Conditions of registration: Date of last inspection 24th March 2006 Brief Description of the Service: Keer Sands is registered with the Commission for Social Care Inspection to provide care and accommodation for up to ten older people. The home is situated in a rural area of Carnforth but only a short drive from the many facilities and amenities available in Lancaster and Morecambe. The majority of accommodation at the home is offered on a single room basis and there is one shared room. There are toilet and bathroom facilities on both floors and a variety of communal areas including a lounge, dining room and conservatory. Prior to their admission, assessments are carried out of residents’ daily care needs to ensure the home can offer an appropriate level of care. Information gathered at the time of the assessment is used to form a care plan, which is regularly reviewed throughout the service user’s stay at the home. Care is provided on a 24 hour basis including waking watch cover throughout the night. Several of the carers employed at Keer Sands have NVQ qualifications in care at level 2 or above. Residents’ health care needs are addressed in conjunction with visiting health care professionals such as district nurses and general practitioners. There are a variety of social activities offered at the home including home based activities and trips and outings. The registered provider of the home is Mrs Anne Plevey. Mrs Plevey works in the home on a full time basis. This is an overview of what the inspector found during the inspection. This home has been inspected against the National Minimum Standards for older people introduced in April 2002. The residents (a term used to collectively describe the people who live in the home) their relatives and visiting health professionals were invited to take part in a written survey. A total of 34 questionnaires were received, 8 from the people receiving care, 10 from relatives and 4 from health professionals. All the surveys returned were very praiseworthy about all aspects of care at Keer Sands. Comments included: “Everything is excellent” “We get excellent care” “No need for any concern – everything is excellent” And “ This home is an exceptionally well managed home” The inspection was unannounced in that the homeowners, staff and people living in the home did not know it was going to take place. This inspection was over a 3.25-hour period during the day on 06/06/07 and looked at various aspects of care. The drive down the lane to the home is a picturesque experience. As you approach the home it appears well maintained and window boxes enhance its appearance. In the report there are references to the “tracking process”, this is a method whereby the inspector focuses on a small group of residents. All records relating to these individuals are examined, along with the rooms they occupy in the home. Residents are invited to discuss their experiences of the home with the inspector; this is not to the exclusion of the other residents who contributed in many ways. This inspection included discussion with residents, staff and the homeowner in addition to viewing the home’s required written information such as the administration of medication records. The residents written plans of care were also viewed for 3 people. The plan of care is a document outlining the needs of the individual resident and how these are to be met. The plans of care cover all aspects of the individual’s life including health, personal care and social activities. Thereby ensuring people are content in the care they receive. The staff stated they enjoyed their work at Keer Sands and spoke to the inspector in a professional manner about the residents. They were also full of praise for Anne the homeowner who manages the home on a day to day basis. Further information was gained from a Pre Inspection Questionnaire completed by Mrs Anne Plevey the homeowner. This information included details of staff training, policies and procedures and how the homeowner ensures people receive the care they need. What the service does well:
Keer Sands is a small family run business, which has a very homely atmosphere. Here people are cared for as individuals and have their needs met by staff who are trained and competent. The people who live at Keer Sands are full of praise about the care they receive. People new to Keer Sands and their relatives are provided with opportunities to visit the home and allow them to make an informed decision.

The assessment is the basis for drawing up the Care Plan for the individual. People are asked about what their needs and wishes are, to ensure the care they receive is as they choose. Documentation is kept up-to-date and the use of daily records makes sure that individual need is regularly re-assessed. Each person living at Keer Sands has a written contract on the terms and conditions that apply to their stay in the home. The completed questionnaires received from the people who live at Keer Sands reflect that they were able to choose this home indeed people commented during the inspection that they feel they were lucky to live in this lovely home. One resident said “ I am so lucky to live here, the staff are so good and everything is excellent” She continued to say “I only came for a trial to see if I could like it, I don’t want to go back home now, so I am staying” Standard 6 was not assessed, as this home does not offer intermediate care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. Quality in this outcome area is excellent This judgement has been made using available evidence including a visit to this service. People benefit from living in a home where their health is promoted and their needs and aspirations are fully met. EVIDENCE: The homes policies and procedures in relation to the administration and storage of medication are robust. Staff who administer medication receive training in ‘Intermediate Medication Awareness’. The residents who are prescribed medication have signed a medicine declaration form outlining their wishes to self-medicate or have their medication administered by the staff.

There was information of District Nurse involvement for one of the three people case tracked. The District Nurse visited during the inspection and spoke of the how the home always follows the advice given. Another care plan reflected the programme that had been advised to enhance one persons mobility. There was a record of staff carrying out the advice offered and the successful outcome. The completed questionnaires received from the residents and their relatives included the following comments; “Any health problems are dealt with promptly” “The home fully meets the needs of my relative” “We get excellent care” “ When my Mum attends hospital appointments Anne (homeowner) always goes with her and Anne phones me as soon as they get back to let me know what has been said” A visiting health professional made the following comments on the completed questionnaire: “There is always a senior member of staff on duty and they know the residents well” The residents spoken with during this inspection said that as soon as they appear unwell or say they don’t feel well, their GP is called. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): Standards 12,13,14 and 15 Quality in this outcome area is excellent This judgement has been made using available evidence including a visit to this service. People are encouraged to make choices in all aspects of their life, leaving them content in the care they receive. EVIDENCE: There is a set menu at Keer Sands, which is based upon people’s known likes. People are told what is for the next meal and asked if they would like that or something else. Dislikes are also recorded to ensure an alternative is offered. The people spoken with all said the food was “excellent and all home made.” One resident said “I have put weight on since living here as the food is so good” Keer Sands Residential Home DS0000009869.V333621.R01.S.doc Version 5.2 Page 14 The completed questionnaires from a relatives indicated that the food was good and one stated ‘The food is excellent’. The meal being taken during this inspection looked appetising and was being enjoyed by all the residents. The following activities are some of the ones enjoyed by the residents: Music, Bingo, quizzes, walks, car rides and an entertainer comes in monthly. The residents said they are quite content with what is offered and if they want to do something different they would be helped to do it. One resident said she likes to go for a short walk each day and also said “ We are fortunate that we can join in activities and sit with company or we can have the privacy of our own room” Another resident said “we sometimes go out for a car ride and this is always welcome” The mobile library visits and some residents enjoy being able to choose books. Visitors are welcome at anytime according to the completed questionnaires received from the residents and their families. Comments included: “Visiting is always encouraged and always a very pleasant welcome feeling is experienced” “Visitors are always made welcome and given a cup of tea and cakes.” Keer Sands Residential Home DS0000009869.V333621.R01.S.doc Version 5.2 Page 15 Complaints and Protection

People benefit from living in a safe environment where they are protected and their opinion is all important. EVIDENCE: The people living at Keer Sands said that if they weren’t happy about something they would tell Anne (service provider) and she would rectify it. One resident added that she “can’t imagine not being happy about something as the care is excellent in every way” They also said they were comfortable and secure. One resident said “We are safe here knowing that the staff look after us so well” The homes policies and procedures ensure the people who live there are protected from abuse. The homeowner said all complaints would be recorded along with the action taken to rectify it. Neither the home nor the Commission for Social Care Inspection have received any complaints about Keer sands in the last 15 months. The resident’s benefit from living in a clean, hygienic and pleasant environment EVIDENCE: The home has a pleasant lounge with a separate dining room and a conservatory. The home was described by a relative of one resident as being ‘Bright and airy’. The home is well furnished and pleasantly decorated. There are ornaments, pictures and other such items that enhance the homely appearance of Keer sands.
Keer Sands Residential Home DS0000009869.V333621.R01.S.doc Version 5.2 Page 18 Some of the bedrooms and the ground floor bathroom have been redecorated since the last inspection in March 2006. The resident’s bedrooms contained their personal possessions and as such had a homely appearance. One resident had a large amount of personal possessions and there was recognition by the homeowner of how difficult it is for someone to leave their home and have to part with some of their possessions too. As such the home endeavours to help people to keep their possessions with them. Some people have brought their own furniture from their home helping them to feel that Keer Sands is their home. There are various aids around the home to help people keep their independence. These included handrails, chair lift and raised seats on the toilets. Completed questionnaires included many positive statements about the home including: “It is in a lovely spot in the countryside and such a super home” “The furniture and fittings are first class” One residents asked said “I love my bedroom it is so pretty and comfortable and I can look out of the window onto such lovely views” Keer Sands Residential Home DS0000009869.V333621.R01.S.doc Version 5.2 Page 19 Staffing

JUDGEMENT – we looked at outcomes for the following standard(s): Standards 27,28,29 and 30 Quality in this outcome area is excellent This judgement has been made using available evidence including a visit to this service. The residents benefit from living in a home where they are cared for by staff who are competent to do so. EVIDENCE: From looking at staff records and talking with the staff it is evident that staff training continues to be seen as a high priority in this home, ensuring the staff are competent to carry out their role and meet the needs of the residents. 6 care staff have achieved a National Vocational Qualification in care. This is 75 of the staff, which exceeds the National target. In addition to this staff receive training in Food Hygiene Health and Safety First Aid Moving and handling Medication Awareness
Keer Sands Residential Home DS0000009869.V333621.R01.S.doc Version 5.2 Page 20 Some staff have also had training in Dementia Awareness as some of the current residents have early symptoms of dementia. The staff files viewed contained the relevant information including an application form, 2 written references, Criminal Record Bureau clearances, induction record and records of training. A formal annual staff appraisal is carried out, with each member of staff. Regular supervision is held. The homeowner said that in a small home like this where the she works alongside the staff, there is a clear picture of each staff member’s performance where training needs are easily identified. During the inspection it was explained to Mrs Plevey that there is need to formalise and record staff supervision sessions. This was immediately accepted and will take place 6 times a year as required by the Care Homes Regulations. The residents’ benefit from living in a home that is well managed ensuring that people are well cared for. EVIDENCE: There is a quality monitoring system in place to ensure the residents have a say about the quality of the care they receive. The 6 residents spoken with were all very content in the home and felt that Anne (homeowner) was a very caring person. Anne is aware of the need to maintain a safe environment and ensures that all safety checks are carried out. Risk assessments are in place in relation to the building ensuring that any hazards are prevented. All doors have magnetically controlled valves on them. This means that doors can be held open safely and would be automatically closed should the fire alarm be sounded. The residents are encouraged to look after their own finances and where they lack capacity or choose not to, their family assists them. All the completed questionnaires reflect the home is well managed, including the following comment: “This home is extremely well managed” When asked how could the home improve one resident said “To improve on excellent would be impossible” Keer Sands Residential Home DS0000009869.V333621.R01.S.doc Version 5.2 Page 23 SCORING OF OUTCOMES
