Conditions of registration: None Date of last inspection 7/3/05 Brief Description of the Service: High Lee Barn is a privately owned and family run residential care home registered to provide accommodation and care for up to twelve older people. The establishment is situated in a rural hill top location with panoramic views across the surrounding countryside. The property, a converted barn, is well maintained throughout and furnished and fitted to a high standard. The accommodation is arranged on two floors and provides ten single bedrooms and one shared room, all with the benefit of en-suite facilities. Well maintained garden areas are provided for the use of the service users. The establishment places a high priority on staff training which is clearly reflected in the standard of care afforded to the service users. A range of activities are provided for the service users to participate in, if they choose to do so, and service users are encouraged to maintain links with the local community. This is an overview of what the inspector found during the inspection. This inspection took place on 8th August 2005. Three service users and one member of staff were consulted as to their views on the standard of services and facilities provided within the establishment. The residents spoken to all expressed great satisfaction with the facilities and care they receive at High Lee Barn. What the service does well: What has improved since the last inspection?

The intended outcomes for Standards 1 – 6 are: 1. 2. 3. 4. 5. 6. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 1,2,6 Service users are provided with a written contract stating the terms and conditions provided within the home. EVIDENCE: The establishment holds a statement of purpose and service user guide which contains all of the information as required in Schedule I of the Care Homes Regulations. The document, however, was not particularly easy to read and rather muddled and would benefit from being re-organised. Each service user has a contract of residence which details the rights and responsibilities of each party and what is and is not included in the weekly charge of £450. The intended outcomes for Standards 7 – 11 are: 7. 8. 9. 10. 11. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 7,8 Service users needs are assessed and the home has a good approach to promoting the service users health care. EVIDENCE: Each service user has an individual plan of care generated from the comprehensive assessment process. The care plan is reviewed on a weekly basis and ensures that the service users physical and emotional needs are met. Daily recordings are detailed and clearly link to the needs identified in the care plan. All personal and health care tasks are performed in private with privacy and dignity promoted at all times. Service users health care needs are addressed at the initial assessment and for part of their plan of care. All residents have a medical assessment by their G.P every six months, with optical, chiropody and dental treatment being accessed as required. Tissue viability is assessed by the district nursing team and no service user is currently at risk of developing pressure sores. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 12,13 The lifestyles of the service users match their expectations and preferences. Service users are encouraged to maintain contact with family and friends, and they are made to feel welcome. EVIDENCE: Service users preferences regarding their daily living activities are assessed and flexible support given to enable them to follow their preferred routines. Residents were observed to be exercising choice in where and with whom to spend their time. A range of activities are available for the service users to participate in if they choose to do so. The high staffing levels and relaxed routine of the establishment enables spontaneous outings to take place which enables the service users to maintain contact with the local community. Several of the service users regularly visit a local church and join in the social activities provided there. JUDGEMENT – we looked at outcomes for standard(s) 16,18 The home has robust systems to ensure that service users are safeguarded from abuse and that complaints are dealt with promptly. EVIDENCE: The home holds a complaints procedure which is freely available to the service users. All staff have received training in adult protection issues. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 19,20,23,24,25,26 Service users live in a safe, clean and well maintained environment. EVIDENCE: The establishment is situated in a rural hilltop location with panoramic views over the surrounding countryside. The property, a stone built converted barn, is well maintained both internally and externally, with a routine programme of refurbishment and redecoration. Safe, accessible and well maintained garden areas were being enjoyed by some service users at the time of the inspection. Service users private accommodation consists of ten single bedrooms and one shared room. All bedrooms have the benefit of en-suite facilities and are furnished and fitted to a good standard. JUDGEMENT – we looked at outcomes for standard(s) 27,30 The home is adequately staffed by a competent and motivated workforce. EVIDENCE: From records examined and following observations and discussions at the time of the inspection sufficient staff are deployed to meet the needs of the service users. Staff rota’s examined indicated that although the names of the staff working are written down the actual hours worked are not, in addition, the rota was not dated. The establishment is mainly staffed by members of the family and deployed flexibly to meet the social and emotional needs of the service users. There is a commitment to NVQ training within the home. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. JUDGEMENT – we looked at outcomes for standard(s) 31, 32 The home is well run and managed by an experienced and qualified management team. EVIDENCE: The current Registered Manager of the establishment, Mrs Pat Beaumont is qualified and experienced to run the home. She has completed her NVQ IV Registered Managers Award, and demonstrates a clear sense of leadership. Mrs Beaumont has indicated that she wishes Mr Simeon Beaumont to be registered as manager of the establishment and will submit an application to the CSCI for registration.
