Conditions of registration: None Date of last inspection 02/12/04 Brief Description of the Service: The home is situated in the Preston area of Paignton and is easily accessible by public transport. It is set in private grounds with a pleasant sheltered garden. The home overlooks the sea at Paignton and is approached from a driveway, where parking is available for several cars, to the right of the building. It occupies a position that overlooks Torbay with panoramic views across to Brixham. The home is registered to provide accomodation to a maximum of 30 people of either gender, over the age of 65 years requiring nursing and/or personal care because of physival frailty, illness or disability. A registered nurse is on duty at all times supported by a team of Health Care Assistants. The home has a range of equipment and adaptations to cater for the needs of the residents. This is an overview of what the inspector found during the inspection. The inspection took place over 6 and a half hours and was announced. Information was received from the registered provider before the inspection as was written feedback from 3 residents and 3 visitors/relatives. A full tour of the home took place and personal records of 3 residents and 2 staff were inspected. The inspector spoke to 2 members of staff on duty and 20 of the residents as well as the registered owners, manager and administrator. What the service does well: What has improved since the last inspection?
A digital telephone system has been introduced in the home - this enables residents who do not have a private telephone system to be able to speak on a portable phone wherever they are in the home. It also means that staff can communicate with each other without having to run around the home and waste valuable time. The system is also linked to the nurse call system, which decreases the disruption to residents caused by noisy buzzers. All beds with the exception of 1 (the resident prefers a divan and is not assessed as requiring an adjustable bed) are now height adjustable. An additional 3 specialist mattresses have been purchased to benefit residents at high risk of skin damage due to pressure.

All clients requiring them have their own sliding sheets to enable them to turn in bed with assistance from staff; and all clients who are non weight bearing have been provided with their own hoist sling. The intended outcomes for Standards 1 – 6 are: 1. 2. 3. 4. 5. 6. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 1,2,3 People are provided with sufficient information about the services and facilities to make an informed decision about their admission to this home. The admissions procedure enables the staff to make a professional judgement about how the needs of individuals will be met. EVIDENCE: The home’s Statement of Purpose is easily available in the reception area of the home along with a copy of the latest inspection report. Each resident’s room had a copy of a useful information guide. The inspector examined the personal records held within the home on behalf of 3 of the residents (10 ) these contained copies of contracts and terms of residency. The inspector found that information about the individuals’ health, personal and social care had been sought and most residents are visited in their current settings to enable the registered nurses to make a professional judgement about how needs will be met before offering the individual the opportunity of admission.

The intended outcomes for Standards 7 – 11 are: 7. 8. 9. 10. 11. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 7,8,9,10 The health care needs of residents are regularly reviewed and appropriate action is taken to meet those needs. EVIDENCE: The home provides a comprehensive care planning process for residents based on continual assessment of their needs. Three care plans viewed during the inspection were fully completed and appropriately reviewed. The review processes in the home also includes an annul review of the placement with the Service Users and their representatives. Feedback from 3 residents indicated that they feel well cared for, are treated well by the staff and that their privacy is respected. Documentation provided evidence that residents are visited by General Practitioners, chiropodist, dentist and opticians and that they are referred to specialist community and hospital health specialists when necessary. The inspector was informed that an aroma therapist also visits and residents enjoy both the therapeutic and relaxing benefit of this as well as the social contact. Written feedback from 3 relatives/visitors indicates that they are kept informed of important matters affecting their relative/friend and all were satisfied with the overall care provided.

Storage was safe and orderly the home uses a monitored dosage system, which is well organised and easily audited. The intended outcomes for Standards 12 - 15 are: 12. 13. 14. 15. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 12,13,14,15 Social activities are organised and meet the needs of most residents. Meals are nutritious and balanced and offer a healthy and varied diet for residents. The visiting arrangements are flexible and meet the needs of residents and visitors alike. Residents are encouraged to make choices about how they live their lives within the community of this home. EVIDENCE: One person living at the home said that they did feel bored sometimes; others said that the activities arranged and offered in the home suited their needs and preferences. The monthly activity calendar displayed on notice boards in the home included an exercise class, an outing/trip, arts and crafts sessions and ‘mental stimulus’, which the inspector was told was quizzes. The residents told the inspector they enjoyed the quizzes, 1 resident has taken up a new hobby of painting since his admission to the home. The home employs a member of the care staff to specifically organise and offer the social activity programme. The written feedback received from 3 residents indicated that 2 of them were satisfied with the social activities provided in the home. The feedback about food was positive all of the residents spoken to said how good it was; on the day of inspection lunch was served, residents were offered ham, new potatoes, cauliflower cheese and mixed vegetables, one non meat

Resident’s weights are regularly monitored and records seen by the inspector indicated that 3 residents had gained weight (not excessively) since their admission. The people living in the home told the inspector they were happy with the visiting arrangements, written feedback from 3 relatives/visitors indicate that they feel welcomed into the home and are able to visit their relative/friend in private. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 16,18 People are safe living in this home and know who to speak to if they are dissatisfied. EVIDENCE: The home has a comprehensive complaints procedure. This was available to residents in the ‘useful’ Guide found in their private accommodation and displayed in the reception area of the home. Those residents’, who were asked, said that they knew who to complain to if they had any concerns and felt confident that these would be dealt with in a sensitive way by the staff. The inspector also examined the Protection Of Vulnerable Adults policy and procedural manual; the 3 residents who provided written feedback to the commission indicated that they feel safe living in the home and know who to speak to if they are unhappy with their care. Written feedback was also received from 3 relatives/visitors, 2 of these indicated that they were aware of the complaints procedure all 3 said they had not needed to make a complaint. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 19,26 The providers maintain an attractively presented environment for residents and staff that is well maintained and safe. EVIDENCE: A tour of the home provided evidence that the providers maintain an attractively presented environment for residents and staff that is well maintained. Resident’s rooms contained personal items of furniture and ornaments and pictures. All of those spoken to said they liked their rooms, some were pleased that their rooms had sea views others were equally satisfied with rooms leading onto the enclosed attractive garden. The home appeared well equipped to meet the needs of those residents identified with moving and handling risks and disabilities that affect their capability to bathe. Specialist mattresses were seen in place for those residents requiring them as were height adjustable beds.

Safety notices were displayed throughout the home and written information given to the Commission by the provider indicates that fire equipment is regularly maintained and tested as are water temperature checks. The central heating system was checked in February by an approved contractor. The lift was last inspected in September 2004. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission considers Standards 27, 29, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 27,29,30 The procedures for the recruitment of staff are robust and offer protection to people living in the home. The number and skills of staff available in the home meets the needs of those living in the home. EVIDENCE: The residents told the inspector that there were enough staff on duty, they said they ‘worked hard’ and were ‘very caring’ –‘nothing is too much trouble’. The staff spoken to also advised the inspector that they felt there was sufficient numbers of staff on duty and said they had access to training and development. The home continues to be accredited with Plymouth University and the Nurses & Midwifery Council to facilitate placements for student nurses. The inspector examined the personnel files of 2 recently employed members of staff these provided evidence that the recruitment process is fair, equitable and safe. The inspector examined training records these indicate that most staff receive regular training/updates however the records did not provide duration of training sessions/courses and therefore the inspector was unable to ascertain that all staff receive at least 3 paid days training/year. Written feedback from 3 relatives visors was received 2 indicated that there is always sufficient numbers of staff the other indicates that ‘mostly’ there are enough. The home has the Investors in People Award.

Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 31,32,33,35,37,38 The home is being managed properly, there is clear leadership, guidance and direction to staff to ensure residents receive consistent quality care. EVIDENCE: Residents, visitors and staff made positive comments about the management team in the home saying they felt comfortable approaching them with Issues. Communication systems are regular through staff handovers, but formal meetings are not held, the provider said these had been held in the past but had not been found to be productive. Questionnaires/surveys are performed and include residents, visitors and staff information from these is collated generally the feedback is positive but where issues are raised actions are taken to improve the service. The inspector examined the training records of the registered manager these confirmed that the manager attends regular training to update her skills and practise.

Equipment is regularly maintained and staff are trained on health and safety matters during their induction and updated regularly. Plans to replace carpets, curtains and bedding provide evidence of reinvestment in the business as does the recent increased provision of specialist mattresses and adjustable bedding. All of the records seen during the inspection were clear, well maintained and secure. One resident asked for some money which was being held in the office on her behalf, the administrator brought the records and the balance to her - the inspector witnessed that the money and records were held in individual envelopes and were seen to be checked and correct.
