What the care home does well

The home has now got a stable staff team, who are enthusiastic and work well together. There is good teamwork and it was noticeable that this is having a good effect on the care of residents all of whom looked well cared for and were content.

What has improved since the last inspection?

There has been very good improvement in the homes environmental standards and there is an on going programme of repair and refurbishment. The home is more comfortable and homely. Staff receive regular supervision and support and told the inspector that it is a pleasure to work at the home. Meals have improved, the home now employs a qualified chef, and residents told the inspector that the food is good and that there are always choices.

As stated by the pharmacy inspector the home needs to review the medication policy.

The service should employ a suitably qualified and experienced manager who is registered with the Commission for Social Care Inspection The service is registered to accommodate a maximum of 22 service users to include up to 20 service users in the category OP (older persons 65 and over ) and 2 service users in the category PD (physical disability) who must be 60 or over.. The registered manager must achieve the Registered Managers Award by April 2006 26th July 2005 3. Date of last inspection Brief Description of the Service: Palmtree Lodge care home is situated within Cleveleys town centre, only a short walk from shops and close to the sea front. The home provides residential care for twenty-two persons on ground and first floor levels. All bedrooms have ensuite facilities and comply with minimum space requirements. There are two lounges and a dining/lounge area. A passenger lift provides access to the first floor. There is a small front patio area with benches for residents to enjoy watching the daily comings and going in the town. New owners have recently taken over control of the home and have a registered manager to manage the home. This was an announced inspection and took place over a five hour period from 10.00 am. Care records were looked at and all the eleven residents living at the home at the time of the inspection were spoken to. Comments received from the residents are, “ I am extremely happy with the care I receive.” “The staff are helpful and kind and do a wonderful job. The care is excellent”. “Its very good here and I am very happy”. “If I want other food they will go out and buy it for me.” The inspector toured the building, spoke to individual staff, had discussion with management and consulted records and policies and procedures. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 9 and 10 The Pharmacy inspector visited the home on 6th January 2006 and reported that the medication handling systems have improved over the previous months evidenced by the medication administration records and the general handling systems of the home. Staff are clearly more comfortable with medicines administration and the management continues to support the manager on making improvements. All outstanding requirements have been addressed with only minor improvement now needed, the manager is advised that the medication policy seen on the day of inspection requires further development to ensure it is fully reflective of medication handling practices within the home. Care given by staff respects the privacy and dignity of residents. EVIDENCE: The pharmacy inspector found that medicines administration and handling is now usually double-checked by another designated carer and this has resulted in clear, accurate and detailed records. Only minor inaccuracies were noted on the records. The receipt of medication was now accurately recorded for all Palmtree Lodge DS0000063455.V280384.R01.S.doc Version 5.1 Page 10 medication entering the home; this was an outstanding requirement from the last inspection that has now been fully met. Handwritten records and all general medication administration were now routinely double-checked this had resulted in an overall improvement in the accuracy of all the records. The medication policy had been updated from the last pharmacist inspection and it was recognised that it contained useful medicines handling information. However it was recognised and this was discussed at the time of inspection that some of the details within the policies were not relevant to the “residential” setting. Of particular note was the disposal of medicines policy which was not relevant as it detailed what is required in a nursing home which is clearly not relevant to this home. The manager is advised to review these policies and ensure they are detailed, relevant and staff are fully aware of their contents. This policy, once reviewed and re-written, should be sent to the pharmacy inspector as part of the “action plan”. All service users spoken to were very complimentary about how staff carry out their duties, they said personal care is provided in a dignified and private manner. One relative said that they are very happy with care given. Service users and staff said medical examinations and treatment are carried out in the privacy of service users rooms. The homes policies and procedures require staff to deal with service users at all times with respect and dignity, this is covered in staff induction and training. Individual discussion with staff also confirmed this to be the case. All staff on duty were spoken to separately and said that there is a marked improvement in the atmosphere in the home and they thoroughly enjoy working there. In a recent satisfaction survey carried out by the home one visitor said, “I enjoy coming to the home and I play the piano for residents, staff are always friendly and helpful.” Palmtree Lodge DS0000063455.V280384.R01.S.doc Version 5.1 Page 11 Daily Life and Social Activities The intended outcomes for Standards 12 - 15 are: 12. 13. 14. 15. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 14 and 15 Those residents able to manage their own finances are encouraged to continue doing so. Appropriate records are maintained of the home’s involvement in holding monies for residents. The homes routines are flexible according to the needs and wishes of residents. The rooms of service users are furnished and fitted with personal items and possessions. Access to personal records and information is enabled in accordance with the Data Protection Act 1998 The home provides residents with a good varied diet with choices available. EVIDENCE: Residents assessments were seen, these include establishing whether a resident needs help with managing their finances, most residents families deal with their finances and the home looks after personal allowances. These are kept securely and good records are maintained of money going in and out. Residents always have access to their money for any items they require. This was confirmed in conversation with residents. On being admitted to the home residents can bring items belonging to them in order to make their rooms personal. This was noted to be the case when going around resident’s rooms. Resident’s files showed that they or their representatives sign the assessments and care plans and that they have access to information kept on them. Residents said that there has been a marked improvement in meals provided by the home. The inspector was able to sample a meal; this was of good quality and nourishing. The home has employed a qualified chef to prepare meals. Records of meals provided show that there is a good variety of food available to residents with alternatives always available. One resident told the inspector, “If I want other food they will go out and get it for me”. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 19 There has been a big improvement in general maintenance of the home. EVIDENCE: Management of the home has carried out a full audit of maintenance in the home and has replaced or repaired broken items. There is a good reporting system in place to ensure repairs are carried out when necessary and records are kept of this. The inspector carried out a tour of the home and found that there has been a marked improvement in all rooms used by residents. In a recent satisfaction survey one visitor commented, “ There has been a big improvement in the environment, we are glad to see that refurbishment is taking place and that planned improvement will be accomplished.” Residents said they find the home is more homely and comfortable. Service users’ needs are met by the numbers and skill mix of staff. Service users are in safe hands at all times. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 28 and 30 All staff receive are trained to carry out their duties. The home places high importance in training staff. EVIDENCE: Staff records showed that all staff receive a full induction programme, which includes all aspects of the homes philosophy and training in the care of the elderly. The induction is in line with requirements of the Skills To Care (formally TOPPS) programme. Of the eleven staff employed by the home there are six qualified to NVQ2 in care and two are currently doing this. One member of staff has NVQ3 and two are doing this. Staff on duty were spoken to individually and confirmed that they receive a full induction and are provided with good training to carry out their duties. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 35, 36 and 38 There are good systems in place to ensure resident’s monies are safely handled. All staff receive regular supervision sessions from senior staff. There are Health and Safety policies and procedures in place, which ensures residents and staff health and welfare is protected. EVIDENCE: The inspector looked at personal allowances held for residents, records were up to date and showed exactly what money is kept for individual residents. Residents said they have access to their allowances when they require to pay for items. All monies are kept securely in the office safe. Staff spoken to say they receive good support in to enable them to do their jobs. Staff receive have health and safety training, records showed this to be the case. Moving and handling training is given for all staff and records showed they have all completed this training. Staff have first aid, food hygiene, and infection control training. Records showed that all safety checks take place, including fire, electricity and hot water.
