Conditions of registration: 1. This home is registered for a maximum of 4 service users in the category of LD (learning disability) 10th March 2006 Date of last inspection Brief Description of the Service: 12 Woodland Road is a care home for four adults with a learning disability. It is in a residential area of Ellesmere Port, close to shops, pub and the town centre. The home is a dormer bungalow and service users are accommodated on the ground floor. All the bedrooms are single and are individually furnished and decorated. Each of the bedrooms contains hoists and other lifting aids to assist service users. The home has one bathroom, one shower with toilet and two separate toilets. Communal space consists of a dining area in the kitchen, lounge and conservatory. Car parking space is available to the front and rear of the home. Staff are on duty twenty-four hours a day to care for service users. The current charge for living at the home is £1350 per week. This is an overview of what the inspector found during the inspection. An unannounced visit took place on the 17 January 2007and lasted 3.5 hours. The visit was carried out by Val Flannery, Regulatory Inspector This visit was just one part of the inspection. Before the visit the home manager was also asked to complete a questionnaire to provide up to date information about services in the home. CSCI questionnaires were also made available for residents, families, health and social care professionals to find out their views. Other information received since the last key inspection was also reviewed. During the visit various records and the premises were looked at. Service users and staff were spoken with and they gave their views about the service. What the service does well:
The home provides relevant information on the service offered. Possible new residents are able to visit the home before deciding to move in so they know about the home before they move in. Staff from the home carry out an assessment so that the resident, their relatives and the staff at the home know that the person’s needs can be met there. Although the current service users have limited communication, they are actively involved in making decisions on how they live their lives. Wherever possible service users are encouraged to make decisions about the care they receive and are offered choices around everyday living routines. Individual service users have plans of care that includes a weekly activity programme and shows the level of level of help they need with these activities. Service users are supported by staff to regularly use community-based services. The residents receive the healthcare they need from doctors, nurses and others, such as dentists, to keep them well and healthy. The staff team help them with the personal care they need to help them take part in their everyday lives. The home is well looked after so that it’s comfortable and safe for the residents. Staff are well trained and supervised so that they can give the best possible help and support to the residents. The manager is qualified and has a number of years experience caring for service users with a learning disability. The intended outcomes for Standards 1 – 5 are: 1. 2. 3. 4. 5. Prospective service users have the information they need to make an informed choice about where to live. Prospective users’ individual aspirations and needs are assessed. Prospective service users know that the home that they will choose will meet their needs and aspirations. Prospective service users have an opportunity to visit and to “test drive” the home. Each service user has an individual written contract or statement of terms and conditions with the home. The Commission consider Standard 2 the key standard to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 1, 2, 3, 4, & 5 Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. Information is available on the service offered by the home and there is a procedure to follow when new residents move into the home. New residents, therefore, would have enough information to decide if their needs could be met at the home. EVIDENCE: The current service user group have lived in the home for a number of years so the admission procedure has not been ‘tested’. However, MacIntyre have developed procedures that ensure prospective service users are made aware of the service offered by the home. This can include introductory visits and overnight stays to meet other service users and staff. Staff from the home as well as the care manager from the placing authority would carry out full assessments. A copy of the service user guide and statement of purpose are kept in the home and would be made available to prospective service users and/or their representatives. Currently the home has a block contract with the funding authority to care for the service users. The manager said that the organisation is in discussion with Social Services about individual service user contracts. A copy of the placement agreement between the organisation and the individual service users is kept on their files. Service users are supported to take risks as part of an independent lifestyle. Service users know that information about them is handled appropriately, and that their confidences are kept. The Commission considers Standards 6, 7 and 9 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 6, 7, 8 & 9 Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. Records are in place that show that changes to service users’ needs are identified and acted upon. This ensures staff have the information to help service user make ‘safe’ choices about what they do each day. EVIDENCE: Although the residents have limited communication, staff were seen talking with and supporting service users to make decisions on daily living. For example, where they wish to sit, if they wished to go to their bedrooms and helping them with their mid-day meal. Comprehensive plans of care and background information are available on individual service users. This sets out how service users preferred to be cared for, their likes and dislikes, family involvement and how their healthcare needs are to be met. Risk assessments are in place that show service users are encouraged and supported to take responsible risks, both in the home and in the local community. Staff spoken with were aware of the needs and

The manager said that the yearly review of the service offered is to be carried out by Social Services later this month. Service users are offered a healthy diet and enjoy their meals and mealtimes. The Commission considers Standards 12, 13, 15, 16 and 17 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 11, 12, 13, 14, 15, 16 & 17 Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. Staff support service users to take part in a range of activities, both in the home and in the local community. This helps ensure service users maintain links with the local community. EVIDENCE: The plans of care, and other records, seen during the visit showed that each service user has an individual weekly schedule which involves them doing activities in the home and in the community. These included visits to hydrotherapy, sound beam, music and movement, reflexology and going out to local attractions such as Eastham Ferry and the Boat Museum. Also included were trips out to local shops and other local amenities. On the day of the visit one service user was seen returning from a hydrotherapy session. The registered manager said that rather then going away on an annual holiday they organised days out for the service users. These confirmed that they are encouraged and helped to keep in touch with the residents. During the visit staff were seen helping service users move about the home, use the bathroom and providing regular drinks. Staff were also seen helping service users with their mid-day meal. Service users were seen moving between their bedrooms and the communal areas, with staff assistance. Staff were seen offering assistance to service users in a caring manner and were sensitive to their capabilities. For example, staff talked with service users and explained what they were doing. They closed bedroom doors when caring for service users in their bedrooms. Daily routines are flexible and are dependent on service users’ wishes; for example, when they wish to eat their meals. Advice and guidance has been sought from dieticians on the food offered to service users. All of the service users have swallowing/eating difficulties and staff have provided separate, laminated lists of what each service user can eat. These are on display in the dining area. Service users retain, administer and control their own medication where appropriate, and are protected by the home’s policies and procedures for dealing with medicines. The ageing, illness and death of a service user are handled with respect and as the individual would wish. The Commission considers Standards 18, 19, and 20 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 18, 19 & 20 Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. The home provides appropriate personal and healthcare support to service users. This ensures their assessed care needs are met and that they stay well and healthy. EVIDENCE: The records seen during the visit showed that the healthcare needs of service users are monitored and action is taken to resolve any problems. A comment was seen in the complaints/compliment book whereby a healthcare professional complimented the staff on their actions with regard to service users’ healthcare needs. Service users’ files are kept in their bedrooms and include: • a record of any care or treatment from doctors, nurses and others such as dentists and opticians • personal medical information • a detailed guide about how to support the service user, called ‘My Life’ • a file titled ‘Daily Health and Well Being’. Macintyre Care - 12 Woodland Road DS0000006682.V307519.R01.S.doc Version 5.2 Page 14 MacIntyre have provided policies and procedures on the administration of medication to service users by staff, a copy of which is kept in the home. Services users have a locked wall cabinet in their bedrooms where their personal medication is kept. The organisation also provides training for staff on the administration of medication so they can give residents their medication safely. Help with service users’ personal care was seen being carried out by staff in service users’ own rooms. This was done in a calm and sensitive manner. Both male and female staff work at the home so all residents can be cared for by staff of the same gender. The Commission considers Standards 22, and 23 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 22 & 23 Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. Procedures are in place to deal with any complaints or adult protection issues that may arise. These ensure that the residents and/or their relatives are listened to and that residents are protected from possible harm. EVIDENCE: A copy of the complaints procedure was on display in the home. The manager said the home have not received any complaints nor made any referrals under the adult protection procedure since the last inspection. The complaints/ compliments book was seen during the visit. A copy of the government guidelines on adult protection titled ‘No Secrets’ is kept in the home. The manager said three staff are receiving up dated training on adult protection on the 29 January 07. Staff spoken with during the visit were aware of what action to take if they received a complaint or were concerned about an adult protection issue. The Commission considers Standards 24, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 24, 25, 26, 27, Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. The home is well looked after so that service users continue to live in comfortable and safe surroundings. They all have single rooms so their privacy and dignity is maintained. EVIDENCE: The home is a domestic type building that provides service users with wellmaintained, comfortable and homely surroundings. Bedrooms are individually decorated and furnished. Hoists/lifting aids are also provided for service users with mobility problems. There are one bathroom, one shower and two separate toilets for service users. The bathroom and shower have lifting equipment in them and the doors to all the bathrooms and toilets can be locked for service users’ privacy. Communal areas for service users consist of a lounge, kitchen and conservatory. A section of the garden, with raised flowerbeds, has been made safe for the residents to use.

JUDGEMENT – we looked at outcomes for the following standard(s): 31, 32, 33, 34, 35 & 36 Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. Staff receive training, support and supervision to ensure they are able to meet the needs of service users. Thorough recruitments processes are in place to make sure that residents are protected from possible harm. EVIDENCE: The manager and support staff spoken with during the visit were aware of their role and responsibilities in caring for the service users. Staff were seen talking with service users, helping them move around the home and responding to their requests for assistance. It was evident that the staff on duty were aware of the service users’ capabilities; service users were seen approaching staff and were comfortable in their company. Staff spoken with were able to talk in detail about the needs of service users and how these needs were to be met. They also said that they are able to discuss any concerns/worries with the manager and they would seek advice to ensure service users receive the help they need. The staffing rota showed that there are normally at least two staff on duty during the day and one waking staff plus one staff sleeping in during the night. On the day of the visit additional staff were on duty to support service users with their daily activities.

Three staff are working towards their NVQ Level 3. A separate list provided by the manager showed that staff have access to a range of training opportunities including: • Fire Awareness • Food Hygiene • Manual Handling • Risk assessment • Person Centred Planning • Medication Individual staff have a Personal Development Portfolio that sets out the training they have attended and also includes their training needs. Records were seen that showed satisfactory recruitment procedures are in place including obtaining two references and Criminal Record Bureau disclosures before any new staff start working at the home. JUDGEMENT – we looked at outcomes for the following standard(s): 37, 38, 39, 40 & 43 Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. The registered manager is qualified and experienced at running services for people with learning disabilities so that residents live in a home that is run well in their best interests. EVIDENCE: The manager confirmed that she completed her NVQ Level 4 in care and the registered managers award in January 2006. She has attended a number of courses to further update her management skills in the day-to-day running of the home. She is currently doing a City and Guilds certificate in learning disability and is an NVQ assessor. Staff spoken with said they receive supervision and support from the manager and that she will address issues about their performance with them. They also said the manager is ‘very committed to providing a good home for service users to live in’.

These include health and safety, adult protection and managing service users’ finances. During the visit, a service engineer from British Gas was in the home carrying out repairs to radiators. A copy of the service record for the hoists was also seen. The manager said that insurance cover is in place for kitchen equipment and the lifting appliance in the bathroom. The manager said that MacIntyre surveyed service users and relatives in the spring of 2006 to find out what they thought of the home.
