Conditions of registration: 29 OP (N) and 1 named male service user under 65 years of age(N) Date of last inspection 13 December 2004 Brief Description of the Service: Riversdale Nursing Home is family owned by Mrs M Armstrong and Mrs K Armstrong-Shone. The acting manager of the home is Julie Cotgrave, a first level nurse. The home is registered to provide nursing care for up to 30 elderly people. The home is a three storey house set in secluded mature gardens, close to the shops and amenities within West Kirby. Two lounges and a dining room are located on the ground floor and overlook the rear gardens. Both single and shared accommodation is available and some have the benefit of ensuite facilities. A hairdressing salon is also provided.A selection of bathrooms and toilets are provided on all floors and assisted bathing facilities are available. This is an overview of what the inspector found during the inspection. The inspection was conducted over a period of seven hours in one day. During the inspection, service users, staff and relatives were spoken to, to obtain their views of the home and the service provided. Care records were inspected and were found to be extremely informative and detailed. These documents provide staff with all the necessary information to enable them to provide the appropriate level of care for each of the service users. Staff records were inspected and were found to contain all the necessary documentation to ensure that service users are protected and that staff are given appropriate training and supervision. A tour of the premises found that all areas were clean and bright and were decorated and furnished to a high standard. Plans are in place to improve the laundry facilities within the home which is due to commence within the next few weeks. Service users enjoy an active and varied programme of activities and commented that they really enjoyed these. What the service does well: What has improved since the last inspection? The laundry facilities at the home, whilst adequate, are to be improved. A new laundry facility is to be provided and it is hoped that this work will commence at the end of August 2005. The intended outcomes for Standards 1 – 6 are: 1. 2. 3. 4. 5. 6. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 1, 3 Comprehensive assessments are taken on prospective service users to ensure that the home can meet their individual care needs. EVIDENCE: The statement of purpose for the home has been reviewed and updated. A copy has been placed in each bedroom and further copies are available from the home on request. Pre-admission assessments are undertaken by the acting manager or one of the qualified nurses. This is undertaken to ensure that the home can meet the prospective service users’ individual care needs and to enable any necessary equipment to be provided prior to admission. The assessment involves gathering information from the service user, their family, GP, hospital staff and any other person involved in their care. The assessments seen during the inspection, of those service users recently admitted to the home were found to contain sufficient information for a plan of care to be prepared.

The intended outcomes for Standards 7 – 11 are: 7. 8. 9. 10. 11. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 7, 8, 9, 10 The care files are extremely detailed and informative to enable staff to provide the appropriate level of care and thereby ensure the protection of service users. EVIDENCE: A selection of service users care files were inspected. The files contained comprehensive care plans which gave full details of the service users health, care and social needs, together with information on how these needs will be met. Risk assessments were in place together with details on how the risks will be reduced or eliminated. The care plans are written in a language that all staff can understand and include individual preferences and choices. The files also contain detailed information regarding visits made to and by GP’s and other care professionals. The home contacts care specialists such as the Tissue Viability Nurse, Continence Advisor and others as necessary. Medications were found to be ordered, stored, administered, recorded and disposed of in accordance with the home’s policy and procedure. The storage area was seen to be clean and organised and no unnecessary or unwanted medications were held.

Evidence of this training was seen on the staff files. Personal care is given to service users either in their bedroom or in the bathroom as appropriate. Service users may use their own bedroom or in one of the communal areas as they wish. The office can be made available for private meetings. The use of a mobile telephone is available to enable service users to speak with friends and family in private. Mail is delivered unopened and service users are addressed by the name of their choice. Staff were observed to knock on doors and to wait for an answer prior to entering. Screens are provided in shared bedrooms to ensure that privacy and dignity are protected. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 12, 13, 14, 15 A high level of social activity takes place to provide service users with stimulation and social interaction. EVIDENCE: Service users spoken to during the inspection commented favourably on the home and on the care provided. At the time of the inspection, the activities co-ordinator was assisting a group of service users with flower arranging. Service users said that they really enjoyed the activities that take place and showed the Inspector the flowerbeds and hanging baskets in the garden that they had assisted with producing. A programme of activities is prepared and is displayed in the home. This programme is varied and incorporates both group and individual activities involving all service users who wish to participate. The activities co-ordinator is employed for twenty hours each week and assists service users with card games, craft, bingo and reminiscence activities. A group of service users have recently been on a barge trip and enjoyed a picnic lunch. A barbeque is planned for next month. Some service users were proud to show the Inspector some of the other items they had made during craft sessions and intend to sell these to raise money for outings.

Some service users go out to church and services are held in the home for those who are unable to, or choose not to go out. The home does not deal with any of the service users finances and so family members or an advocate is advised to act on their behalf. Service users are encouraged to bring in personal items and bedrooms were seen to contain pictures, photographs and items of memorabilia. It was evident that the staff strive to provide a homely environment for the service users. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 16, 18 The home has a satisfactory complaints system with evidence that the views of service users and relatives were listened to. EVIDENCE: A complaints procedure is in place and information on how to make a complaint is detailed in the Statement of Purpose. This information is also displayed on the notice board in the entrance area. No complaints have been received by the home since the last inspection. The home give all service users, relatives and visitors the opportunity to compliment the home as well as to complain and a high number of complimentary cards have been received and are displayed. Service users and relatives spoken to confirmed that they had never had cause for complaint, but would be confident in speaking to the manager or owner in the knowledge that their concerns would be addressed. Policies and procedures are in place to ensure the protection of the service users. All staff are required to sign to indicate that they have read, understand and will comply with these. Training has been given to all staff on the different types of abuse and of the action to be taken in the event of it being suspected. Trained staff have recently been given training on the Nursing and Midwifery Councils’ Code of Professional Conduct. Staff spoken to were able to demonstrate their knowledge of Adult Protection. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 19, 26 Recent investment has significantly improved the appearance of this home creating a comfortable and safe environment for those living there and visiting. EVIDENCE: Improvements continue to be made within the home to provide a pleasant environment for the service users. All bedrooms have now been provided with new furniture and rooms are decorated as necessary. New carpets have been provided in bedrooms and enhance the service users private facilities. All rooms on the second floor of the home are provided with en-suite facilities. Corridors have been provided with new lighting to further enhance the home. Communal areas are comfortable and are furnished and decorated to a high standard. A hairdressing salon is provided and is fully fitted with all necessary equipment, including a hairdressing basin. The hairdresser visits the home each week. The garden area is bright with seating and covered areas where additional seating has been provided. Service users said that they enjoyed the garden and were looking forward to the planned barbeque. Infection control policies are in place and it was evident that staff follow all procedures. Plans are in place to improve the laundry facilities within the home, and all laundry is handled carefully in the current laundry facility. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission considers Standards 27, 29, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 27, 29, 30 The staff have a good understanding of the service users’ support needs. This is evident from the positive relationships which have been formed between the staff and service users. EVIDENCE: The staff rota provides evidence that the home employs and deploys staff in sufficient numbers to meet the needs of the service users. A qualified nurse is on duty at all times, supported by care staff who have all been given training in the care of elderly people. All prospective staff are required to complete an application form prior to being called for interview. Two references are taken and checks are made through the Criminal Records Bureau and the Protection of Vulnerable Adults register to ensure the protection of service users. All new staff are required to complete a comprehensive induction training programme which is signed for by both the manager and the member of staff. Training continues to be given to all staff and it is expected that over 85 of the staff will hold NVQ qualifications by the end of 2005. In house training is given to staff as well as attendance at external training courses. Several of the staff have been attending a training course on an Introduction to Care Standards to give them greater insight into the standards and the reasons for the implementation of these. A record is held on staff files of all training undertaken. Staff records inspected were found to contain all necessary information together with evidence that appropriate checks have been made

Supervision is given to all staff on a regular basis and annual appraisals are due to commence. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 31, 33, 35, 38 The systems for service users consultation are good with a variety of evidence that indicates that service users’ views are both sought and acted upon. EVIDENCE: The acting manager of the home and one of the owners have now achieved an NVQ in management at level 4. The acting manager is a qualified nurse who also holds qualifications in the care of older people and in training and assessing. An application to register the acting manager has been submitted to the Commission for Social Care Inspection and is being processed The acting manager has an open door policy and is available to service users, relatives and staff. Service users and staff spoken to during the inspection confirmed that the manager is approachable and accessible.

Group staff meetings are held and also specific staff meetings i.e. qualified staff, care staff, ancillary staff. The Registered Person has implemented the ‘Blue Cross’ quality monitoring standard and audits are regularly undertaken. Recent audits have been made on care, catering, administration and housekeeping. The audits involve gathering the views of service users, staff, relatives and care professionals. The findings are summarised in the service user guide. The home is currently working towards achieving Investors In People accreditation. Policies and procedures are in place to protect the interests of each service user and these have recently been reviewed and updated. The home does not deal with any of the service users finances. Secure facilities are provided to store valuables and accurate records are held of these. All staff have been given training on health and safety and are required to read the policies and procedures relating to these. Safety certificates inspected were seen to be in place and up to date.
