This inspection was carried out on 13th December 2005.

The inspector made no statutory requirements on the home as a result of this inspection and there were no outstanding actions from the previous inspection report.

What the care home does well

All of the residents, who were spoken to on the day of inspection, spoke very highly of the Manager, the staff members, the food and their bedrooms. The building is maintained safely, and all the records required by the Inspector on the day of inspection, were found to be in order. The registered providers of Manorfield are very much in evidence, and there is a really happy atmosphere throughout the home.

What has improved since the last inspection?

Since the previous inspection the new extension is now fully operational with attractively furnished en-suite bedrooms. There is a purpose built walk in shower room, which is fully equipped to assist residents with their bathing needs. Staffing levels have been increased and National Vocational Qualification level 2 and 3 training is ongoing. The Registered Manager Ms Buckley has recently completed her Registered Manager`s Award and Mrs Leach has registered to commence training in the new year. An activities organiser has been employed to broaden the scope of activities available to the residents.

What the care home could do better:

At present everything is satisfactory.

The owners are Mr and Mrs R Buckley (seniors), Mrs M Buckley, Mrs L Leach and Miss Buckley.Miss C Buckley and Mrs L Leach are actively involved in the day-to-day running of the establishment. Miss C Buckley RGN is the Registered Manager. The establishment is purpose built, set in its own grounds in the rural setting of Earnley. All rooms except one are single and they are at ground floor level. Extensive works have recently taken place to increase the size of the home and improve the communal facilities for the residents. Eight en-suite bedrooms have been added, of which four are now occupied. The final stage of work is to re-site the kitchen and extend the lounge dining room. The home will then be able to accommodate twenty-two persons. This was an unannounced inspection, the second of a minimum of two statutory visits that an inspector must make to each care home during the course of a year. The first inspection was undertaken earlier in the year. At this inspection the Inspector looked at standards that were not looked at during the previous inspection, as well as any outstanding issues from the last report. The reader is advised to look at the reports of both inspections for a full picture of the home. Where standards have not changed the report remains the same. Miss Buckley and Mrs Leach assisted the Inspector throughout the inspection. During this inspection the Inspector examined records of care being provided to residents, as well as records of any accidents, issues, concerns or complaints, to make sure that the residents at Manorfield were being taken care of. The inspector met new residents who were all very positive with regard to the care they were receiving and said that the Manager and the staff members were “really kind” and” very caring”. The Inspector would like to thank everyone who cooperated with her on the day of this inspection. There were no requirements following the previous inspection and all standards were met or exceeded. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 5 The Registered Manager ensures that prospective residents are invited to visit the home and wherever possible move in for a trial stay before making a decision to stay permanently. EVIDENCE: A recently admitted resident told the inspector that she and her family had visited the home before deciding to live there. A Statement of Purpose and Service Users Guide were provided to ensure she had all the relevant information about what to expect from the home. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 10 Residents said they felt the staff at all time treated them respectively. EVIDENCE: Each resident has their own door key available to protect their privacy. The inspector was pleased to see that staff knocked on resident’s doors and awaited an invite before just walking in. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): These standards were not assessed on this occasion. At the previous inspection these standards were met in full. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 16 Residents and their relatives or representatives are confident that their complaints will be listened to and taken seriously. EVIDENCE: Manorfield has a complaint’s procedure that is made available to everyone. There were no complaints recorded at the time of the inspection. The registered providers consult daily with each resident to ensure their needs are being met. Service users have sufficient and suitable lavatories and washing facilities. Service users have the specialist equipment they require to maximise their independence. Service users’ own rooms suit their needs. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 19,20,21,25,26 Residents live in a safe, well-maintained environment and have access to safe and comfortable indoor and outdoor communal facilities. Residents have sufficient and suitable lavatories and washing facilities and access to the specialist equipment they require to maximise their independence. The home has been extended to include a further eight en-suite rooms, new laundry, kitchen and extra spacious shower room. The lounge and dining room are currently being extended. EVIDENCE: Several building contractors were working in the home on the day of this unannounced inspection. The management and staff are doing their utmost to keep the disruption to a minimum. The building complies with the requirements of the Fire Officer and Environmental Health Officer. When the final stage of the building work is complete Miss Buckley has agreed to confirm in writing to the Commission for Social Care Inspection, in order that the certificate can be amended. The furnishings and lighting are of a high standard, which residents said they liked. Pipe work and radiators are guarded or have safe low temperature surface to prevent risk of burns to residents. Standards of hygiene and cleanliness through out the home are very good. Service users’ needs are met by the numbers and skill mix of staff. Service users are in safe hands at all times. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 28 29 & 30 Residents are in safe hands at all times. A robust recruitment procedure is followed to ensure the right type of staff are employed to care for vulnerable people. Staff have the necessary skills and confidence to perform their role in a professional manner. EVIDENCE: The Manager informed the Inspector that all staff members had received induction training together with fire safety, manual handling, first aid and adult protection. In addition the Inspector was told that all staff members who administer medication were soon to receive up date training. New staff are confirmed in post, following completion of a satisfactory Criminal Records Bureau check. A new member of staff said he was currently receiving induction training and was looking forward to attending a training course in food hygiene. Staff records are kept up to date and staff sign to confirm any training received. The inspector was informed that soon 50 of the care staff will be trained to National Vocational Qualification level 2. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 33,38 Policies, procedures and practices are regularly reviewed in light of changing legislation and following good practice. The registered providers do their best to act in the best interests of staff and residents. The Registered Manager confirmed that risk assessments are undertaken and recorded. EVIDENCE: Residents meetings are held and residents are encouraged to voice their opinions about the daily happenings of the home. The home has a Quality Assurance policy and feedback is sought from relatives and other interested parties. An audit of the homes findings will be examined at the next inspection. During the building works staff remain vigilant to ensure the health and safety of the residents is protected. Residents spoken with in private said that Manorfield was a comfortable home and that the staff were attentive.
