Old age, Not falling within any other category (6) Physical disability over 65 years of age (40) Date of last inspection 19/04/05 Brief Description of the Service: Merafield View is a purpose built Home situated in Plympton St Mary, on the outskirts of Plymouth Devon, with level access to local shops. The Home is registered to care for a maximum of 40 service users of either gender over the age of 65 years of age with physical disability/frailty or illness requiring nursing and/or personal care. The Home is arranged on 3 floors. Most accommodation is on the lower ground or first floors and the main communal areas are at ground floor level – all floors can be accessed by a passenger lift. There is a lounge and dining room and a sun lounge on the lower ground floor. The home is owned and administered by Stagecare Ltd. This is an overview of what the inspector found during the inspection. This unannounced inspection took place over 2 hours and 45 minutes and was conducted with the manager. The focus of the inspection was to assess the core standards not assessed at the last inspection in April and to assess the impact that the recent change of ownership has had on the home. Service Users were observed generally throughout the inspection but not spoken to directly as they were engaged in activities, seeing visitors or resting. Evidence for the report was collected from a tour of the home, discussion with the manager and the regulation 26 (monthly visit by the provider) report provided to the CSCI from November 2005. Although some hold open devices have been fitted in the home one is still needed on the laundry door. The manager is aware of this and will order one. The intended outcomes for Standards 1 – 6 are: 1. 2. 3. 4. 5. 6. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 1 Information and opportunities to visit the home are given to people to help them to decide if they want to live at Merafield. EVIDENCE: The manager said that the Statement of Purpose is still not accurate as it has not been updated since Stagecare Ltd took over ownership of the home in September 2005. She said that prospective clients come to look around the home when possible to help them make a decision about whether they want to move in or not and this had happened within the last few days. The intended outcomes for Standards 7 – 11 are: 7. 8. 9. 10. 11. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 10 Interactions between staff and residents promotes and protects patients privacy and dignity. EVIDENCE: A tour of the home confirmed that staff treat the clients with respect talking to them in an appropriate manner, knocking on doors before entering and ensuring their privacy when giving care. There is a public telephone in the home for anybody to use and clients can have a private phone fitted in their room if they wish (at their own expense). Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 19, 23, 25 and 26. The standard of the environment within the home is good providing clients with attractive and homely place to live. The overall quality of the furniture and fittings is good providing a safe environment. The home is clean and hygienic. EVIDENCE: The home changed ownership in September 2005 and since then the providers have bought many new adjustable beds and 2 profiling beds that help with the care of higher dependency clients. New lifting and handling equipment has been provided and specialist pressure relieving equipment is now provided by the company, following a risk assessment. During the recent change of ownership the local fire service and environmental health department confirmed the home complies with their requirements. Redecoration of the home and some of the rooms is also underway. There are bathrooms and toilets throughout the home designed to meet the needs of the client group and some rooms benefit from en–suite facilities. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 28 Clients needs are met by a satisfactory mix of nurses, carers and ancillary staff who are trained and competent to do their job. EVIDENCE: The manager is a first level nurse (RGN) who is supported by a number of trained nurses and care staff. Many of the care staff are qualified to NVQ level 2 or 3 and there is ongoing training for staff that includes statutory training and training in subjects relevant to their specialist area. A number of staff were attending a lifting and handling update in the home during the inspection. The care staff are supported by domestic, catering, administrative and maintenance staff. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 38 The systems in place in the home generally promote and protect the health and safety and welfare of the clients and staff. EVIDENCE: Following the last inspection a requirement was made that a qualified first aider was on duty at all times in line with the regulation. The manager said that all the trained nurses have attended a first aid course in order to comply with the requirement. Certificates were examined during the inspection as evidence of this. Some hold open devices have been fitted to fire doors that are regularly kept open. One still has to be fitted to the door to the laundry.
