What the care home does well

Pre-admission assessments are completed to a very good standard and is the basis for good care planning for the residents. Recruitment procedures are wide-ranging and well carried out. This means that staff employed in the home are competent to meet the diverse needs of the residents. The manager of the home is very experienced in supporting people with mental health needs and has an open style of management. And works hard to ensure the residents receive the care that they need.

What has improved since the last inspection?

Major refurbishment in the home has greatly enhanced the living environment for all the residents.

The home is owned and managed by Northumbria Nursing & Residential Care, which specialises in providing services for people with mental health needs and problems. It is situated in Gosforth in the city of Newcastle upon Tyne close to local shops and good public transport links. The building has mainly single bedrooms some of these having en-suite facilities. There are separate bathroom and toilet facilities and, a number of lounge and dining rooms. The home has two laundry rooms and a single kitchen, also a garden area to the interior. The philosophy of care is to support the residents in their activities of daily living and to provide for their physical and mental health needs. Fees range from £383 to £547 this does not include the costs for hairdressing, toiletries and chiropody. The Visit: An unannounced visit was made on 22/05/07 During the visit we: • • • • • • Talked with people who use the service, relatives, staff, the deputy manager & visitors. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 3 & 6. Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. Comprehensive assessments are carried out before and after admission to ensure that peoples needs can be planned and properly met. Detailed information is available to help people make informed choices about the service before moving in. The home does not provide intermediate care. EVIDENCE: Four residents pre-admission assessment documents were read all of these were comprehensively completed to a good standard. Copies of the homes service user guide, statement of purpose and complaints policy are given to all new clients and their relatives. She said if she had any concerns she would approach the manager or staff and was confident these would be addressed appropriately. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 7, 8, 9 and 10. Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. The standard of care planning is good. Residents are included in the process and are sufficiently detailed to guide staff practice. This ensures that residents are supported in a way that they prefer and in a sensitive manner. Healthcare needs are well met and adopt a multi-agency approach. This helps residents to stay healthy. Robust medication systems are in place to make sure that residents are not put at risk. EVIDENCE: There was a good range of assessments completed, which included: • assessments for nutrition, • continence, North Road Care Homes DS0000045428.V338232.R01.S.doc Version 5.2 Page 11 • moving and handling, • behavioural and mental health, • social, physical health needs, • pressure sore risk and, • risk assessments. All assessments are regularly evaluated. Care plans were well written with monthly evaluations of these being recorded. However, social care plans do not make best use of the information from assessments and are therefore, not as detailed in this area. Reviews of the residents care needs are being held at regular intervals with their relatives in attendance these records showed evidence of good consultation with relatives taking place. Records of visiting professionals showed healthcare needs are being fully addressed. A visiting podiatrist said the home had provided her with good information about the clients she had come to see and spoke well of the home. The systems for the storage, administration, receipt and disposal of medications were checked and were satisfactory. Two residents controlled medication stock balances were checked and were correct the records for these were also satisfactory. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 12, 13, 14 & 15. Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. Residents are encouraged to rebuild skills and retain independence, and are fairly well supported to make choices within a risk assessment framework. The residents have good opportunities to take part in a variety of leisure pursuits and interests, which help them maintain links with the local community and keep and develop social skills. Mealtimes are flexible to suit individual preferences and lifestyles. Residents are given plenty of choice and sensitively supported to eat meals where they have specific needs. EVIDENCE: A relative spoken to said she could visit the home at any time and was always made welcome by the staff who were supportive and helpful. She said meals in the home were good as she often came in at mealtimes and assisted her mother to eat. The weekly activities programme for the residents showed a fair North Road Care Homes DS0000045428.V338232.R01.S.doc Version 5.2 Page 13 range of activities provided. A resident was spoken to who said she goes out each week with a member of staff to a café she used prior to her admission. Another resident goes out to church each week. A resident had recently celebrated her 100th birthday the Lord Mayor of Newcastle had visited the home to mark the occasion. Father O’ Conner visits from the Holy Trinity church and brings Communion for the residents. A mobile cinema visited the home in October the residents selected the film to watch. A 14 seater mini-bus has been booked for June, July and August for residents weekly trips out to local places of interest. A resident who is capable has the key to their own bedroom door she prefers to keep her room locked during the day. Records showed evidence of residents having flexible rising and retiring bedtimes also that staff respected what clothes each resident liked to wear. Menus showed a good range and choice of food being provided. Two residents spoken to said the food in the home was good. The lunchtime meal was observed this was cauliflower cheese or spaghetti and cheese omelette with strawberry gateaux with cream or fresh fruit salad, cold or hot drinks were provided with the meal. Dining tables were set with table cloths, placemats serviettes and condiments. Staff were available to offer support during the meal and interacted well with residents. A satisfactory complaints procedure is in place and clearly displayed to ensure that complaints are dealt with effectively and to the satisfaction of the complainant. Clear protection procedures are in place to protect residents from risk of harm. EVIDENCE: No complaints have been received by the home since the last inspection visit. Members of staff were aware of the procedures to follow for the protection of vulnerable adults. Copies of the homes complaints procedures are given to residents and their relatives. A relative spoken to said they would be confident if they had any concerns that these would be properly addressed by the manager. Service users live in a safe, well-maintained environment. Service users have access to safe and comfortable indoor and outdoor communal facilities. Service users have sufficient and suitable lavatories and washing facilities. Service users have the specialist equipment they require to maximise their independence. Service users’ own rooms suit their needs. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 19, 22, 24 & 26. Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. The building is clean and generally well maintained. Major refurbishment has resulted in a more comfortable setting for residents to live. EVIDENCE: Since the last inspection visit the home has had a major refurbishment. A new dining area and conservatory has enhanced the environment in the residential side of the home. Improvements were made to toilets and bathroom areas. Additional lounge and quite room facilities are provided for nursing residents also an additional bathroom and toilet. The number of en-suite facilities for residents has also increased. The inner courtyard was paved with raised flower bed areas and new garden furniture bought, all of these developments have greatly improved the living environment for the residents. Most of the North Road Care Homes DS0000045428.V338232.R01.S.doc Version 5.2 Page 16 bedrooms were well decorated clean and tidy and personalised according to individual taste. The window pane in bedroom 16 was cracked and the window restrictor in bedroom 17 wasn’t effective and the room needs redecoration. The standard of décor and the carpet in room 19 were showing signs of wear and tear. The kitchen was very clean and tidy with plenty of food stocks. The splash back to the sink was worn. Cleaning schedules were in place and were satisfactory. A bathroom on the ground floor the bathwater was tested and was satisfactory but the door to this room is badly damaged. A sitting bathroom scales was also badly deteriorated. Lounge areas in the home are decorated and furnished to a good standard. Overall, the living areas for residents is of a good standard there were no unpleasant smells or odours. The Commission consider all the above are key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 27, 28, 29 & 30. Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. Sufficient numbers of staff are in post to meet the diverse needs of residents. Opportunities for training are good. This enables staff to learn new skills to better support residents in all aspects of their lives. Robust recruitment procedures are in place which help to prevent risk of harm to residents. EVIDENCE: Staff duty rosters were checked these showed good levels of staffing are being maintained. Staff on duty on the day of the inspection were as rostered. The homes staffing establishment is good. Two staff members said opportunities for training is good also that supervision and appraisal is regularly structured. Records in the home show that 80 of staff are trained to National Vocational Qualification level 2 or above. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 31, 33, 34, 35 & 38. Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. The manager has an open style and is clearly present in the home to give clear direction and support to staff. This ensures that the service is led by the needs and wishes of residents. Quality assurance systems are in place but are still being developed. This will help to shape the quality of the service and ensure that it is run in the best interests of the residents. Good systems and practises are in place that help to ensure residents and staff are safe from risk of harm. North Road Care Homes DS0000045428.V338232.R01.S.doc Version 5.2 Page 20 EVIDENCE: The manager of the home is a very experienced Registered Mental Nurse with many years of working with older people with enduring mental health needs. Records in the home show that she puts the needs of the residents first. Staff training records are centred around the residents needs. Regular audits of the homes environment and care records are carried out to a satisfactory standard. Relatives questionnaires have been sent out positive feedback about home services has been received. Relatives and staff meetings minutes were read these were appropriate and showed actions taken to resolve issues raised at these meetings. Regular audits of health and safety matters are properly structured and the records of these are good. The homes fire log book and accident book records are completed to a good standard. Residents financial records these are well structured there is evidence of regular expenditures all transactions have two staff signatures, receipts are kept and a clear audit trail can be followed. Staff in the home spoken to are knowledgeable about control of infection matters and procedures and these are followed. Utility certificates for the home were checked and all of these were found to be satisfactory.
