Physical Disability - Code PD, maximum number of places: 5 The maximum number of service users who can be accommodated is: 61 Date of last inspection Brief Description of the Service: Hundens Park is a care home providing both nursing and personal care to sixty one older people. The home is located In a residential area on the outskirts of Darlington. The home is on a regular bus route into the centre of Darlington. Accommodation is provided over two floors that are serviced by a lift with stair access as well. It is a purpose built home with spacious communal areas with all bedrooms having en suite facilities. Assisted bathrooms and showers are available on both floors. The home is set in pleasant grounds and the first floor has a balcony area for people to use if they wish to sit outside. Barchester Health Care owns the home. Fees range from £334.00 to £383.40 per week; this does not include hairdressing, newspapers or toiletries. This is an overview of what the inspector found during the inspection. This inspection was carried out over a period of 7 hours on the 3rd and 4th of July 2007. The home did not know the inspection was going to take place. The plan for the inspection was to check whether the home had implemented the requirements and recommendations made at the previous inspection; to talk with the people about living in the home; to meet with care staff and the home’s management team; and to look at records. The manager had completed a questionnaire about the home prior to this visit. Also people who use or who are involved with the service had also returned some survey forms. Some of this information has been included within this report. What the service does well: What has improved since the last inspection?
The manager and staff team have worked very hard since the last inspection and improvements have been made in several areas. The recording in care plans is clearer and staff have written guidance on how to meet people diverse and individual needs. Risk assessments are contained in peoples plans of care and give good clear directives to staff on how to risk manage people in there care. Information held about staff and the way in which staff training is recorded has been improved and up to date records are kept on staff supervision sessions that are now being held on a regular basis. Several areas of the home have been redecorated and refurbished. The garden at the home was lovely. New seating has been placed in a secluded area at the back of the home and there is also a fountain and pond. The intended outcomes for Standards 1 – 6 are: 1. 2. 3. 4. 5. 6. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 1,2,3&6 Quality in this outcome area is good. People can be assured that their needs are appropriately assessed prior to admission to the home and that they are given sufficient information to make an informed choice before moving into the home. This judgement has been made using available evidence including a visit to this service. EVIDENCE: The home’s Statement of Purpose and Service User Guide have been updated. These guides contain information that is required to enable people to make an informed choice about where to live. Evidence was seen that service users or their representatives have signed a contract with the home. The contracts were detailed and included a breakdown of the fees and who was responsible for paying them. The manager confirmed that a visit to prospective clients always takes place before their admission to the home, to carry out an assessment of needs. Also, each care plan looked at contained assessments from the placing authority that were responsible for commissioning people’s care at the home. 75 of the surveys returned stated that they had been given sufficient information about the home before moving into it. One relative said ‘we were made to feel very welcome when we came to look around. We were introduced to the staff on duty and felt very at ease in the place, I had no doubt this was a good care home’. The home does not provide intermediate care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 7,8,9,10 Quality in this outcome area is good. Good systems are in place to ensure that health care needs of the people are met. People can be confident that their privacy and dignity is protected and that they are treated with respect. This judgement has been made using available evidence including a visit to this service. EVIDENCE: A new system for recording how people are cared for has just commenced at the home. The new care plans gave clear instructions to staff on how to meet people’s diverse needs. During the inspection people said they were aware of the documentation used regarding how care was given, most of the care plans had been signed and agreed either by the person receiving the care or else by a relative or representative. The plans need to be kept under constant review by all staff and this must be monitored by the manager. During the inspection several district nurses visited the home, and care plans indicated that people had access to other health care professionals such as the community psychiatric nurse, opticians and chiropodists. One person wrote ‘my G.P. has attended at my request for simple problems’

Stock checked against the balance on the medication record was correct. Policies and procedures were in place regarding medication, the staff on duty were knowledgeable about these. Staff were seen to be treating people with respect and dignity and this was also reflected within the care plans. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 12,13,14,15 Quality in this outcome area is good. The recreational and social needs of people are well catered for which enables people to make daily choices and promotes independence. This judgement has been made using available evidence including a visit to this service. EVIDENCE: There was good practice relating to the promotion of independence and social contact with people outside the home. An activities organiser is employed at the home and there was a varied range of activities offered. People were able choose whether or not they wish to be involved. Trips to local pubs for lunch are organised, outside entertainers visit the home and craft activities also take place. One person explained that they had been out the previous evening to the local bingo hall and had a really good time. Another person said they had really enjoyed making lavender bags for the summer fate. Several people commented on the food during the inspection and said that the food was good. Observation of the lunchtime meal provided evidence that people were able to make choices about the meals they wished to eat. One service user said the food was ‘great’ another said ‘you get a nice choice of meals, you can have a glass of wine or beer as well. I’m not easy to please where food is concerned, but this is top notch’

JUDGEMENT – we looked at outcomes for the following standard(s): 16&18 Quality in this outcome area is good. Service users can be confidant that their concerns and complaints are dealt with appropriately and that safeguards are in place to protect them from abuse. This judgement has been made using available evidence including a visit to this service. EVIDENCE: There are adequate written policies and procedures in place to deal with complaints and the care staff spoken to confirm they were aware of these. Staff knowledge of these help ensure that they were able to address any issues or anxieties of the residents, relatives and visitors to the home. One person wrote ‘ I have never, to date been unhappy about anything in the home, but I do know whom to approach if I needed to complain’ Staff training has taken place in the protection of vulnerable adults in abuse. Staff recruitment procedures were adequate and staff were employed and deployed following appropriate CRB and POVA checks. JUDGEMENT – we looked at outcomes for the following standard(s): 19&26 Quality in this outcome area is good The home was clean, comfortable and well-maintained providing service users with a safe and pleasant environment This judgement has been made using available evidence including a visit to this service. EVIDENCE: At the inspection the home was nice and clean and fresh. The communal areas were bright, spacious and nicely decorated. One person wrote, ‘I am very lucky to be living in such a nice pleasant home’. Another said ’I like to see the fresh flowers even the small vases in the dining room make the place look really nice’. Staff had recently planted patio tubs with summer plants, and transformed the area at the back of the home into a secluded seating area with new patio furniture and sun umbrellas. Service user can be confident that staff are trained and on duty in sufficient numbers to meet their assessed needs. This judgement has been made using available evidence including a visit to this service. EVIDENCE: Staff training records were being updated. There was evidence of on going training for staff in such things as moving and handling of service users, fire awareness and prevention of falls, and the protection of vulnerable adults. The staff have available a training computer which they can access for training information. External training in care is also sourced for staff. All trained nurses are supported in keeping up dated with current nursing practice, in accordance with the Nursing and Midwifery Council. From the rota supplied at the inspection and from information in pre inspection questionnaire there was sufficient care staff on duty to meet the assessed needs of the people who were using the service. People said that staff were always around and answered the call bells quickly. One person said ‘the staff are kind and helpful’. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. JUDGEMENT – we looked at outcomes for the following standard(s): 31,33,35 &38 Quality in this outcome area is good. Service users can be confident that the home is well managed. Systems and safeguards are in place to ensure the health, safety and welfare of service users and staff is protected. This judgement has been made using available evidence including a visit to this service. EVIDENCE: The manager of the home is experienced in managing a care service. There was an open, friendly culture between the management team and the staff at the home, and staff said they felt very well supported in their work. People said that the manager was very approachable and they would go to her or any of the staff if they had any concerns. A service satisfaction questionnaire is also used within the home, some of these are sent to service users families and visitors, the results of these are available at the home.

All transactions are recorded correctly and receipts are kept. The homes health and safety file was examined; all equipment in the home is regularly checked with valid certificates issued.
