Conditions of registration: Date of last inspection 12th December 2005 Brief Description of the Service: Normanby House provides personal care and accommodation for up to 25 older people and is owned and managed by Anchor Trust. The home is a semi-detached property, which consists in part of a converted dwelling house and purpose built accommodation. Resident accommodation is over three floors and there is access to all levels by a vertical passenger lift. The home has a small patio area with flowerbeds and a summerhouse. There is a car park to the rear of the home. The front access to the premises in Belgrave Crescent is close to local amenities such as shops and public transport. This is an overview of what the inspector found during the inspection. The evidence used to inform this report was obtained from the information documented in previous inspection reports, a pre-inspection questionnaire completed by the registered manager of the service, a site visit, completed survey forms and discussions with 5 service users, 3 staff and the registered manager. Prior to the inspection seven survey forms were sent to service users and one was returned, three survey forms were sent to GPs and one was returned and three forms were sent to care managers and two were returned. This unannounced inspection took place on the 22nd January 2007, commencing at 9.30am. A number of records were inspected including service users’ assessments, care plans, medication and health and safety records. A tour of the home was made with the registered manager and staff were observed interacting with a number of service users. Weekly charges ranged from £317 to £425 on the date of the inspection. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 2 and 6 Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. The assessments prior to admission are comprehensive and provide informed decisions about moving into the home. EVIDENCE: An examination of four service users files showed that comprehensive assessments are made prior to admission to the home. An assessment is made to ensure that the environment of the home and the skills of the staff team can meet the needs of all individuals admitted. A member of the senior staff team visits all potential service users in their own home or in hospital and completes a written assessment to ensure that the home meets the needs of potential service users. The intended outcomes for Standards 7 – 11 are: 7. 8. 9. 10. 11. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 7, 8, 9 and 10 Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. There are good systems in place to ensure that the health needs of service users are met. EVIDENCE: Care plans for four service users were examined at random. All files seen contained detailed care plans, care plan reviews, risk assessments and an action plan to minimise any risks identified, dietary preferences, nutrition monitoring, details of any falls and a dependency assessment. The files were up to date and in good order. Reviews are held as necessary and at least once per month. Review documents were available on individuals’ files. All service users spoken to stated that they were treated with respect by the staff and that they were allowed choice to do what they wanted when they wanted. Some service users are self-medicating and staff use a monitored dosage system to assist service users with their medication when they are unable to care for the medication themselves. Medication and medication records checked were kept securely, were in order and up to date. Six identified members of staff who have received medication training are responsible for the

All service users are registered with a GP and access to specialist health services are arranged through their surgeries. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 12, 13, 14 and 15 Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. Visiting arrangements are flexible allowing service users to maintain good and regular contact with family and friends. Dietary needs of service users are met. EVIDENCE: An activity organiser has recently been appointed and she is at present discussing with service users what activities they would like including in the activity programme. At the present time a hairdresser visits, there is a monthly church service held in the home, a monthly coffee morning, two monthly cheese and wine evenings and an exercise class. Dial-a-ride is used to take service users who require transport to places of interest. Several service users can access community services themselves without any transport being provided. On the day of the inspection a service user stated “I’m just going to Whitby over the moors. I really like going through the moors. I usually go out everyday and just come back for my meals”. The menu clearly showed that service users had choice and only had to choose when they arrived for their midday meal. All service users spoken to stated that the food was very good and they could always choose what they wished to eat. Diets

JUDGEMENT – we looked at outcomes for the following standard(s): 16 and 18 Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. There are satisfactory complaints and abuse procedures in place and staff are aware of what action to take if any complaints or alleged abuse situations occur. EVIDENCE: There is a complaints policy. Service users have a copy and there is a copy displayed near the entrance hall to the home. Five complaints have been made during the past year. All the complaints have been dealt with within the timescale detailed in the policy. Service users spoken to stated that they would complain if they needed to direct to a member of staff on duty. The home has a copy of the North Yorkshire and York Vulnerable Adults Policy and staff spoken to knew what action to take in the case of a suspected abuse being reported. The Commission considers Standards 19 and 26 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 19 and 26 Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. The standard of the environment within the home is good providing service users with an attractive and homely place in which to live. EVIDENCE: The home is pleasant, homely and clean. The communal areas and some of the bedrooms have been decorated since the last inspection. There is a programme of refurbishing some bedrooms that is still ongoing. Work is undertaken on a bedroom as it becomes available and it is decorated, new carpets and curtains fitted. All service users spoken to stated that they were happy with their rooms. Many of the service users use their own room as a bed sitting room and only use the dining room at mealtimes. Several service users stated that they had their breakfast in their own rooms. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 27, 28, 29 and 30 Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. The standard of vetting and the recruitment process is good and the service users receive a good standard of care from the staff. EVIDENCE: The duty rota showed that there were two care staff and a member of the senior staff team on duty at all times during the day. Night staff had access to an on-call member of the senior staff team by telephone. There is usually a housekeeper on duty and there is a chef manager. Three staff records were examined. Full employment checks are made before a new member of staff commences work. The records showed that the necessary checks had been made. Staff records showed that training had been undertaken and staff spoken to confirmed this. A number of staff have obtained NVQ Level 2 in care and the senior team have also undertaken further training. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 31, 33, 35 and 38 Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. The management of the home is undertaken by a manager, deputy manager and senior carers. The service users are is safe hands. EVIDENCE: The manager of the home has been registered as the registered manager by the Commission for Social Care Inspection since the last inspection. She has been able to appoint a deputy manager. The home reviews its performance and has a quality assurance system that includes the views of service users on an annual basis. The organisation reviews the home on a regular basis using its own quality assurance programme. A number of service users deposit personal allowance money with the home and records are kept of income and expenditure. The health and safety records checked showed that proper
