Conditions of registration: Total number of service users not to exceed a maximum of 30 Date of last inspection 16 December 2004 Brief Description of the Service: Old Roselyon is a detached 400-year-old Manor House that was converted for nursing home use in 1973 and has subsequently been extended. The home is registered to provide care with nursing for up to 30 service users. Service user accommodation is provided on two floors. The first floor is serviced by a stair lift. Single and double rooms are available. All rooms have washbasins and some have en-suite facilities. The property has pleasant well tended gardens and there is adequate car parking. This is an overview of what the inspector found during the inspection. This was an unannounced inspection organised to look at the care standards in the home. I arrived at 1.30pm and the inspection finished at 5.30pm. The registered manager and staff assisted by providing information contained in records and discussion. Various service users and visitors were met at the home and discussed a range of topics about living in the home. A partial tour of the home was completed through the inspection process. The intended outcomes for Standards 1 – 6 are: 1. 2. 3. 4. 5. 6. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 1, 2, 3 and 5. The registered person has provided a statement of purpose and service users guide that details the facilities and services available within the home. The registered manager will assess all prospective service users prior to admission to ensure that the home can meet their care needs. EVIDENCE: The registered manager has completed the statement of purpose and service user guide. A requirement is made regarding this standard and the registered manager agreed that the contracts would be issued with the service users guide or kept on service users files to comply with Regulation 17-Schedule 4(8). The senior carer discussed recent admission procedures occurring at the home and stated that prospective service users and relatives are invited to the home and given appropriate information. The records for recent admissions were inspected and the assessment process and liaison with other agencies was detailed and recorded appropriately. The intended outcomes for Standards 7 – 11 are: 7. 8. 9. 10. 11. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 7, 8 and 9. The health care needs of service users are met with evidence of multi disciplinary working taking place on a regular basis. EVIDENCE: Care plans are generated from a comprehensive assessment and incorporate all aspects of health and personal care needs. A detailed checklist is used to ensure new care plans are thorough and that appropriate risk assessments are also included. Reviews take place regularly and signatures obtained whenever possible. The nurse on duty stated that a qualified nurse took responsibility for the generation of 4 service users care plans and the communication required with other staff. All service users are registered with local General Practitioners and have access to routine health checks. Qualified nursing staff take responsibility for service users nursing needs and care staff provide support for service users who require assistance with personal care. Occupational therapists, community nursing and other specialist staff visit the home as needed. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 12 and 13. Service users exercise their choice to enjoy the privacy of their own rooms or join other service users in communal areas for company or meals. Service users maintain contact with visiting relatives and friends at the home. EVIDENCE: Service users were met in communal areas or their own rooms. Service users bring items of furniture and possessions to personalise their bedrooms. The home has flexible routines that allow for personal preferences and dependency levels. The nurse on duty stated that the majority of service users required nursing care. Service users expressed approval that regular visits are received from their relatives. JUDGEMENT – we looked at outcomes for standard(s) 18 The registered manager ensures staff have an appropriate understanding and information to protect service users from abuse. EVIDENCE: The registered manager has produced relevant documents and has discussed adult protection issues with the care team at staff meetings. The complaints procedure is available to service users through the service users guide. A code of practise for staff has been introduced and staff contracts preclude staff from assisting in the making of or benefiting from service users wills. Service users have access to safe and comfortable indoor and outdoor communal facilities. Service users have sufficient and suitable lavatories and washing facilities. Service users have the specialist equipment they require to maximise their independence. Service users’ own rooms suit their needs. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 19, 20, 21 and 22. The registered person is investing in the home to improve the facilities and provide a comfortable, safe environment for service users. EVIDENCE: Accommodation is provided on the ground and first floors, with a stair lift to assist service users. Overhead tracking and hoists are provided in 2 bathrooms on the ground and first floor. The registered providers have recently installed a new hot water and central heating boiler. The registered manager stated that low surface temperature radiators are included in the development plan for the home. Sufficient bathroom, shower and toilet facilities are provided for service users. The home provides 2 separate large and bright lounge areas and a dining room. One lounge area is designated for service users that may smoke. Service users were observed to relax in the comfort of the communal areas or the privacy of their rooms during the day. Service users may choose to take their meals in their rooms, dining room or lounge areas.

Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission considers Standards 27, 29, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 27 and 28 Care staff have a good understanding of service users support needs and sufficient staffing are provided to meet these needs. EVIDENCE: The home is supported by a stable team of staff, sufficient in numbers and skill mix to meet the needs of the service users. In addition to the registered manager there is usually 1 qualified nurse and 5 care assistants per shift throughout the day. At night there is 1 waking qualified nurse and 1 waking care assistant. All staff left in charge of the home are at least 21 years of age. The home was organised to provide for the welfare of service users. Service users spoken to expressed approval of the attitude of care staff, they did not have to wait for attention from staff and that they felt safe at the home. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 31, 32 and 38. The registered persons are providing clear and effective leadership and management to maintain a safe and comfortable home for service users. EVIDENCE: The registered manager is a registered nurse who has achieved NVQ level 4 in care and management. He has extensive experience of managing a home. The registered manager is open and approachable and communicates a clear sense of direction and leadership. There are clear lines of accountability within the home. The home has a health and safety policy and the registered manager has a good knowledge surrounding the legislation regarding health and safety. Risk assessments relating to environmental issues are in place, boilers and appliances are serviced regularly and fire precautions are maintained.
