This inspection was carried out on 27th February 2006.

What the care home does well

Staff morale is high resulting in an enthusiastic workforce that works positively with service users to improve their whole quality of life. Recent investment has significantly improved the appearance of this home creating a comfortable and safe environment for those living there and visiting. The Registered Manager is of good character and able to discharge her responsibilities fully. She has a supportive, open approach to running the home, which benefits the service users, staff and relatives.The Manager is well supported by her senior staff in providing clear leadership throughout the home with all staff demonstrating an awareness of their roles and responsibilities.

What has improved since the last inspection?

The standard of care planning has improved with assessments for service users being completed in full and all identified problems have care plans in place. The standard of vetting and recruitment practices has improved with all the required checks being made. Regular audits of medication are taking place.

What the care home could do better:

The home should provide ceiling blinds in the conservatory to ensure the room is at a comfortable temperature for service users whilst they are eating. Two care records stated that reviews had been completed but no records were found. The home needs to maintain records of these.

Date of last inspection Brief Description of the Service: Kingswood Lodge is part of the Southern Cross Care Centres, and provides personal and nursing care for the elderly. The Home itself is an extended listed building sited in grounds shared by its sister home. Kingswood Lodge however, has its own level car park and gardens. Accommodation is provided across the building on two floors and divided into units. The home has undergone refurbishment and redecoration to improve the environment for the service users. This unannounced inspection took place over 4.30 hours on one day in February 2006. Seven service users were spoken with to gain their views on the home and three staff members and the Registered Manager were also spoken with. Staff were observed going about their duties and interacting with each other and service users. The requirements and recommendations made at the last inspection were followed up and records relating to the service users care, duty rotas, activities and personnel files, were inspected and a part tour of the home took place with a number of service users rooms inspected. The home continues to improve and no requirements or recommendations remain outstanding. Only one requirement has been issued at this inspection and this is for the home to provide ceiling blinds in the conservatory. All standards assessed at this inspection and the last inspection have now been met or exceeded except for one; however once this requirement is addressed this standard will be met. All service users spoken with praised all aspects of the home to include the staff, food and environment. All said they are very contented and if they had any concerns they would share them with the staff. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 7 There is a clear and consistent care planning system in place to adequately provide staff with the information they need to satisfactorily meet service users needs. EVIDENCE: Five care plans were examined in detail and the requirements issued at the last inspection were followed up. Assessments were completed on all service users except one who required a continence assessment. This was completed during the inspection. The home has addressed the requirement that all service users must have care plans for all identified problems. One recommendation has been addressed in full and one in part. The Registered Manager is going to ensure that on moving and handling assessments the type of sling is recorded if the service user requires equipment. It is recommended that the person undertaking the assessments sign them, as one assessment was found that had not been signed. The home should maintain records of service users reviews. A requirement issued at the last inspection for service users to have care plans for all ‘as and when’ prescribed medication has mostly been addressed. The Registered Manager is going to review this type of medication for several service users, as they have not been used. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 15 Improvements have been made with the standard of food provided by the home offering service users choice, variety and catering for special dietary needs. EVIDENCE: The home has a new activities coordinator who started this week and is in the process of planning a new programme for service users. A new chef has also started within the last month. The overall cleanliness of the kitchen has improved. The chef is in the process of sorting out all the kitchen records and devising new menus. Service users spoken with said the food is very good and has improved since the new chef has been in post. Service users have sufficient and suitable lavatories and washing facilities. Service users have the specialist equipment they require to maximise their independence. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): Recent investment has significantly improved the appearance of this home creating a comfortable and safe environment for those living there and visiting. However the temperature in the conservatory could be uncomfortable for service users without ceiling blinds. EVIDENCE: A part tour of the home took place with a number of service users rooms seen. Service users said they like their rooms and they are happy with the cleanliness of the home. No other issues were identified with the environment except that the conservatory, which is used as a dining room, does not have any ceiling blinds. The sun had made the room very warm on the day of the inspection and if the sun is very bright it can be harsh on anyone’s eyes. The home must provide blinds to ensure service users are kept at a comfortable temperature. The Commission consider all the above are key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 27 Staff morale is high resulting in an enthusiastic workforce that works positively with service users to improve their whole quality of life. EVIDENCE: An anonymous concern was followed up that related to staffing. The concern was around a member of staff working a late shift followed by a night shift. The Registered Manager said this was happening but she has now stopped this practice; however on the off duty this member of staff was down to work this shift the following week. The Registered Manager was rectifying this during the inspection. All service users complimented the staff saying they are friendly and very helpful. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 31 The Registered Manager is of good character and able to discharge her responsibilities fully. She has a supportive, open approach to running the home, which benefits the service users, staff and relatives. EVIDENCE: Since the last inspection the Commission has registered the Manager. At this inspection she demonstrated her competence in managing the home. Any issues identified were immediately rectified during the inspection. The Registered Manager said there are areas she needs to improve and has plans in place to address these.
