This inspection was carried out on 7th October 2009.

It is an annual review prepared by CQC after examining previous reports and information from the provider. At the time of this report, CQC judged the service to be Good.

The inspector made no statutory requirements on the home as a result of this inspection and there were no outstanding actions from the previous inspection report.

If yes, what have they been: Date of last key inspection: Date of last annual service review (if applicable): Brief description of the service Paternoster House is a purpose built care home providing nursing and personal care for 40 older people. It is situated near to Cirencester town centre, and is managed by The Orders of St John Care Trust. The accommodation is purpose built, and is provided on two floors. A staircase and a shaft lift provide access to the first floor. Residents private accommodation is provided in single rooms on both floors, two of which have an en-suite facility. All rooms have a wash hand basin. Spacious and easily accessible toilets and bathrooms are conveniently situated around the home. Hoisting equipment and assisted bathing and showering facilities are provided, and throughout the home there is level access, grab rails and a resident call system. The home provides two spacious sitting rooms, a large lounge and dining area, and a small sitting room on the first floor also. Information about the home is available in the Service User Guide, called the Residents Handbook, which is issued to prospective residents, and a copy of the most recent inspection report is available in the home for anyone to read. The registered manager has left the home. Annual Service Review Page 2 of 5 Annual Service Review Page 3 of 5 Service update since the last key inspection or annual service review: What did we do for this annual service review? We looked at all the information that we have received or asked for since the last key inspection, which was on the 28th January 2008. This included: What the service has told us about things that have happened in the service; these are called notifications and are a legal requirement; Relevant information from other organisations; What other people have told us about the service. What has this told us about the service? The long-serving manager of Paternoster House retired at the end of last year, and there was a period of a few months when the post remained vacant until a replacement could be appointed. The person that has now been appointed is experienced, but has yet to be registered by CQC for the role. There have been two incidents when concerns about particular staff members practice were reported to us. The home took the appropriate actions at the time, and the staff disciplinary procedures were instigated. We received a number of notifications about residents sustaining pressure ulcers. When we contacted the home to discuss these we remained dissatisfied with the responses we received. We took the decision to consult directly with The Orders of St John Care Trust, and requested a full investigation into these incidents. An investigation was duly carried out, with actions and measures adopted to address the unsatisfactory circumstances and reduce the risks of them occuring again. These circumstances have caused us to have a degree of concern about how the home has been managed over recent months, and about how this has affected the outcomes for some of the residents living at Paternoster House. Consideration of this information has meant that we have decided to carry out a key inspection of the home before the 28th January 2010.
