Conditions of registration: 1. At any time up to 10 beds can be used to provide care for service users in either category OP or DE(E) 16th November 2005 Date of last inspection Brief Description of the Service: Lawrence Court is a purpose built care home with nursing located in Byker. It is situated on the same site as another care home owned by the same company. The home provides nursing care for 23 older persons and the ground floor and provides social and personal care for 23 older persons on the first floor. The two floors are accessible via stairs and a passenger lift. There are two lounges and one dining room on each floor and there are designated smoking lounges. All of the bedrooms are single en suite and there are adapted bathing, showers, and toilet facilities throughout the home. The lower ground floor is not readily accessible to residents. This floor has the staff room, kitchens and laundry services. The home has an accessible garden area and there is ample car parking. The home is close to local shops and leisure facilities and is on a local bus route. The current scale of charges is £356 per week. This is an overview of what the inspector found during the inspection. This was a key unannounced inspection and took place over two days and involved one inspector. Residents care records, staff rota, recruitment/training files plus additional statutory records were examined. The inspector had informal and formal discussion with the manager, deputy, two staff, and three ancillary staff. Six residents and five relative were spoken to during the visit. There were twenty one requirements identified during the last inspection and all but three of which have been fully met. Some of the requirements have remained outstanding however they have been adjusted to reflect the work that has been carried out. There have been no additional requirements made. The intended outcomes for Standards 1 – 6 are: 1. 2. 3. 4. 5. 6. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 3&6 Quality in this outcome area is good. This judgment has been made using available evidence including a visit to this service. Residents are only admitted into the home after a detailed assessment including information obtained from Social Services, previous professional carers, the residents and their families. The home is not registered for therefore does not provide intermediate care. EVIDENCE: Care plans have detailed information to make sure that the home can meet the needs of the prospective resident. The Manager is involved in the decisions and in the majority of instances visits the residents himself prior to their admission. The care plans contained both the Social Services assessment and additional information obtained from other places including residents, relatives and in some cases the professionals caring from the residents at the placement they were in at the time prior to admission.

The intended outcomes for Standards 7 – 11 are: 7. 8. 9. 10. 11. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 7, 8, 9 & 10 Quality in this outcome area is good. This judgment has been made using available evidence including a visit to this service. The residents have their needs met by the home and the staff are skilled in providing care in a sensitive and dignified manner. The residents receive their prescribed medication in line with safe working practices and the medicines are well managed and safely disposed of as necessary. EVIDENCE: Four care plans were examined, they were completed well, with relevant risk assessments for the prevention of falls, nutrition, moving and assisting, and continence promotion. The plans showed that they are regularly reviewed and updated. The home are supported by the community nursing service and they provide the nursing advice as necessary. The care plans showed that the residents have access to all NHS services and facilities. A number of assessment tools are in use, and they were reviewed monthly, and were dated and signed by the author. Daily reporting of residents

The staff have a good knowledge of residents care needs with evidence of good partnership workings with other professionals. Throughout the visit staff were treating residents with respect and dignity. Personal care was given in privacy, staff used residents preferred name at all times. Residents were complementary about the staff in the home and felt that they were able to have privacy in their own rooms and the staff respects that this. One relatives’ comments card was returned, it was answered positively and there were no negative comments made. One resident comment card was returned which was also positive about the home and included the comment “activities very good”. The medicines in the home are generally well managed and safely disposed of as necessary. Where hand written entries were required on the Medicines Administration Records these were not always signed as necessary, therefore an effective audit could not be carried out. This must be addressed. The treatment room was very tidy and was well organised. Two residents medication was examined as part of the case tracking and they were being administered appropriately in line with good practice guidlines. There was a separate list of signatures of staff on record for auditing purposes and this was up date for new staff. There has been a problem with ordering medicines since the last inspection when a member of staff innapropriatley altered a prescription prior to it going to the pharmacist. This was investigated by both the Police and the home, and was found to have been done by staff without the intention to defraud or cause harm. This has been dealt with by the Manager with additional supervision, staff training and a change in the procedures. There is now no longer a risk of this re-occurring, it was dealt with effectively.
