What the care home does well

Service users have access to the medical services they need. Staff respect individual privacy and dignity. Service users are well supported to maintain their chosen lifestyle and maintain contact with their friends and families. Pendennis provides a clean, comfortable and homely environment for its residents. Odour control in the home is of a commendably high standard. Staff receive the training they need to do their jobs. The home is well managed.

What the care home could do better:

Medication issues were not reviewed at this inspection. The labelling of skin creams, identified as a shortfall at the last inspection will be reviewed at the next inspection.

Bedrooms are located on all three floors and a mezzanine level between the first and second floors. The home is situated in its own grounds, to which service users have ramped access. Within the home, all the bedrooms are single, and all but two have en suite bathroom facilities. The home has a passenger lift, stair lifts to the mezzanine level and a variety of aids and adaptations for physically disabled service users. Care at the home is provided for up to twenty service users, who are over 65 and who may or may not have a physical disability or dementia. At the time of this inspection, the home had been under the management of Mrs. Bull for 14 months Pendennis Residential Home DS0000062705.V262477.R01.S.doc Version 5.1 Page 5 SUMMARY This is an overview of what the inspector found during the inspection. During this inspection the inspector spoke with 8 service users, 5 staff and a visiting relative. Parts of the building were inspected including communal areas, the kitchen and 7 service users’ rooms. Mrs. Bull, the Registered Manager was not present though Mr. Bull was seen briefly at the beginning of the inspection. What the service does well: What has improved since the last inspection? Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 8 and 10 Service users can feel confident that their health care needs are being met by the home. Individual privacy and dignity is appropriately respected by the home’s staff. EVIDENCE: Service users confirmed that they were registered with the GP and had access to routine medical checks such as hearing and sight tests. Chiropody services are also available to service users. One service user spoke of a programme of ongoing treatment at the local hospital. All the service users spoken with were confident that their medical needs were being adequately catered for. Medication systems were not examined on this occasion and the outstanding requirement and recommendation have therefore yet to be reviewed. Service users all confirmed that they were treated respectfully yes staff and this was confirmed by the inspector’s observations. Individual privacy is respected. Staff were seen knocking on service users’ doors before entering and service users confirmed that medical examinations take place in their own rooms. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 12, 13 and 14 Service users are well supported to pursue the lifestyle of their choice and maintain contact with those who are significant to them. EVIDENCE: The inspection commenced at 9:30am. As the morning progressed, service users were seen pursuing their own routines with assistance as necessary. Some had risen early and were quietly reading newspapers, doing crossword or chatting. Others were in their rooms. A notice of forthcoming activities was seen posted in the dining room. Service users confirmed that routines were flexible and that they were able to exercise choices in respect of such matters as routine, and religious observance. One service user who is a lifelong vegetarian confirmed that this choice was both respected and appropriately catered for. Individual rooms contained personal possessions. Information on advocacy is prominently displayed in the home’s foyer for service users and their relatives. During this inspection, a number of visitors were seen in the home including two visiting relatives and two legal advisors for one service user. One service user was celebrating a birthday and received a telephone call and visit from family members. The inspector spoke with one family visitor. He expressed general satisfaction with the visiting arrangements. There were regular occasions on which he took his relative out. A service user confirmed that her family visit the home several times a week and were always made welcome. Service users live in a safe, well-maintained environment. Service users have access to safe and comfortable indoor and outdoor communal facilities. Service users have sufficient and suitable lavatories and washing facilities. Service users have the specialist equipment they require to maximise their independence. Service users’ own rooms suit their needs. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 20, 24 and 26 Service users at Pendennis benefit from homely and comfortable surroundings which are clean, hygienic and meet their individual needs. EVIDENCE: Pendennis is located close to local shops and services and ramped access is provided to the home’s grounds. Accommodation is provided over three floors with a mezzanine level between the first and second floors. A shaft lift is in place which can accommodate wheelchairs. Two stair lifts provide access to the mezzanine level. All service users occupy individual rooms and all but two have en-suite facilities. Furnishings and décor both in communal areas and in the individual rooms are comfortable, homely and well-maintained. All areas were warm with plenty of access to natural light. Individual rooms contained personal possessions, photographs and pictures which some service users discussed with the Inspector. Service users felt that their rooms contained all the things they needed and were adequately comfortable. This inspection was conducted at a relatively early part of the day. All areas of the home visited were clean and free from offensive odours. Odour control in the home is of a commendably high standard. The Laundry facilities, situated in an outbuilding, have cleanable walls and an impermeable floor. Hand washing facilities are available to staff in the laundry. Colour coded cleaning equipment was seen in use during the inspection. Service users’ needs are met by the numbers and skill mix of staff. Service users are in safe hands at all times. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 28 Service users can feel confident that staff involved in their care and support are receiving appropriate training. EVIDENCE: The staff on duty were comprised of a range of ages and experience. There is an ongoing training programme to meet national targets in respect of NVQ training. Induction training to national standards is provided which includes the policies and procedures unique to the home. Staff confirmed that they have access to health and safety training in such areas as moving and handling. Each staff member has an individual training plan which includes training needs identified through supervision. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 31 Pendennis is a home which is well managed for the benefit of service users. EVIDENCE: Mrs. Bull, the Registered Manager holds an NVQ level 4 in Care and the Registered Manager’s award. The inspection demonstrated that the home is run efficiently and for the benefit of service users. Service users spoke warmly of Mrs. Bull’s management of the home. Staff expressed general satisfaction with the ethos of the home which has developed since she assumed managerial responsibility and felt that they were well supported. During the inspection neither Mrs. Bull nor her husband / co-owner were present. Nonetheless, staff were clear about their roles and what was required during their absence. The home was generally well maintained and was providing a comfortable, clean and safe environment for service users. Mrs. Bull has maintained regular contact with the Commission to provide information required by regulation and to clarify areas of practice.
