on the 18th January 2006, which started at 11.00am until 12.00pm.There were two service users in at the time. A return visit on the same day was carried out between 15.00 hrs until 16.30 hrs as two other service users had arrived home. The registered manager was also on duty that afternoon A tour of the home was carried out which included two of the service users private accommodation, a selection of records were looked at and time was spent observing activity in the home, talking and listening to service users and staff. The focus of the inspection was a number of key standards. There were also discussions with staff and the manager of the home. What the service does well: What has improved since the last inspection?
The home has encouraged and supported the staff team regarding obtaining their NVQ qualifications. 50 of staff is qualified to NVQ Level 2 or above. The organisation has obtained the views of health and social care professionals and other stakeholders through their quality assurance systems. Advice was given to the registered manager that copies of the outcome to people’s views should be held by the home as evidence that this has been carried out. The intended outcomes for Standards 1 – 5 are: 1. 2. 3. 4. 5. Prospective service users have the information they need to make an informed choice about where to live. Prospective users’ individual aspirations and needs are assessed. Prospective service users know that the home that they will choose will meet their needs and aspirations. Prospective service users have an opportunity to visit and to “test drive” the home. Each service user has an individual written contract or statement of terms and conditions with the home. The Commission consider Standard 2 the key standard to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 1 and 2 Prospective service users can be confident before moving into the home that their needs can be met. EVIDENCE: No new resident has been admitted to the home for a number of years. Appropriate admission procedures continue to be in place however, which when followed would ensure that a prospective resident’s needs would be fully assessed before them being admitted and that a programmed introduction to the home would be arranged. Service users make decisions about their lives with assistance as needed. Service users are consulted on, and participate in, all aspects of life in the home. Service users are supported to take risks as part of an independent lifestyle. Service users know that information about them is handled appropriately, and that their confidences are kept. The Commission considers Standards 6, 7 and 9 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 9 and 10 Service users are supported in making decisions about their personal lives. EVIDENCE: The five service users living at Greenacres have lived together for sometime. It was evident throughout the time spent in the home that service users are supported in making decisions including risk taking as part of supporting their independence. One service user enjoys catching the bus independently into the centre of Ripon and stated that he would miss this if ever he would have to move. Two other service users had been out to day centres. Service users are offered a healthy diet and enjoy their meals and mealtimes. The Commission considers Standards 12, 13, 15, 16 and 17 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 17 Service users dietary needs are met. EVIDENCE: Service users confirmed that they are involved in choosing the menus and are offered a choice of food at each meal. The lunchtime meal is usually a snack. The main meal is in the evening as it is generally eaten when all the service users are home from activities and day centres. Service users said that the meals at Greenacres are very good. The intended outcomes for Standards 18 - 21 are: 18. 19. 20. 21. Service users receive personal support in the way they prefer and require. Service users’ physical and emotional health needs are met. Service users retain, administer and control their own medication where appropriate, and are protected by the home’s policies and procedures for dealing with medicines. The ageing, illness and death of a service user are handled with respect and as the individual would wish. The Commission considers Standards 18, 19, and 20 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 18, 19 and 21 Service users health and personal care needs are well met. EVIDENCE: Each person living in the home is registered with a GP through whom specialist health services are accessed if required. Staff support them in having regular health checks and checks with their dentist and optician. Several residents said that they were happy with these arrangements. Service users are supported as and when required with personal care, as one service user discussed how they were supported when bathing. Discussions were held regarding the ageing process of service users living at Greenacres. The staff and the registered manager were able to demonstrate their knowledge and understanding, as to the needs of older people with learning disabilities. The Commission considers Standards 22, and 23 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 23 Service users are listened to and protected. EVIDENCE: The organisations policies and procedures are in place regarding the protection of vulnerable adults. The local adult protection policy is also in place. Service users confirmed that if they had any concerns that they would approach staff on duty or wait to see the registered manager it would then be acted upon. Shared spaces complement and supplement service users’ individual rooms. Service users have the specialist equipment they require to maximise their independence. The home is clean and hygienic. The Commission considers Standards 24, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 25, 26 and 30 The environment of the home is maintained to a good standard and provides service users with a clean and homely place in which to live. EVIDENCE: Two service users bedrooms were inspected. Both had been furnished to a good standard. All areas of the home were warm, well lit, ventilated and clean. Three bedrooms have had new radiators fitted. However these rooms are in need of being redecorated. This work has been programmed to be completed before the end of March 2006. There is also some outstanding work remaining regarding the storage of the hot water this work is also to be completed by the end of March 2006. JUDGEMENT – we looked at outcomes for the following standard(s): 34 Service users are protected by the home’s vigorous recruitment procedures. EVIDENCE: Two staff files were inspected. Staff records showed that the registered manager undertakes thorough checks before staff commence working at the home. CRB checks have been carried out and staff had been checked against the POVA first list. All staff complete application forms. Two written references are obtained. Staff files also contained records of supervision. Copies of training certificates are also held on each individual staff file. JUDGEMENT – we looked at outcomes for the following standard(s): 37 and 42 Service users’ health safety and welfare are promoted and protected. EVIDENCE: Throughout the day from discussions held with service users and staff and through observation, Greenacres continues to be managed well with a committed staff team. The organisations health and safety policies and procedures are in place.
