Date of last inspection 19.5.05 Brief Description of the Service: Manor Court is a large home situated in a residential area of Efford, a short distance from the local shopping centre. The home is a purpose built, 3 storey building with accommodation provided on the 1st and 2nd floors: a passenger lift provides access to all floors. All thirty-seven bedrooms are single and selfcontained, providing a small kitchen area and an en suite shower room. The home provides care and accommodation for up to 37 residents for reason of old age, physical disability or dementia. This is an overview of what the inspector found during the inspection. The inspection was undertaken over 3 days. All visits were unannounced and took place on 14th and 20th October and 3rd November 2005. The inspection was undertaken over a 3-week period to monitor the quality of care being provided at a time when there were staff shortages. The inspector spoke to a total of 30 residents, one visitor, toured the building on each occasion and examined care records and the documentation relating to staff recruitment. This report also includes the findings of an unannounced visit made to Manor Court on 14th September 2005 in response to a complaint made to the Commission regarding staff shortages. What the service does well: What has improved since the last inspection? What they could do better:
Care staff must receive training in issues relating to the care of older people with dementia. Care plans must be updated to provide a description of the residents’ current care needs and residents must be involved in the review of their plan. Quality Assurance process must be expanded to include consultation with the residents. Staff must not work a waking night shift after having worked an afternoon shift. The intended outcomes for Standards 1 – 6 are: 1. 2. 3. 4. 5. 6. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 3, 5 Thorough and comprehensive systems for admission allow service users and their relatives to be confident that their needs can be met. EVIDENCE: One newly admitted resident described how she had been able to visit Manor Court before making a decision to move in. A pre-admission assessment had been undertaken for this resident. The intended outcomes for Standards 7 – 11 are: 7. 8. 9. 10. 11. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 7, 8, 9, 10 Residents’ health and personal care needs are being met. Residents are treated respectfully. EVIDENCE: Those care staff identified at the previous inspection as demonstrating a lack of understanding of the care needs of older people and particularly of those with dementia, are no longer employed at Manor Court. Residents said that the staff were very kind and caring and, despite the staff shortages, they felt very well cared for. One resident who had recently returned from hospital following a serious illness described how well he is being cared for and the effort the care and catering staff were making in supporting him with his changing dietary needs. Although this information was written in the daily care notes accessible to all care staff the care plan required updating. One newly admitted resident’s care plan was partially completed: a clear description of her care needs was written in the daily care notes. Medication is stored safely and the records are well maintained. The controlled drug register had been changed for a type that prevents tampering and the balance of controlled drugs was checked and found to be accurate. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 12, 15 Meals are nutritious and varied. Leisure and social activities have been planned to encourage resident involvement. EVIDENCE: During this period of staff restructuring, the Registered Manager has taken the opportunity to review how leisure activities are offered to residents. Musicians are invited to Manor Court monthly and care staff facilitate activities following consultation with residents: those identified for the forthcoming week included a clothes faire. Care staff said that they were confident that more frequent activities would be provided once staffing arrangements were stabilised. Residents said how much they enjoy their interaction and conversation with the staff. Meals are varied and nutritious: residents said that supper is now offered each evening. Residents are provided with food items to enable them to prepare snacks and drinks in their own rooms. JUDGEMENT – we looked at outcomes for standard(s) 16, 18 Residents are confident that concerns and complaints are treated seriously and that they will be protected from abuse. EVIDENCE: The recent actions of the Registered Manager and her staff team have demonstrated that they will not tolerate disrespectful behaviour towards residents. Residents said that the atmosphere in the home had improved greatly over the past month and they were confident that their concerns have been dealt with appropriately. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 24, 26 The service users live in a pleasant, well-maintained home that is comfortable and warm and which provides sufficient facilities to meet their needs. EVIDENCE: The home is purpose built, well maintained and meets the needs of residents with physical disabilities. All communal rooms and bedrooms were pleasantly decorated and furnished and residents and visitors said that it was always very clean. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission considers Standards 27, 29, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 27, 29, 30 Residents are cared for by motivated and caring staff. EVIDENCE: Residents said that the quality of care continues to improve and that they feel very well cared for. Those staff identified as rude and abrupt at the last inspection are no longer employed at Manor Court. Care staff have been supported by Agency staff to maintain adequate staffing levels. At the visit made to Manor Court on 14th September, it was found that one member of care staff was working a waking night shift after having worked an afternoon shift: this is not good practice and an immediate requirement notice was issued to ensure this did not happen again. The dementia care training identified to occur following the previous inspection was postponed due to the staffing issues: the training booklets had been purchased and the Deputy Manager said that the training would be planned once all the newly appointed staff start their employment at Manor Court. Those care staff consulted said that they felt very well supported and had confidence in the Registered Manager, Ms Attridge and the newly appointed Deputy Manager. The files of a number of newly appointed staff were examined and these contained the required documentation including written references and Criminal Record Bureau checks. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 31, 32, 33, 36, 38 The Registered Manager and her staff team strive to ensure that the residents live in a well managed home that provides a stimulating and safe environment that respects and protects their rights. EVIDENCE: Ms Attridge, Registered Manager is qualified to NVQ 4 in Care and has a Diploma in Management. She has demonstrated her ability to deal with difficult issues that affect the well-being of residents and has worked in co-operation with the residents, their families and the Commission to address the issues of poor staff attitude raised at the last inspection. Staff confirmed that they are well supported and have had supervision throughout this difficult time: due to the nature of the staffing difficulties, supervisions have not always been recorded. The Quality Assurance system does not yet include formal consultation with residents. Health and safety practices were satisfactory ensuring that equipment was maintained in good working order and staff had received training in fire safety, first aid and manual handling.
