Conditions of registration: None Date of last inspection 7th December 2004 Brief Description of the Service: 4 Ivyhurst is a bungalow property in a quiet cul-de-sac in the Aigburth area of Liverpool. It provides accommodation and personal support for two younger persons in single bedrooms. The home is staffed throughout the day with two members of staff and a sleep-in staff at night. There are a number of aids to promote the safety and comfort for its residents. It is easily accessible by public transport the home has a mini bus to enable the residents to access community facilities. This is an overview of what the inspector found during the inspection. This was an unannounced inspection. On the day of the inspection both residents were at home and two staff were on duty. The inspection lasted for two hours during which time the inspector spoke to staff, observed interaction of residents with staff, inspection of the house, activities offered and inspection of resident’s file, staff records, fire and accident book and other information which is required to be kept in the care home. The home focuses on meeting the needs of the individual residents with a strong emphasis on the residents accessing community facilities and educational opportunities at the local college. The residents benefit from a staff group that has a low staff turnover and sickness level. Thus enabling the staff to develop a relationship and understanding of the residents’ needs since they only able to communicate their feelings and wishes through non-verbal communication. The residents are supported in maintaining contact with their family and have holidays each year, which is included in their weekly fee. The home provides detailed care plans and risk assessments, which are regularly review to ensure that the resident’s assessed needs are met and advice is sought from specialist health professionals to ensure the health and safety of the residents. Prospective users’ individual aspirations and needs are assessed. Prospective service users’ know that the home that they will choose will meet their needs and aspirations. Prospective service users have an opportunity to visit and to “test drive” the home. Each service user has an individual written contract or statement of terms and conditions with the home. The Commission consider Standard 2 the key standard to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 1,2,3,4,5 The home would make their own assessment of the resident needs to ensure that they would be able to provide the necessary care and support to the resident. EVIDENCE: The home has a detailed Statement of Purpose setting out the services provided at 4 Ivyhurst Close and details of their admission process. The home has not had a new admission for many years. Service users know that information about them is handled appropriately, and that their confidences are kept The Commission considers Standards 6, 7 and 9 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 6,7,8,9,10, Each resident has a detailed care plans and risk assessments to promote their physical and emotional well- being. EVIDENCE: Each resident has detail care plans, protocols and risk assessments showing clearly how their assessed needs would be met. These are reviewed at regular intervals with other health professionals. In addition staff training is linked to the resident’s needs. Service users are offered a healthy diet and enjoy their meals and mealtimes. The Commission considers Standards 12, 13, 15, 16 and 17 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 11,12,13,14,15,16,17 The staff supports the residents to access community services and training opportunities at local colleges to develop skills to improve their quality of life. EVIDENCE: The staff at the home supports the residents to access various community facilities as evidenced in the resident’s weekly activity planner. This includes outings to local shops, restaurants, markets and attendance at the local college one day a week. Currently, one resident attends relaxation class and the other attends a course on communicating through the use of pictures. The resident’s records and photographs displayed in the resident’s bedroom show that the staff supports the resident to maintain contact with family and friends. Observation during the inspection showed that the residents are able to choose to be on their own. The staff in the home have a good rapport with the residents and have learnt over the years some of the residents’ nonverbal communication. The staffing levels in the home enable the resident to be assisted at mealtimes. The home provides special diets as required and staff are trained to provide support to residents that require feeding through artificial method as evidenced in resident’s record and staff training records.

The intended outcomes for Standards 18 - 21 are: 18. 19. 20. 21. Service users receive personal support in the way they prefer and require. Service users’ physical and emotional health needs are met. Service users retain, administer and control their own medication where appropriate, and are protected by the home’s policies and procedures for dealing with medicines. The ageing, illness and death of a service user are handled with respect and as the individual would wish. The Commission considers Standards 18, 19, and 20 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 18,19,20, The staff at the care home provides personal care and assistance to residents in a manner that promote their respect and dignity. EVIDENCE: Observation during the inspection showed that the residents are supported with personal care in the privacy of their of their bedroom, where there are specialist equipment such as “high low” bed, bathing equipment specialist chairs etc to promote the residents safety and well being. Discussion with staff and entries in the resident’s records show that they receive regular health checks from the speech therapist, continence adviser, GP. Etc. The staff administered all of the resident’s medication. The Commission considers Standards 22, and 23 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 22 ,23 The home’s policies and procedures protect the residents, as they are vulnerable due to their disability. EVIDENCE: The home has a complaints procedure and various policies and procedures in place to protect the residents from all forms of abuse. Discussion with staff indicated that they are aware of the differing forms of abuse and are clear on the procedure they need to follow in the event of any allege abuse. The home provides a safe storage for the residents’ monies and records are kept of financial transactions. The Commission considers Standards 24, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 24,25,26,27,28,29,30,31 The home is well maintained and provides a “homely”, comfortable and safe environment for the residents. EVIDENCE: The home is well maintained both internally and externally. A tour of the house showed that the residents bedrooms are personalised with items that reflect their taste and preferences with items such as; photographs, music centres etc. Each resident is assessed by an occupational therapist for aids to promote their safety and well being, as evidenced by equipment in the resident’s bedroom and the bathroom. All equipment in the home is regularly serviced as required by the manufacturer. The communal area is bright and spacious and allows residents easy movement around the room in their wheel chair or other specialist chair. Service users benefit from well supported and supervised staff. The Commission considers Standards 35 the key standard to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 31,32,33,34,35,36 The staff at the care home are provided with the necessary training and support to ensure that the residents’ needs are met. EVIDENCE: The staffing level at the care home reflects the high level of dependency of the residents. The staff have the time to do uninterrupted work with the residents as evidenced in the resident’s individual activity programme. The resident benefit from a staff group that has low staff turnover, sickness and a commitment to ensure that they receive continuity of care from staff who are aware of their likes and dislikes. The staff on duty commented that they enjoyed working at the care home. This was evidenced through inspection of the staffing rota and discussion with the staff on duty. Many of the staff have achieved their NVQ level 2 in Care or are working towards achieving this qualification. The staff on duty commented that they enjoyed working at the care home. The staff receives regular individual supervision from the manager as evidenced in staff files and discussion with the staff. Service users benefit from competent and accountable management of the service. The Commission considers Standards 39, and 42 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 37,38,39,40,41,42,43 The manager of the home provides strong leadership to promote the interests of the residents in an environment where risks are minimised to ensure the health and safety of the residents. EVIDENCE: The registered manager has recently completed her NVQ level 4 Management Award. Discussion with the staff indicated the manager provides strong leadership and direction through regular staff meetings, supervision and being accessible at all times The policies and procedures of the home is develop centrally by Community Integrated Care. The home has a quality assurance system in place to monitor the quality of care provided to the residents. The manager of the home ensures that staff training in food hygiene, fire awareness, moving and handling and first aid are up to date. Discussion with the staff and training records show that staff have attended training in recent months. Regular fire checks, hot water checks are undertaken and a record is
