What the care home does well

The home has a group of caring staff who relate well to residents and carry out their tasks ensuring dignity for residents. Residents spoken to told the inspectors they are happy with how staff care for them.

What has improved since the last inspection?

The home has put in place better assessment and care plan systems to ensure residents are appropriately placed and better cared for. Environmental standards are improving and a programme of redecoration and refurbishment is on going.

What the care home could do better:

The manager needs to ensure that new systems for assessment and care are fully implemented. Formal supervision of staff needs to be implemented and records kept of this. A full audit of bedroom furniture needs to be completed and replacements for broken cabinets and wardrobes purchased.

The service should employ a suitably qualified and experienced manager who is registered with the Commission for Social Care Inspection 2. The service is registered to accommodate a maximum of 22 service users to include up to 20 service users in the category OP (older persons 65 and over ) and 2 named service users in the category PD (physical disability). 3. Radiator covers must be fitted to radiators in service user rooms by 1 August 2005 4. The registered manager must achieve the Registered Managers Award by April 2006 Date of last inspection 24 December 2004 Brief Description of the Service: Palmtree Lodge care home is situated within Cleveleys town centre, only a short walk from shops and close to the sea front. The home provides residential care for twenty two persons on groung and first floor levels. All bedrooms have ensuite facilities and comply with minimum space requirements. There are two lounges and a dining/lounge area. A passenger lift provides access to the first floor. There is a small front patio area with benches for residents to enjoy whatching the daily comings and going in the town. New owners have recently taken over controll of the home and have a registered manager to manage the home. This was an unannounced inspection and was carried out by two inspectors, Mr P Rooney and Mrs F Lacey over a five hour period. Information was gathered from looking at care files and case tracking the care of residents, looking at staff records, looking at policies and procedures, and discussion with residents, management and staff. Survey forms were received from relatives and residents. A full tour of the home was carried out. The inspectors focussed on issues arising from two recent complaints received by the CSCI regarding care delivered by the home and environmental standards. These complaints occurred prior to and during the homes transition to new ownership. The complaints were upheld and reports regarding them are available with this report. The inspectors were pleased to observe during this inspection that there have been marked improvements in environmental standards and good systems have been put in place to ensure continued monitoring of these standards. New assessments and care plans, including risk assessments are being put in place. These describe more clearly the care required for individual residents. The registered manager is currently doing the Registered Managers Award and the new owners are in daily contact with residents in the home and have an office in the home. Staffing has been improved and there is a new full time cleaner to ensure all parts of the home are kept clean. The inspector spoke to residents and a visiting relative about the care they receive, all felt their care needs are met and that staff are friendly and helpful. Care staff were interviewed and staff recruitment records examined. The inspector also looked at the homes policies and procedures and staff handbook. The manager and one of the owners assisted the inspection throughout. Feedback from all these sources was positive and showed that there have been on going improvements in the home. The inspectors did not inspect medication policies and procedures and have asked that the Pharmacy inspector carries out an inspection as soon as possible. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 3 A new assessment system has been introduced which clearly identifies care which is needed and includes risk assessments. EVIDENCE: The inspectors looked at the care files of five residents and noted that a new system has been put in place, which ensures a full assessment takes place prior to placement. These assessments clearly state the physical, medical and social needs of residents. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 7 and 8 Promotion of health and personal care is taken seriously and residents welfare and health is monitored. Some attention is needed to ensure residents social care needs are recorded in their care plans as these were mainly about health needs. At present not all residents files have been updated with the new care plans. The inspectors did not cover policies and procedures in relation to administration of medication as a visit by the pharmacy inspector will follow shortly. EVIDENCE: The care records of residents were examined and the inspectors spoke to residents, and their care files were examined. Residents told the inspectors that they are happy with the care they receive, also a visiting relative confirmed this and said that they had been consulted regarding provision of care. Social care needs including personal histories are to be included in the new care plans. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 12 and 13 Social activities are provided which are varied according to the needs of individual service users. The homes routines are flexible and welcoming to all individuals. Friends and relatives are made welcome when they visit the home. EVIDENCE: The inspector saw a varied programme of activities for residents, which includes entertainment sessions, outings, and visiting shops. Tai Chi sessions have been arranged, with a qualified person residents said they enjoyed this and it gave them opportunities to do gentle exercise. Those who are able go out, one resident uses an electric scooter to visit local shops. The atmosphere in the home was observed to be relaxed, residents told the inspector they are able to rise and retire when they wish and that mealtimes are flexible according to their needs and wishes. Meals may be taken privately if residents wish. One relative was visiting and told the inspector he was always made welcome and could see his relative in privacy. Relatives survey carried out confirmed that they are happy with the care the home provides. Service users and their relatives and friends are confident that their complaints will be listened to, taken seriously and acted upon. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 16 and 18 Arrangements for complaints are good and an effective whistle blowing policy ensures protection of residents. EVIDENCE: The home has a complaints procedure this was seen by the inspector. In discussion with residents and visitors the inspector found that they were aware of what to do if they had a complaint. They said that they found the manager and staff of the home approachable if they were concerned about anything. Information about the complaints procedure is available to residents and is contained in the service users guide. A system for recording complaints is in place and outcomes recorded. There have been two complaints made to the CSCI since the last inspection. The elements of both these complaints were upheld and reports regarding them are available with this report. The first of these complaints was made prior to the new owners taking over the home and the second was made in the middle the new owners taking charge. Since then the new owners have worked hard to improve standards at the home. There is a procedure available to protect residents from abuse including a whistle blowing policy. Staff interviewed demonstrated an awareness and understanding of these policies. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 19 and 26 The standard of furnishings and decoration needs improving. There have been marked improvements to the cleanliness of the home. The exterior of the home has been improved and external seating is provided at the front of the home. EVIDENCE: The inspectors carried out a full tour of the home and found that there is an on going programme of decoration in place. A system for checking repairs has been put in place staff are able to record in a maintenance book things which need attention. Items in this record are checked regularly by the owners. It was observed that some of the bedroom furniture is in need of replacement and some curtain rails need replacing. Service users needs are met by the numbers and skill mix of staff. Service users are in safe hands at all times. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission considers Standards 27, 29, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 27 and 29 The policy and procedures for the recruitment and training of staff are robust and provide safeguards, which protect residents. The number of staff on duty was sufficient to meet the needs of residents living at the home. EVIDENCE: The inspectors saw staff files including new staff, all contained the necessary recruitment forms and checks to ensure the protection of residents. These include an application form, two references a POVA and CRB check. Induction and training records were seen and showed all staff receive an induction and training in providing care required for residents living at the home. Records also showed that five staff have completed NVQ 2 and two are currently doing this qualification. These figures show that there are at least 50 of staff who have a qualification in providing care. Staff were interviewed and confirmed they receive induction and training, they felt there have been improvements since the new owners took over. The owners said that in order to further promote good practice they are going to implement an employee of the month award. Rotas examined showed that there are sufficient staff on duty to meet the needs of residents. Sufficient staff were observed to be on duty at the time of the inspection. A new cleaner has been employed to ensure all parts of the home are kept clean. This ensures care staff have more time in providing care. Residents spoken to told the inspector staff are caring pleasant and helpful. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 31,32,33 and 36 The new management within the home has brought in procedures which will help to provide leadership, guidance and continuity for residents and staff, who told the inspector that that they are aware of the management structure. Residents are consulted about how the home is run. There are Health and Safety policies and procedures in place, which ensures residents and staff health and welfare is protected. EVIDENCE: The Registered manager is in the process of completing NVQ level 4 in management. He has qualifications in management and has put management systems into place to ensure the good running of the home. Staff and residents spoken to said management was open an approachable. The manager and one of the owners is available to residents in the home on a daily basis, this was confirmed by residents, who described management as helpful and lines of accountability are clear within the home. There was evidence of consultation having taken place with residents. On the day of the inspection comments from staff, service users and visitors were positive about the home. Comment cards distributed to residents and relatives for the inspection were also positive. Systems are being put in place to ensure support staff and ancillary staff receive formal supervision. During this inspection there was no evidence that this has previously been done, and there were no records of current supervision having taken place. The inspector saw the homes health and safety policies and staff records showed they receive training in this and in moving and handling.
