Conditions of registration: Date of last inspection 2nd November 2004 Brief Description of the Service: Ancyra Health Limited is the owners of Kingsway Care Home and based in Langley Park, County Durham.The home was first registered in 1992.The home is registered to provide 24hr care for 47 service users 65 and over with mental health problems, the additional category allows them to take 10 service users under 65. This is an overview of what the inspector found during the inspection. This inspection took place on 13th May 2005 over 6 hours. During the inspection time was spent talking to service users, staff ad one relative. We looked around the building and a number of records were examined. This home had a change in management last summer, comments cards received and discussion with staff and a relative showed that there had been overall improvements in the management, care service delivery and environment within the home. What the service does well: What has improved since the last inspection?
Since the last inspection the layout of the upper floor dining room has been changed and an extra table put in place to provide more space and comfort for service users when dining. The uneven paved garden patio area to the rear of the building has been relaid and now provides safe accessible access to service users. Staff training has improved and staff feel more competent in carrying out their role. A relative said she can see ongoing improvements each time she visits and a more positive management approach. The intended outcomes for Standards 1 – 6 are: 1. 2. 3. 4. 5. 6. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 2 and 3 Each service user is provided with a Statement of Terms and Conditions at the point of moving into the home. New service users are admitted only on the basis of a full assessment undertaken by people trained to do so, and to which the prospective service user or their representative had been included in the process. This process of including and involving the service user and their representative in the admission process supports them with making the decision as to where they wish to live. EVIDENCE: Each service user has a copy of the Statement of Terms and Conditions of residency in the home. Inspection of these documents showed that they had been signed by the service user or relatives on their behalf as to their agreement. Individual records are kept for each service user and inspection of records contained a care management and home assessment carried out by the manager. The intended outcomes for Standards 7 – 11 are: 7. 8. 9. 10. 11. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 7 and 8 The care planning system remains limited in information and undeveloped in specfic areas and may not provide staff with information to satisfactorily meet the needs of the service users. EVIDENCE: Individual care plans are available but little progress has been made on the recommendation to ensure that service users or their representative sign as to their agreement with the care plan. A general risk assessment was in place but a significant event where a service user presented aggression showed there was no record of a risk assessment in place, associated plan or what preventative measures had been taken. Evaluation of care plans showed they were inconsistent and limited in information . The manager said a new care plan format is currently being developed by the company. The intended outcomes for Standards 12 - 15 are: 12. 13. 14. 15. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 12, 13, 14 and 15 The range and choice of activities available in the home are limited, which will impact on staff workload till the appointment of a new activities co-ordinator. There has been an improvement in the layout of the dining area on the upper floor creating a more comfortable environment. Visiting arrangements and community links are good. EVIDENCE: Discussion with manager, staff and service users showed that activities in the home were limited, due to the activities co-ordinator recently resigning. Staff said they are arranging activities in the interim period but it depends on the workload in the home. The manager said a new activities co-ordinator will commence in post next week. A relative spoken to said she visits when she can due to distance and is always made welcome by the staff, she commented psoitively on the care saying “Good caring staff and communication”. A service user said he goes out daily into the community with no restriction. Since the last inspection information on advocacy services has been obtained and is now displayed in the home for service user/relative information.

The intended outcomes for Standards 16 - 18 are: 16. 17. 18. Service users and their relatives and friends are confident that their complaints will be listened to, taken seriously and acted upon. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 16,17 and 18 The home has a satisfactory complaints system in place, with some evidence that relatives are aware of the process. Service users have access to information on advocacy services in the area if they wish to have someone speak on their behalf. Staff were confident in demonstrating an awareness of POVA to ensure the protection of service users living in the home. EVIDENCE: A complaints policy and procedure is in place. There have been no recorded complaints since the last inspection. A relative spoken to was aware of the complaints procedure. Information on advocacy services is available and displayed in the home for service user/relative information. A postal system of voting is in place if service users wish to participate in the electoral process. Since the last inspection a record that all staff had attended POVA (Protection of vulnerable adults) training was available. Staff during discussion were able to demonstrate an awareness of POVA. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 19 Relaying of patio area to rear of building has improved accessibility and safety for service users. EVIDENCE: Since the last inspection the paved patio area in the garden to the rear of the building has been relaid and to maintain health and safety of service users when sitting in the garden. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission considers Standards 27, 29, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 28 and 30 Records viewed and discussion with staff showed that staff are trained and competent to do their job maintaining the safety and well-being of service users. EVIDENCE: The home has achieved over 50 of care staff obtaining NVQ2 in care, which enhances the skills and knowledge of the staff within their role of responsibility in delivering the care service to the service users in the home. Staff during discussion were positive towards self-development and gaining knowledge of conditions that affect older people. Individual records of training were viewed for staff, which were up to date and applicable to the skills needed to deliver the care service to the people living in the home. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 31,35,37 and 38 The registered manager is positive towards self-development to enhance her skills and abilities to provide a good standard of care service delivery to service users. Robust procedures for the safekeeping of service users money are in place. Records in general including health and safety are up to date with the exception of care plans. EVIDENCE: The manager said since the last inspection she is nearing completion of the Registered Managers Award , which will enhance her skills in management of the home and service delivery. Individual records examined for money held in the home on behalf of service users were accurate, up to date and two signatures recorded for all transactions.

The manager was able to demonstrate that as far as is reasonably practical the health and safety and welfare of service users, staff and other visitors to the home was maintained. Health and safety records inspected were current and up to date. Staff during discusion said they had completed training on lifting and handling, fire safety and first aid.
