What the care home does well

The home works hard to support residents` to access community and educational facilities. There is a calm and comfortable environment in the home with staff supporting residents` sensitively and professionally. The home has not had many changes in the staff team since the last inspection visit this allows the residents` to get to know the people who support them very well. The manager and the staff team are committed to providing a high standard of care and support, which meets resident`s assessed needs.

What has improved since the last inspection?

Since the last inspection visit the manager has started to review care plans and risk assessments. This work will improve the level and type of support offered to residents`.

What the care home could do better:

The home needs to continue to improve the information they hold about residents and make sure important information is kept in such a way that the staff team can access it easily. Also the information is clear and provides them with clear guidance and instruction on the best way to support residents. The home needs to look at the way it is involved in decision-making by residents` especially when they are spending their own money.

The home has five single bedrooms, a lounge/dining room and bathrooms on the ground and first floor. There is a courtyard type area at the rear of the home and off road parking for approximately three cars. The home has no passenger lift. There were 4 people living at 17 Penkett Road at the time of the visit. The inspection was unannounced and took approximately two and half hours. Residents were coming in and out of the home during the visit and the inspector spoke to the manager and two members of the staff team. Prospective service users know that the home that they will choose will meet their needs and aspirations. Prospective service users have an opportunity to visit and to “test drive” the home. Each service user has an individual written contract or statement of terms and conditions with the home. The Commission consider Standard 2 the key standard to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 2&4 The home’s assessment procedures enable them to offer placements to prospective residents’ on the basis they will be able to meet their aspirations and care needs. The home supports and encourages prospective residents’ to “test drive” the home prior to them deciding if the want to live there. EVIDENCE: The home has an assessment procedure with documentation that is completed to indicate prospective residents’ care and behavioural needs. At the time of the visit the home had one vacancy the manager told the inspector the process would be followed in detail. This would ensure the home could meet the individuals needs and they are would fit in well with the people already living at the home. Records from past admissions to the home show prospective residents’ are invited to visit on a number of occasions before they make the decision to move in. Service users know their assessed and changing needs and personal goals are reflected in their individual Plan. Service users make decisions about their lives with assistance as needed. Service users are consulted on, and participate in, all aspects of life in the home. Service users are supported to take risks as part of an independent lifestyle. Service users know that information about them is handled appropriately, and that their confidences are kept. The Commission considers Standards 6, 7 and 9 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 6,9 & 10. The home is improving and streamlining the care planning and risk assessment strategies this is having a positive impact on the level and type of support offered to residents. The home has good systems and facilities in place to ensure residents’ information is handled appropriately and sensitively. EVIDENCE: Since the last visit to the home the manager has begun to review the care planning and risk assessment strategies. He told the inspector he intended to streamline the information held about residents’ to ensure the most up to date and relevant information is easily accessible for the staff team. The inspector viewed a sample of files and discussed the planned changes and acknowledges they will be beneficial to both the staff team and residents. The manager told the inspector he had attended a training course regarding risk assessment frameworks and was currently reviewing all risk assessments and support plans. Penkett Road (17) DS0000018927.V287160.R01.S.doc Version 5.1 Page 10 The manager has implemented a new system to record daily information about residents’ to ensure the staff team are observing residents’ behaviour and mood while taking part in activities. This to help them to support residents’ to make informed decisions and choices regarding activities they appear to enjoy and gain practical skills from. At the time of the visit the manager and staff team where producing pen pictures about residents’. This is to help all members of the team including bank and agency staff get to know residents’ in a more holistic way. Residents’ information is held in secure facilities within a locked room. Examination of documentary evidence indicates the staff team document information about residents’ in a professional, sensitive and non-judgemental manner. Service users have opportunities for personal development. Service users are able to take part in age, peer and culturally appropriate activities. Service users are part of the local community. Service users engage in appropriate leisure activities. Service users have appropriate personal, family and sexual relationships. Service users’ rights are respected and responsibilities recognised in their daily lives. Service users are offered a healthy diet and enjoy their meals and mealtimes. The Commission considers Standards 12, 13, 15, 16 and 17 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 11,12,15,16 & 17. Links with the community are good and support and enrich residents’ social and educational opportunities. The home’s financial dealings with residents’ with regard payments for the home’s minibus is not open, transparent or equitable. EVIDENCE: The home supports residents’ to take part in a range of activities that are age and culturally appropriate. And seeks out opportunities for personal development such as colleges courses and individual therapies such as music and movement. Examination of documentation indicates residents’ are asked to make a financial contribution to the upkeep of the home’s minibus. However documentation such as the Statement of Purpose and the Service Users guide state ‘the service has an adapted vehicle that can be accessed by all the service users’. Records show some residents’ pay the contribution but others do not yet all use the minibus. The inspector advised the manager this situation was not equitable and should be reviewed. Penkett Road (17) DS0000018927.V287160.R01.S.doc Version 5.1 Page 12 It does not appear that relatives or independent advocates have been involved in supporting residents’ in the decision making regarding if contributing to the home’s minibus is best value for money. The staff team prepare residents’ meals with residents’ involved in meal preparation as much as possible. Service users receive personal support in the way they prefer and require. Service users’ physical and emotional health needs are met. Service users retain, administer and control their own medication where appropriate, and are protected by the home’s policies and procedures for dealing with medicines. The ageing, illness and death of a service user are handled with respect and as the individual would wish. The Commission considers Standards 18, 19, and 20 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 19, 20 & 21 The medication at the home is well managed promoting good health. Overall the home continues to meet the physical and emotional needs of residents’. EVIDENCE: The ongoing review of the care planning and risk assessment strategies will have a positive impact on residents’ physical and emotional health care needs. Examination of residents’ files, discussions with the manager, members of the staff team indicate the home works hard to support residents’ with their care and emotional needs. The inspector checked residents’ medications and found them to be maintained with accurate records being kept. During the visit the inspector spoke to the manager and members of the staff team about the issues of illness and the death of a resident. The home has good systems in place and the staff team demonstrates awareness for the need for sensitivity at these times. Service users feel their views are listened to and acted on. Service users are protected from abuse, neglect and self-harm. The Commission considers Standards 22, and 23 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 22 & 23 Arrangements for protecting residents are not satisfactory placing them at possible risk of financial abuse. The home acts proactively with regard to the management of complaints and concerns. EVIDENCE: The issue of residents’ paying a contribution for the upkeep of the home’s minibus without full information being disclosed and no independent advice being sought is raised as a concern in an earlier section of this report. The manager told the inspector protection of vulnerable adults training is mandatory within the organisation. The inspector discussed with the manager the need to ensure the home’s adult protection procedures dovetail with Wirral social services protocols. The home has a detailed complaints policy and procedure and had produced an easy read format with pictorial references for residents’ use. Service users live in a homely, comfortable and safe environment. Service users’ bedrooms suit their needs and lifestyles. Service users’ bedrooms promote their independence. Service users’ toilets and bathrooms provide sufficient privacy and meet their individual needs. Shared spaces complement and supplement service users’ individual rooms. Service users have the specialist equipment they require to maximise their independence. The home is clean and hygienic. The Commission considers Standards 24, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 24 & 30 The quality of the furnishings and decoration of the home is mixed providing in most parts a comfortable and safe environment for those living there. Most areas of the home were clean and tidy providing a safe and hygienic place for residents’ to live. EVIDENCE: The following information was gained at the last inspection visit and remains current at this visit: The inspector viewed the lounge, which was pleasantly decorated however the carpet was badly fitted by the door and could become a tripping hazard. The manager was advised to have it looked at and repaired as soon as possible. The inspector noted the sofa and chairs were covered in what looked like a coated plastic the inspector discussed with the manager how comfortable this would be for residents. The inspector viewed one bedroom and found the decoration to be worn and damaged with the carpet badly stained and untidy. Service users benefit from well supported and supervised staff. The Commission considers Standards 32, 34 and 35 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 32, 34 & 35 The organisation is committed to providing the staff team with training to enable them to support residents’ in the most appropriate way. The organisation’s recruitment practices protect residents’ from possible forms of abuse. EVIDENCE: The organisation that owns the home does not hold staff files on the premises for security reasons. However they have produced a pro forma that holds the information required by the Commission. With the understanding that the full records can be viewed at their headquarters if deemed necessary. A sample of records were looked at and the inspector found they held the required information, including confirmation that employment and medical references had been sought and POVA checks had been carried out. All support workers working at the home have gained NVQ level 3 and the organisation is committed to providing periodic specialist training to enable the staff team to offer the most appropriate support to residents’. The inspector observed members of the staff team supporting residents’ in a supportive, affectionate and sensitive manner. Service users are confident their views underpin all self-monitoring, review and development by the home. Service users’ rights and best interests are safeguarded by the home’s policies and procedures. Service users benefit from competent and accountable management of the service. The Commission considers Standards 37, 39, and 42 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 37, 39, 40 & 42. The owners are not being open and transparent in their dealings with residents with regard to the upkeep of the home’s minibus. The home works proactively to promote residents’ health, safety and welfare. EVIDENCE: The manager has been in post since the end of November 05 he has a wide range of experience in the social care field and has supported adults with a learning disability for approximately eight years. The inspector acknowledges there has been improvement in the daily information being recorded about residents’. And the ongoing streamlining of care plans and the reviewing of risk assessment information As detailed in earlier parts of the report the issue of residents’ contributing to the upkeep of the home’s minibus without involving independent advocates to ensure the contribution is in the best interests of residents’.
