Conditions of registration: Date of last inspection 8th December 2005 Brief Description of the Service: High Lee Barn is a privately owned and family run residential care home registered to provide accommodation and care for up to twelve older people. The establishment is situated in a rural hill top location with panoramic views across the surrounding countryside. The property, a converted barn, is well maintained throughout and furnished and fitted to a high standard. The accommodation is arranged on two floors and provides ten single bedrooms and one shared room, all with the benefit of en-suite facilities. Well maintained garden areas are provided for the use of the service users. The establishment places a high priority on staff training which is clearly reflected in the standard of care afforded to the service users. A range of activities are provided for the service users to participate in, if they choose to do so, and service users are encouraged to maintain links with the local community. The current weekly charge at High Lee Barn is £450. Additional charges are made for hairdressing, personal toiletries and personal newspapers and periodicals. This is an overview of what the inspector found during the inspection. This report brings together evidence gathered during a Key Inspection. This included an unannounced visit to High Lee Barn on 7th November 2006 by one inspector over a period of 7 hours. During this visit discussions were held with service users, relatives and staff, records were examined and all areas of the home were seen. In addition to this visit comment cards were sent out to relatives/visitors, to give people an opportunity to share their views of the service with CSCI. Six replies were received. A pre inspection questionnaire was returned by the home’s manager which contained useful information. The last inspection was on 8th December 2005, and no additional visits to the home have been made. In April 2006 the Commission for Social Care Inspection (CSCI) made some changes to the way in which care services are inspected. Care services are now judged against outcome groups. The inspection report is divided into separate sections for each outcome group for example Choice of Home. An overall judgement is made for each outcome group based on the findings of the inspection. The judgements reflect how well the service delivers outcomes to the people using the service. The judgements categories are “excellent”, “good”, “adequate” and “poor”. The home has a relaxed and welcoming atmosphere. Service users and visiting relatives spoken to during the inspection confirmed that this is always the case. All service users have an assessment prior to moving into the home, and are encouraged to visit, several times if they wish, before deciding to move in. The staff appeared to be working together as a team and meeting the needs of the service users in a sensitive and dignified manner, with appropriate use of

Staffing levels are sufficiently high to enable impromptu activities and outings to take place to enable service users to maintain contact with the local community. Service users described the food as “very good” with their preferences being taken into account. The home is well maintained throughout and service users expressed satisfaction with the communal areas and their own rooms. Service users and visitors confirmed that standards of cleanliness and hygiene are always of a high standard. All six of the comment cards received showed that they were satisfied with the overall care provided by the home. What has improved since the last inspection? What they could do better:

Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 3,5,6 Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. All service users have a pre admission assessment to make sure the home can meet their needs. Service users are provided with a written contract stating the terms and conditions provided within the home. EVIDENCE: Each service user has a contract of residence which details the rights and responsibilities of each party and what is and is not included in the weekly charge of £450. Prospective service users are encouraged to visit the home before deciding if they want to move in. The home has revised it’s statement of purpose and service user guide, which gives up to date information about the services and facilities provided.. The home is in the process of setting up a website and this document will shortly be available on line. Intermediate care is not provided within the home. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 7,8,9,10 Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. Service users health and personal care needs are fully assessed and recorded in their plan of care, which means they will receive consistent care. Service users are protected by the medication practices within the home. EVIDENCE: All service users have an individual care plan.. The care plans seen contained detailed information covering all aspects of health and personal care and activities of daily living. The care plans contained risk assessments for moving and handling, falls, tissue viability (risk of pressure damage to the skin) and nutrition. The care plans are reviewed on a monthly basis. A daily record is kept for each resident which clearly shows how they have spent their time.

One service user described being treated “very well and they can’t do enough for you”. Medication practices within the home are generally safe. The medication is securely and appropriately stored. The medication is supplied in individual cassettes from a local pharmacy. Stocks of medication held reconciled with records kept. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 12,13,14,15 Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. The routines within the home are flexible giving service users choice and control over their lives. Service users enjoy a varied and nutritious diet. EVIDENCE: Service users preferences regarding daily routines are recorded and staff are flexible in the support they give to make sure these choices are respected. Residents were able to choose where and with whom they spend their time. A range of activities are available for the service users to join in with if they choose to do so. The high staffing levels and relaxed routine means that spontaneous outings to take place which helps the service users to maintain contact with the local community. During the inspection staff were spending

Service users said they liked the meals the served at the home, and that their personal preferences were taken into account. The main meal of the day is served at lunchtime, and on the day of the inspection this was: roast chicken, broccoli, carrots and cauliflower, followed by chocolate sponge. A record is now kept of all meals eaten by the service which makes it possible to check that the residents receive a varied and nutritious diet. Relatives said that they were always made to feel welcome when they visit the home and are kept informed of any issues affecting their relatives’ welfare. All of the respondents to the relative/visitor comment cards indicated that they are welcomed into the home, with an addition comment made which was “I feel that my mother receives excellent care in a warm and welcoming environment”. One visitor spoken to during the visit said “I am very satisfied with the care provided”, and another said “You won’t find anything wrong here, it’s always warm and always clean, and very friendly”. JUDGEMENT – we looked at outcomes for the following standard(s): 16,18 Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. The home has a complaints procedure which relatives and relatives know about. All staff are aware of Protection of Vulnerable Adults policies and procedures. EVIDENCE: The home has a complaints procedure a copy of which is in the service user guide. All of the returned comment cards showed that relatives and visitors were aware of the home’s complaints procedure, however, none had any occasion to make a complaint. All staff have received training in the protection of vulnerable adults and staff showed an understanding of the procedure to follow if abuse was reported or suspected. JUDGEMENT – we looked at outcomes for the following standard(s): 19,20,21,23,24,25,26 Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. Service users live in a safe, clean and well maintained home. EVIDENCE: The home is in a rural hilltop location with panoramic views over the surrounding countryside. It is well maintained both internally and externally, with a routine programme of refurbishment and redecoration. A new kitchen has recently been installed and there are plans for total refurbishment of the home. Safe, accessible and well maintained garden areas are provided for the use of the service users. Bedrooms are highly personalised reflecting the individual’s interests and tastes. There are spacious and comfortable lounge and dining areas. All parts of the home were seen to be clean and hygienic. Visiting relatives confirmed that this was always the case. Service users have access to all parts of the building by the provision of a stair lift. The laundry facilities are well equipped and comply with the regulations. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 27,28,29,30 Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. The home is adequately staffed by a competent, well trained and motivated workforce. Service users are protected by the home’s recruitment procedure. EVIDENCE: Staffing levels are sufficient to meet the needs of the service users. This is a family run home with relatives of the owner providing most of the care. The staff work flexibly to make sure that the service user’s social and emotional needs are met. The duty rota now includes the full name of staff and the hours that they actually work, so that there is an accurate record of who is working in the home at all times. Staff turnover and there have been no new employees since the last inspection. All staff working at the home have had Criminal Records Bureau (CRB) and Protection of Vulnerable Adults (POVA) checks. Additional training planned includes Protection of Vulnerable Adults refresher, moving and handling, fire training and first aid. The returned comment cards showed that relatives and visitors felt there were always sufficient numbers of staff on duty, with additional comments made “we are very satisfied with the care provided by the competent staff”, and “the staff can’t do enough for you”. The staff team were observed to be meeting the service users’ needs in a sensitive manner with appropriate use of informality and humour. Service users’ expressed satisfaction with the staff team as a whole, and made the following comments: “they can’t do enough for you”, “they are all very kind”, and “he’s a smasher”. JUDGEMENT – we looked at outcomes for the following standard(s): 31,32,33,35,38 Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. The home is well run and managed by an experienced and qualified management team. Service users’ health, safety and wellbeing is protected by the homes policies and procedures. EVIDENCE: The management team are committed to ensuring an open and positive atmosphere is maintained within the home. The Registered Manager is

She has completed NVQ IV Registered Managers Award, and demonstrates a clear sense of leadership. An application has been submitted to the Commission for Social Care Inspection for the assistant manager to be registered as manager of the home, he too is qualified and experienced. The central registration team are currently processing the application. Health and safety policies and procedures are in place and all staff have received training in safe working practices. A weekly health and safety audit is carried out and recorded.. There is a quality monitoring system in place with feedback actively sought from service users, visitors and health professionals.
