Conditions of registration: none Date of last inspection 18/1/05 Brief Description of the Service: North Road Care Homes is a care home with nursing. Providing care for older people with enduring mental health problems. Care in the home is provided by Registered Mental Nurses supported by care staff. The home is owned and managed by Northumbria Nursing & Residential Care which specialises in providing services for people with mental health needs and problems. The home is situated in Gosforth in the city of Newcastle upon Tyne close to local shops and good public transport links. The building has mainly single bedrooms some of these having en-suite facilities. There are seperate bathroom and toilet facilities. There are a number of lounge and dining rooms. The home has two laundry rooms and a single kitchen, also a garden area to the interior. The philosophy of care is to support the residents in their activities of daily living and to provide for their physical and mental health needs. This is an overview of what the inspector found during the inspection. This inspection was announced and took place over 5 hours. The Inspector looked around some parts of the building 10 residents were spoken to and 5 members of staff. A number of the homes records were inspected. What the service does well: What has improved since the last inspection? What they could do better:
Some areas of maintenance work needs to be better fulfilled and met. Fire arrangements regarding not chocking open fire doors need to be strictly adhered to. Residents clothing needs to returned to their rightful owner after being laundered. Cleaning schedules in the kitchen must be kept and maintained.

The intended outcomes for Standards 1 – 6 are: 1. 2. 3. 4. 5. 6. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 3. No service user moves into the home without having had his/her needs assessed and being assured that these will be met. EVIDENCE: Four residents care records were inspected. These showed that good preadmission assessments had been completed in all four to a good standard. Gender of staff for personal care tasks was identified and being met. Residents social histories were well completed, also food likes, dislikes. The intended outcomes for Standards 7 – 11 are: 7. 8. 9. 10. 11. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 7,8,9,10. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the homes policies and procedures for dealing with medicines. Service users are treated with respect and their right to dignity is upheld. EVIDENCE: Four residents care records were inspected these showed a good range and variety of care plans had been written. There was evidence that these were being regularly evaluated and reviewed. Some use of latin abreviations should be discontinued otherwise these records were of a good standard. The records showed that GPs were regularly reviewing residents health care needs. Records for Dental, Optical and Chiropody services were also found to be satisfactory. The systems for the administration, storage, management and disposal of medicines were checked and found to be satisfactory. Records seen, show that staff are instucted to knock before entering residents bedrooms. Care records document that staff encourage residents to decide what clothes to wear each day. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 12,13,14,15. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family, friends, representatives and the local community. EVIDENCE: Records were inspected that show a Craft therapist visits the home every 2 weeks. At the last session collages were made which are on display in the home.Two recent mini-bus outings had taken place with trips to Rothbury and South Shields for 16 of the residents. In those residents care records that were seen individual social care plans were written and were satisfactory. A weekly activities programme for the residents was found to be satisfactory. Records showed that relatives were taking residents out for walks. A barbaque evening has been organised to take place in August. Visiting entertainers visit the home every month a singer is due on the 20th of July. Menus in the home were seen these showed that a good variety and choice of food is being offered to the residents. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 16,18. Service users and their relatives and friends are confident that their complaints will be listened to, taking seriously and acted upon. Service users are protected from abuse. EVIDENCE: Since the last inspection there has been one new complaint. This was investigated by the homes manager with a satisfactory outcome. The homes POVA policy was seen and is of a satisfactory standard. POVA incidents are dealt with appropriately. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 19,24,26. Overall service users live in a safe well maintained environment. However some matters identified in this report require to be addressed. Service users live in safe, comfortable bedrooms with their own possessions around them. The home is clean, pleasant and hygienic. EVIDENCE: In those parts of the building that were inspected the majority was being well maintained. However the following matters need to be addressed. A number of residents bedroom door closures were found to be too fierce in closing. Some corridoor fire doors were being chocked open. In a number of bedrooms window limiters were found to be inadequate. Most resident bedrooms that were seen were found to be nicely personalised and furnished. The treatment room, the storage cupboard doors here need repairing. The kitchen was visited and was found to generally clean and tidy. However records of the cleaning schedules for the kitchen were not being kept or maintained. A number of relatives responded in questionaires that they were happy with home services and facilities. One exception to this was the laundry, they described finding other residents clothes in their relatives wardrobes. This was checked and

In those parts of the home that were inspected they were found to be clean and hygienic and odour free. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission considers Standards 27, 29, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 27,29,30. Service users needs are met by the numbers and skill mix of staff. Service users are supported and protected by the homes recruitment policy and practises. Staff are trained and competent to do their jobs. EVIDENCE: The homes duty rosters were inspected and showed the following average staffing in the home each day; Am, 2 Qualified and 11 care staff, Pm, 2 Qualified and 11 care staff, Nights, 1 Qualified and 5 care staff. These staffing levels are in line with the assessed needs of the residents and are satisfactory. Two recently recruited staff files were inspected. Two written references had been received and all appropriate checks had been carried out, job contracts and job descriptions were satisfactory. Staff training records were inspected. These showed that good levels of statutory and client centred training was being carried out. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 33,35,38. The home is run in the best interests of service users. Service users financial interests are safeguarded. The health, safety and welfare of service users and staff are promoted and protected. EVIDENCE: Minutes of relatives and staff meetings were inspected. These were being regularly held and agendas were appropriate for the client group. Evidence was seen of the manager acting upon issues raised by relatives. Residents financial records were scrutinised. These showed evidence of regular expenditures occurring with two staff signatories for all transactions. Money balances were checked and found to be correct. Health and safety audit records were inspected the last of these was carried out in January 05 and was satisfactory. The homes Fire log book and Accident book records were seen and both of these were satisfactory. Standard 19 19 Regulation 23.2(b) 23.2(b) Requirement Residents bedroom door closures all need to be checked to ensure they are not too fierce in closing. Window limiters in resident bedrooms in the older part of the building need checking and some need to be re-newed. The storage cupboard doors in the treatment room require to be repaired. The manager of the home to instruct staff to ensure that residents clothing is placed in the correct bedrooms. The homes cook must keep records of cleaning schedules for the kitchen area and equipment and ensure they are maintained. Corridor fire doors must not be kept chocked open.
