Conditions of registration: 1. Service users to have an associated sensory impairment. Date of last inspection 6th May 2005 Brief Description of the Service: 151 Knaresborough Road is registered to provide residential social and personal care for 8 adults who have learning disabilities and an associated sensory impairment. The home is a large well maintained property situated between Harrogate and Knaresborough and has good access to services and amenities. The registered provider is Homes Together Ltd. The responsible individual is Mr S Jones. The registered manager is Mrs S Linfoot. Knaresborough Road (151) DS0000061613.V272547.R01.S.doc Version 5.0 Page 5 SUMMARY
This is an overview of what the inspector found during the inspection. This was an unannounced inspection carried out on 13th December 2005 as part of the inspection process. The manager, Mrs Linfoot, was available to assist with the inspection and it was also possible to speak to three members of staff and see all of the residents. The residents all communicate in a very specialised way but the staff were able to interpret their responses for the inspector. Many of the findings however are based on observation. It was possible during the inspection to witness some of the care practices and the interactions between the residents and the staff. The inspection also involved a tour of the premises, a check on the requirements and recommendations from the previous inspection, and a check on some of the records kept by the home. The home has a cheerful atmosphere and is clearly run for the benefit of the residents. The inspection showed that the home is well organised and managed, the staff are well trained and aware of their duties, and the residents are well cared for. The inspection took 4.5 hours which includes preparation time. What the service does well: What has improved since the last inspection?

Some repairs / improvements have been made to the environment. Staff training has progressed ensuring that staff are constantly improving their skills to meet the changing needs of the residents. The intended outcomes for Standards 1 – 5 are: 1. 2. 3. 4. 5. Prospective service users have the information they need to make an informed choice about where to live. Prospective users’ individual aspirations and needs are assessed. Prospective service users know that the home that they will choose will meet their needs and aspirations. Prospective service users have an opportunity to visit and to “test drive” the home. Each service user has an individual written contract or statement of terms and conditions with the home. The Commission consider Standard 2 the key standard to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 2,4,5. Residents know that if they choose to live in the home that they will be well cared for. EVIDENCE: Although no new residents have been admitted for over a year procedures are in place and previous admissions showed that assessments involving residents’ family / carers and other professionals are carried out prior to admission to ensure that the staff can meet their needs. Service users make decisions about their lives with assistance as needed. Service users are consulted on, and participate in, all aspects of life in the home. Service users are supported to take risks as part of an independent lifestyle. Service users know that information about them is handled appropriately, and that their confidences are kept. The Commission considers Standards 6, 7 and 9 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 6,7,9. The residents’ health and personal care needs are met and they are encouraged and supported to make choices about their daily lives. This helps them to have some control over their lives in the home. EVIDENCE: All residents have a detailed care plan which identifies their personal and social needs. The plan is developed from the detailed information the staff have gained from their experience of working with them. The residents benefit from having an established and settled staff team. The staff have regular meetings to discuss the changing needs of the residents and review the care plans. This ensures that staff are kept up to date and a consistent approach is achieved. The residents’ files contained all the relevant information required by the care staff and were very well organised, making them easy to read and to use. Service users have appropriate personal, family and sexual relationships. Service users’ rights are respected and responsibilities recognised in their daily lives. Service users are offered a healthy diet and enjoy their meals and mealtimes. The Commission considers Standards 12, 13, 15, 16 and 17 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 12,13,15,16,17. The residents eat well and enjoy a wide range of activities both inside and out of the home and are part of the local community. EVIDENCE: Each resident had an individual programme aimed at developing their skills and staff are available to support them. The staff are constantly looking for new activities which will interest the residents and provide further stimulation and development. All residents use the local facilities in Knaresborough and Harrogate eg swimming pool, cafes, shops and supermarket. Food is an important part of the residents’ lives and the cook and the staff know what they enjoy and ensure their favourites are included in the weekly menus. The cook said that she always uses fresh produce as much as possible and this was witnessed during the inspection. She also said she considered the food budget adequate and the cooking facilities were suitable to provide the meals required. Lunch was served during the inspection and the residents

The intended outcomes for Standards 18 - 21 are: 18. 19. 20. 21. Service users receive personal support in the way they prefer and require. Service users’ physical and emotional health needs are met. Service users retain, administer and control their own medication where appropriate, and are protected by the home’s policies and procedures for dealing with medicines. The ageing, illness and death of a service user are handled with respect and as the individual would wish. The Commission considers Standards 18, 19, and 20 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 18,19,20. Individual resident’s physical and emotional needs are met. EVIDENCE: Residents have their personal support needs identified in detailed care plans and include clear instruction about how support is provided safely and according to the service users preference which ensures that they receive the care in the way they prefer and where possible by the carer they prefer. Staff induction and on going training includes care practice that ensures and promotes privacy and dignity and respect. The home uses the Boots monitored dosage system and has a clear policy for the storage and administration of medication. All staff have had training from Boots and the Community Pharmacist on the Safe Administration of Medication and are starting an accredited course with an external trainer. None of the service users self medicate. All records were well maintained and demonstrated the professional approach taken by the staff and ensures the safe use of medication by the residents. The Commission considers Standards 22, and 23 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 22,23. Residents are protected from abuse, neglect, and self harm. EVIDENCE: The home has a detailed complaints procedure, in suitable formats for the residents. In reality the staff have to observe the demeanour of the residents and investigate any changes. The vulnerable adults procedure is available in the home and training of staff has been recorded. Staff were aware of the types of abuse and the whistle blowing procedure. Because of the vulnerability of the residents it was recommended that the policy be reinforced at staff meetings to help staff to understand the vulnerable adults procedure. There were no concerns about the residents being protected from abuse. The home has a robust recruitment procedure which ensures that suitable people are employed in the home. Shared spaces complement and supplement service users’ individual rooms. Service users have the specialist equipment they require to maximise their independence. The home is clean and hygienic. The Commission considers Standards 24, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 24,30. The home is clean and well provided with specialist equipment to meet the residents needs. Some of the decorations were not up to the required standard. EVIDENCE: The home has suffered from some tree root subsidence and repair work is in the hands of insurers but has been programmed. The decorations especially on the first floor were faded which detracted from the overall smartness of the home. Good cleaning and hygiene programmes were in place to prevent the risk of any infection and the home was clean and free of any offensive odours. The staff considered that there was sufficient specialist equipment in the home to meet the needs of the residents. JUDGEMENT – we looked at outcomes for the following standard(s): 32,34,35. The staff are well trained and well supported by the manager and senior staff which ensures that the residents are supported and the staff are aware of their duties. EVIDENCE: The home has a settled staff team and a core of experienced mainly senior staff who have been at the home for many years, know the residents very well, and provide the stability they need. It was clear that the staff enjoyed their work and work well as a team. The staff are well supervised and supported which ensures they have the skills required to do their work. The home has a comprehensive recruitment procedure which ensures that unsuitable people are not employed. Service users benefit from competent and accountable management of the service. The Commission considers Standards 37, 39, and 42 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 37,39,42. The health and safety and welfare of the residents is consistently promoted by the registered manager and senior staff who provide a very settled staff unit and offer clear leadership and guidance to the support staff. EVIDENCE: The health and safety records were well maintained and up to date and this helps to safeguard the safety of staff and residents in the home. The quality assurance system is being developed to measure and assess that the home is meeting the aims and objectives of its Statement of Purpose. An annual report will be sent to the Commission.
