Conditions of registration: 14.6.04 The staffing issues agreed by you on 28th April 2004 are conditions of this registration Date of last inspection 24/02/2005 Brief Description of the Service: Park Lane Care Home is situated in the residential area of Newport on the outskirts of Barnstaple. There are local shops and facilities within walking distance of the home and it is the only remaining care home which provides nursing in this area of Barnstaple. The home previously consisted of two detached houses that have been connected to make one. It is situated in its own grounds, consisting of lawns, trees and flowerbeds. There is a designated parking area to the front of the building. The home stands on three floors with lift access to all areas. Bedroom accommodation currently consists of mainly single rooms but the home does have a small number of shared rooms available if required. The home is registered to provide care with nursing for 27 older persons. The home accepts day care service users which are included in the overall number.The home changed ownership on 1st June, 2004 and is now owned by Waterfall Nursing Homes Limited. The home continues to undergo major refurbishment, modernisation and updating to meet the national minimum standards. This is an overview of what the inspector found during the inspection. This inspection was unannounced and was arranged as part of the normal programme of inspection. It took place over 4.75 hours. The inspector met or spoke with most of the residents, spoke with the manager, staff and a visiting relative. The inspector looked around the home and a number of records were looked at. The intended outcomes for Standards 1 – 6 are: 1. 2. 3. 4. 5. 6. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 3,6 No resident is admitted to the home without a full assessment having been carried out. EVIDENCE: Residents and their families are encouraged to visit the home prior to admission, although this is not always possible. Prospective residents have a thorough assessment carried out by the home before their admission and are assured whether or not their needs can be met. Assessments carried out by both health and social services and the home were seen in care plans. The home does not provide intermediate care facilities. The intended outcomes for Standards 7 – 11 are: 7. 8. 9. 10. 11. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 7,8,10 Resident’s health and social needs are assessed, documented and well met. Staff promote the privacy and dignity of residents in the home. EVIDENCE: There is a care plan in place for each resident. Three care plans were looked at and all contained clear, concise and up to date information that had been regularly reviewed. On the day of inspection, the inspector saw that all the resident’s health and personal needs were being met, were well cared for and were happy. Staff were seen treating residents with privacy and dignity and talked with residents in a kind and respectful manner. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 12,13,14,15 Dietary needs of residents are well catered for with a balanced and varied selection of food available that meets resident’s tastes and choices. Activities are limited and do not provide residents with the stimulation and recreational interests they would like. Resident’s benefit from visitors being welcomed into the home. EVIDENCE: The home employs an activities co-ordinator for 20 hours a week. Current activities on offer are limited and whilst one resident said staff take him out in his wheelchair, the majority said that they would like to do more in the home. During the inspection, the inspector felt that the modern music playing to high dependency residents was inappropriate. Residents confirmed that visitors are made to feel welcome in the home and offered hospitality whilst visiting. One relative said that the home was “friendly, welcoming and an excellent atmosphere”. Residents confirmed they have a choice of menu and they liked the food in the home. Special diets are catered for. The kitchen is to be totally refurbished as part of the ongoing refurbishment programme. Residents confirmed that they have a choice in their daily lives and there are few restrictions. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 16,18 The home has a robust complaints procedure and residents are confident that all complaints will be taken seriously. Arrangements for protecting residents from harm or abuse are satisfactory. EVIDENCE: The home has a complaints policy and encourages residents to use it if required. No complaints were raised during the inspection. Residents spoken with knew how and who to make a complaint to. Staff currently have abuse training in house which is continuous and ongoing. The manager is planning to attend the recognised Devon County Council Protection of Vulnerable Adults training as soon as a place is available. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 19,26 Residents live in an environment that is currently being updated and renovated to provide a high standard of living accommodation. EVIDENCE: The home continues to be developed, updated and modernised in a development programme. Communal areas on the ground floor (including the 2 lounges, dining room, entrance foyer and hall) have been completed to a high standard with quality fixtures and fittings. Residents who have been part of the transformation are proud of the way the new home looks. Staff are working very hard to ensure that the home is kept as clean as possible, under the difficult conditions arising from the ongoing refurbishment. 2 dedicated cleaners are employed who take a pride in the home and work extremely hard to keep it clean in a structured and organised way. Residents have access to the grounds both at the front and rear of the building. A sensory garden is planned for the near future.

When these are finished, they provide residents with a high standard of living accommodation. One resident thought his room was small, but this is a temporary arrangement until his own room is completed which is very large. There are plans to build a new purpose-built laundry room with modern equipment. The current one is considered unsuitable for long-term use. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission considers Standards 27, 29, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 27 Resident’s needs are met by the number and skill mix of the staff. EVIDENCE: The home was adequately staffed on the day of inspection. Residents confirmed that they were given the help and encouragement they needed – although some times of the day were busier than others and necessitated a short wait. Trained nursing staff and care staff are employed. The home employs a committed staff team who regularly work over and above their allotted hours to cover such events as staff absences, sickness, annual leave etc. The home does not have a high staff turnover. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 38 Reliable systems are in place to ensure the good health, safety and welfare of residents and staff. EVIDENCE: Safe working practices are employed and managed with risk assessments undertaken, particularly whilst the renovation works are taking place. Staff fire training takes place regularly. Records showed that all fire safety checks had been undertaken with the exception of the fire alarm system. The manager stated that she thought that this was simply an error in the recording and that the maintenance man had actually done all the checks. Devon Fire and Rescue had recently carried out an inspection on 21.6.05 and everything was satisfactory. The accident book was not available on the day of inspection. Hazardous substances are secured. Residents said they felt well cared for.
