Conditions of registration: 1 This home is registered for a maximum of 4 service users in the category of LD (learning disability) Date of last inspection 02/12/04 Brief Description of the Service: 12 Woodland Road is a care home for four adults with a learning disability. Located in a residential area of Ellesmere Port the home is close to shops, pub and the town centre. The home is a dormer bungalow, service users are accommodated on the ground floor. All the bedrooms are single and are individually furnished and decorated. Each of the bedrooms contains hoists and other lifting aids to assist service users. The home provides one bathroom, one shower with toilet and two separate toilets. Communal space consists of a dining area in the kitchen, lounge and a recently constructed conservatory. A section of the garden to the rear of the home has been secured to ensure service users safety. Lifting aids are also provided in the bathing area. This is an overview of what the inspector found during the inspection. This inspection was carried out over three hours. One hour was spent reading previous inspection report and reviewing the service history of the home. All the service users were spoken with during the inspection. The staff on duty, including the manager, were also spoken with. A tour of the building was carried out. All the bedrooms were seen during the visit. The secure garden to the rear of the home was also seen. A number of service user and home records were inspected. No relatives or other visitors were in the home during the visit. The service users have limited communication skills. What the service does well:
Service users files contain a lot of information including background details and personal histories. The care plans showed that service users’ care needs have been identified and that these needs are met. Because service users have lived in the home for some time staff have learned how best to meet their needs. Service users are supported by staff to use community facilities which ensures they have a varied and stimulating lifestyle. Risk assessments have identified possible safety issues and how these are to be addressed. The record of menus showed that service users are offered a choice of foods. During the visit staff were seen helping service users with personal care tasks such as dressing, eating, drinking and administrating medication. This was done in a quiet and caring manner. MacIntyre Care have provided policies and procedures on the administration of medication. They also provide a copy of the complaints procedure in picture format for ease of use by service users. Service users’ bedrooms and communal areas are maintained to a high standard. The overall standard of the decoration and furniture provides a warm and safe environment for service users. The manager and staff provide a high standard of care to service users. Prospective users’ individual aspirations and needs are assessed. Prospective service users’ know that the home that they will choose will meet their needs and aspirations. Prospective service users have an opportunity to visit and to “test drive” the home. Each service user has an individual written contract or statement of terms and conditions with the home. The Commission consider Standard 2 the key standard to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 1/2/5 Service users’ care needs are identified, monitored and action taken to meet these needs. The statement of purpose and service user guide ensures information is available on the services offered by the home. EVIDENCE: The current service user group have lived in the home for a number of years. Service users’ plans of care showed that staff have identified their individual care needs and how these needs are to be met. Because of their limited verbal skills the care offered to service users is closely monitored and any changes recorded. A statement of purpose and service user guide was available which showed the services offered by the home. Currently the home has a block contract with the funding authority to care for the service users. The manager said that the organisation is in discussion with the funding authority to have individual service user contracts. JUDGEMENT – we looked at outcomes for standard(s) 6/7/8/9 Services users’ care needs have been identified and included in their plans of care. The home is able to meet these needs. Risk assessments identify possible dangers to service users and safe guards are in place to protect them. EVIDENCE: The person centred plans contain a wide range of information on service users and how their assessed care needs are to be met. Included was the level of staff support required by service users in their daily lives. The care files seen were well laid out and contained information which was up to date. Assessed care needs of service users showed that they need staff support with every aspect of daily living. This includes using the bathroom, toilet, eating and moving about the home. Staff were observed helping service users with these tasks in a sensitive and caring manner. Wherever possible service users are encouraged to take responsible risks as part of their independent lifestyle, for example, when they go out in the garden and moving about the home. Service users are offered a healthy diet and enjoy their meals and mealtimes. The Commission considers Standards 12, 13, 15, 16 and 17 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 11/12/13/14/17 Service users are offered a varied and stimulating lifestyle and are appropriately supported by staff. The menus reflect the service users preferences and choices. EVIDENCE: Plans of care showed that service users are involved in appropriate activities. This can include hydrotherapy, rebound therapy, attendance at a community day centre and day care services provided by Social Services. On the day of the visit service users, supported by staff, went to visit a local beauty spot. The manager said that service users are supported to use local shops and other amenities. Included in the plans of care was the level of staff support required by service users to access these amenities. Mealtimes are flexible with service users able to eat their meals when they wish and at their own pace. The record of menus showed that choices are offered to service users which reflect their personal preferences. One service

The intended outcomes for Standards 18 - 21 are: 18. 19. 20. 21. Service users receive personal support in the way they prefer and require. Service users’ physical and emotional health needs are met. Service users retain, administer and control their own medication where appropriate, and are protected by the home’s policies and procedures for dealing with medicines. The ageing, illness and death of a service user are handled with respect and as the individual would wish. The Commission considers Standards 18, 19, and 20 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 18/19/20 Service users’ health and personal care needs are set out in their plans of care. The home ensures these needs are met. Staff offer full support with service users health and personal care needs. EVIDENCE: Staff were observed helping service users with personal care tasks, for example, using the bathroom, getting dressed and having an afternoon drink. They were also seen talking with service users and responding to their individual personal needs. The plans of care showed service users’ preferences on how they wished to be cared for are recorded. Because service users have lived in the home for some time staff have developed an awareness of their care needs. Plans of care seen showed that service users receive visits from doctors, nurses and other health professionals. The reasons for the visits and the recommended treatment were also recorded. The manager confirmed that providing a comprehensive level of personal and healthcare is a priority. Staff support service users to attend hospital, dental and other appointments. Satisfactory arrangements were observed for the administration of medication to service users. Service users require full assistance with the administration of medication.

The Commission considers Standards 22, and 23 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 22/23 Satisfactory arrangements are in place to respond to complaints and adult protection issues. EVIDENCE: The complaints procedure includes details on how to contact the Commission for Social Care Inspection. A complete copy of the procedure is kept in the ‘Supporting People’ document. Service users, relatives and staff have access to a shortened version of the procedure, a copy of which is on display in the home. MacIntyre Care has produced the procedure in picture format, called ‘Service User guide to making a complaint’, a copy of which is kept on individual service users’ files. The manager said that no complaints have been received since the last inspection. CSCI have not received any complaints about the home. MacIntyre Care have produced an Adults Protection procedure, a copy of which is kept in the home. Also available was a copy of the government guidelines ‘No Secrets’. Shared spaces complement and supplement service users’ individual rooms. Service users have the specialist equipment they require to maximise their independence. The home is clean and hygienic. The Commission considers Standards 24, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 24/25/26/27/28/29/30 A safe and comfortable standard of accommodation is provided for the service users. There is enough shared space to meet service users’ need EVIDENCE: The home is a domestic type building that provides service users with a wellmaintained, comfortable and homely environment. Bedrooms are individually decorated and furnished, hoists/lifting aids are also provided to assist service users with mobility problems. One bathroom, two separate toilets and one shower with toilet are provided for use by service users. Both the bathroom and shower have lifting equipment to help service users with mobility problems. The doors to these areas have locks that will ensure the privacy of service users. Service users have access to communal areas which includes a dining room in the kitchen, lounge(which has recently been partially redecorated) and a recently erected conservatory at the back of the home. A section of the garden has been secured and flower beds raised which will enable service

Service users benefit from well supported and supervised staff. The Commission considers Standards 35 the key standard to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 31/32/33/35/36/ Staff are employed in sufficient numbers to meet service users’ needs. Service users are cared for by appropriately trained and supervised staff. EVIDENCE: The staffing rota showed that there are normally two staff on duty during the day/evening and one waking plus one sleeping in staff during the night. These numbers can include the registered manager. MacIntyre Care provide on-going training for staff including NVQ in care, this will help staff further improve their skills in caring for service users. Six staff have achieved NVQ Level 2/3, three staff are due to commence NVQ and one member of staff is an internal NVQ assessor. Staff spoken with said they receive supervision and support from the manager which ensures they are aware of their responsibilities in caring for the service users. The home has received feedback, via questionnaires, from relatives on how they are doing. Some of the relatives comments included staff ‘are very friendly, enthusiastic and motivated’ and are ‘good advocates for service users’ ‘are willing to learn’.

JUDGEMENT – we looked at outcomes for standard(s) 37/38/42/43 The arrangements for managing the home are satisfactory. The health, safety and welfare of service users underpins the overall management of the home. EVIDENCE: Staff confirmed that the management approach encourages input from service users, relatives and staff on the services offered. They also said the manager offers leadership and direction to ensure the assessed care needs of service users are met. In discussion with the manager she confirmed that she is in the process of completing NVQ Level 4 in management and is also doing the Registered Managers Award. The staff spoken with said they receive support and supervision from the manager. MacIntyre Care have provided a comprehensive range of policies and procedures, copies of which are kept in the home and are available to staff. These and other records have been provided to ensure the service users’ rights and best interests are protected.

A record was available of monthly health and safety audits carried out by the home manager.
