This inspection was carried out on 12th August 2005.

The inspector made no statutory requirements on the home as a result of this inspection and there were no outstanding actions from the previous inspection report.

What the care home does well

The health and personal care needs of residents are well met. Care staff are prompt to report any problems as they arise and contact the appropriate member of the multidisciplinary team as needed. There is a relaxed and friendly atmosphere in the home and staff provide the right level of support for residents needing assistance. The home consistently meets a lot of the standards and offers a very good standard of care. The Manager felt they offer a good standard of care and felt this was reflected in the number of comments she receives from people about the friendliness of the home. The home has a group of Staff and Manager who have worked at the home a long time offering a great stability to the home and to its Residents. Activities are always fully displayed in a large visual notice board in the reception area. Staff receive regular supervision sessions from their line Managers.

What has improved since the last inspection?

The home has undergone further redecoration and refurbishment including the purchase of a selection of new armchairs and coffee tables and the purchase of 3 electrical style beds to assist with nursing. The home now has new documentation from Southern Cross which covers most areas including care plans and is being implemented in most areas. A report was produced from Southern Cross`s own audit team which detailed a recent catering audit.

The service should at all times, employ a suitably qualified and experienced manager who is registered with the CSCI. Date of last inspection 24th November 2004 Brief Description of the Service: Lymewood Court Nursing Home is registered for 46 beds for elderly/physically disabled over the age of 65 years, however, the Home has been granted a condition of registration by the (Previous Area Manager) to admit service users over 55 years.The Home is purpose built in design on a single level for residential services. The Home is situated close to local amenities in Haydock.The Home is privately owned and managed by Southern Cross care management Ltd. The named Responsible Individual is Mrs Jeanette Malham and the Home’s Registered Manager is Mrs Chris Corsair. The inspection took place over 4.5 hours. It was an unannounced visit and was carried out as part of the regulatory requirement for care homes to be inspected at least twice a year. There has been no cause for any visits to the home since the last routine inspection in 24/11/04. A partial tour of the home was carried out. Care records and other care home records were inspected. Some Staff on duty chatted informally with the Inspector and general discussions took place with various Residents and a selection of comment cards were left and in total 3 have been received. During this unannounced inspection, feedback from everyone involved was very positive. It was evident that the Manager continues to implement all parts of the National Minimum Standards. All 3 comment cards indicated they were happy with the care and 1 stated they felt “that there was not always sufficient numbers of staff on duty”. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 7,8,9,10 There continues to be progress made within care plans which include details for health, personal and social care needs of Residents. Medication procedures were found to be in good order with a well organised medical room EVIDENCE: Residents and Relatives were very happy with the care provided at the home, the Manager feels the home really does offer a homely environment. The care records seen had a lot of detailed information to meet these standards. The home has its own medication store room which is kept locked. This area is managed by the Nurses at the home and was found to be very organised and well run. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 12.13.14.15 Social activities and meals are both well managed and there are a variety of choices available for Residents, EVIDENCE: The Residents and Relatives are happy with the activities on offer and explained that they could just go to their bedroom if they didn’t t want to take part. The Staff explained that they try to organise an activity each day and try to do this between them and the activities organisers. There is also a large visual notice board in reception which keeps everyone informed of the events organised by the company. There were compliments about the food and Staff and Residents said that if they didn’t t like what was on the menu that that the Cook would just provide something else. The menu of the day is displayed on a large board in the dining room. Relatives explained `that they are welcome to visit at any time of the day. Service users and their relatives and friends are confident that their complaints will be listened to, taken seriously and acted upon. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 16.18 The home has a complaints policy that Residents, Relatives and Staff know how to use. Staff are trained in “Abuse Awareness” and know about the policies that should be carried out. EVIDENCE: Staff described their policies and what they would do if there was a problem. The complaint record book was seen and gave details of any actions taken by the Manager following a grumble or complaint. The Staff described their recent training for making them aware of abuse and what steps to follow if they needed to carry out the policy. As discussed during previous inspections, policies for managing Service Users’ finances in relation to Regulation 20 should be developed as a matter of priority, this standard was not measured during this inspection as Staff stated there had been no changes to the present procedures and this is an outstanding requirement from the previous inspection. Service users live in a safe, well-maintained environment. Service users have access to safe and comfortable indoor and outdoor communal facilities. Service users have sufficient and suitable lavatories and washing facilities. Service users have the specialist equipment they require to maximise their independence. Service users’ own rooms suit their needs. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 19.23.26 The environment is well managed and kept clean and tidy. Sufficient equipment is available for the Staff to use in ensuring that the good hygiene and cleanliness levels are maintained. There home have not produced an updated maintenance and decorating programme. EVIDENCE: Residents were very happy with the homes facilities especially their bedrooms and that they could bring a lot of their own belongings into their bedroom. The home was very clean and tidy especially the sample of areas seen. The home did not have an updated maintenance and decorating plan to show a planned approach to all parts of the home which would keep the Residents and Staff informed of changes to the home, it should be developed as previously discussed to show compliance and to provide a structured development plan for the Manager to use for future planning of the home. There has been some redecoration and refurbishment to the home since the last inspection were new armchairs and coffees tables had also been purchased. Some baths still had scrapes to bath panels and were in need of repair. Service users needs are met by the numbers and skill mix of staff. Service users are in safe hands at all times. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission considers Standards 27, 29, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 27 Residents and Relatives say they are very happy with the home and the care provided by Staff. EVIDENCE: Residents and Relatives say they are happy with the care and say the Staff are lovely. Everyone in the lounge was seen to be given a good level of care and respect, the atmosphere was very informal and happy were everyone was helped to feel comfortable. Most Staff had worked at the home for many years and offered a great stability to the workforce. The Manager discussed possible changes to the current staffing levels as an initiative to review the companies current levels, advise was given regarding the legal responsibilities of the Company within the Care Home Regulations to always provide suitable numbers of qualified and competently trained members of Staff at all times. It was agreed that any revision to the current Staffing levels would be evidenced by the Company by having written assessments to show written evidence that the Staffing levels are appropriate using a published assessment tool based on the present dependency of Residents at the home. Also any considered changes should include (as a matter of good practice and measurement against the National Minimum Standards) consultation with all parties to elicit their opinions to the proposed Staffing levels by the company. Any changes should not only be evidenced as appropriate but should also be updated in the homes Statement of Purpose. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 31.33.38 The Manager runs a well managed home. Regulation 26 reports should be submitted on a consistently monthly basis to CSCI once carried out by the company representative. EVIDENCE: There were many compliments from the Residents and Staff about the Manager and her open style of management especially in how respectful she was and always open to look into things. Staff explained that if they come across anything broken they record this in the maintenance book and the maintenance man reviews this regularly to carry out repairs which helps to keep the home in a good state of repair. Regulation 26 reports have been carried out but reports have not always submitted on a monthly basis to CSCI.
