What the care home does well

The home provides a high quality environment, with further improvements being planned. Staff are trained to a good standard and the owner and manager are continually looking to improve the service they provide. Records were found to be informative and up to date and all documentation is reviewed and updated on a regular basis. Service user consultation is good and the home has recently achieved Investors In People Accreditation.

What has improved since the last inspection?

Two new bedrooms and a purpose built laundry facility have been built since the last inspection. The programme of redecoration is almost complete and all areas of the home are bright and welcoming. A training programme has been prepared for all staff with specific training packs being issued to all staff.

The acting manager of the home is Julie Cotgrave, a first level nurse. The home is registered to provide nursing care for up to 30 elderly people. The home is a three storey house set in secluded mature gardens, close to the shops and amenities within West Kirby. Two lounges and a dining room are located on the ground floor and overlook the rear gardens. Both single and shared accommodation is available and some have the benefit of ensuite facilities. A hairdressing salon is also provided. A selection of bathrooms and toilets are provided on all floors and assisted bathing facilities are available. Well maintained and secluded gardens are provided with seating, fountain, gazebo and walkways. The inspection was conducted over a period of seven hours in one day. During the inspection, service users, staff and relatives were spoken to, to obtain their views of the home and the service provided. Care records were inspected and were found to be extremely informative and detailed. These documents provide staff with all the necessary information to enable them to provide the appropriate level of care for each of the service users. Staff records were inspected and were found to contain all the necessary documentation to ensure that service users are protected and that staff are given appropriate training and supervision. A tour of the premises found that all areas were clean and bright and were decorated and furnished to a high standard. The redecoration programme was well underway with only the lounge and dining room to be completed. Two new bedrooms have been built overlooking the garden and an application to register these rooms is currently being processed. An application to use two large bedrooms for double occupancy on request of the service users is also being processed. A new purpose built laundry facility has been built since the last inspection and provides all the necessary equipment. Service users enjoy an active and varied programme of activities and commented that they really enjoyed these. What the service does well: What has improved since the last inspection? Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 1, 3 Comprehensive assessments are taken on prospective service users to ensure that the home can meet their individual care needs. EVIDENCE: The statement of purpose for the home has been updated and contains all current information and is a bright, clear, easy to read document. A copy is provided in each service users bedroom and further copies are available from the home on request. A pre-admission assessment is undertaken on all prospective service users by the acting manager or one of the qualified nurses. A five page assessment is completed to ensure that the home can meet the prospective service users individual care needs and to enable any necessary equipment to be provided prior to admission. The assessment involves gathering information from the service users, their family, GP, hospital staff and any other person involved in their care. The assessment also identifies any specific equipment necessary to meet the service users needs and enables the home to obtain the equipment prior to the service users admission where necessary. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 7, 8, 9, 10, 11 The care files are extremely detailed and informative to enable staff to provide the appropriate level of care and thereby ensure the protection of service users. EVIDENCE: The care files for six service users were inspected. The files contained comprehensive care plans giving full information regarding the service user health, care and social needs, together with detailed information on how these needs will be met. Risk assessments were in place together full information on how the risks will be reduced or eliminated. The care plans include information regarding the service users individual preferences and choices to enable staff to meet the service users specific requests. Information is also held on the files of visits made to and by GP’s and other health care professionals. Details of action to be taken following these visits are recorded and appropriate changes are made to care plans as appropriate. The home also keeps a record of communications with family members and other relevant persons. Riversdale Nursing Home DS0000020919.V281973.R01.S.doc Version 5.1 Page 11 Medications were found to be ordered, stored, administered and recorded in accordance with the home’s policy and procedure. The storage area was seen to be clean and organised and no unnecessary or unwanted medications were held. Appropriate disposal arrangements were in place for unwanted medications in accordance with recent changes in legislation. The policy and procedure had been amended to reflect these changes. Risk assessments were seen to be in place for those service users who administer their own medications to ensure that they could do so safely. Policies and procedures are in place to ensure that privacy and dignity are maintained and training in this is given to all staff during the induction training programme. Evidence of this training was seen on the staff files. Personal care is given to service users either in their bedroom or in the bathroom as appropriate. Service users may use their own bedroom or in one of the communal areas as they wish. The office can be made available for private meetings. The use of a mobile telephone is available to enable service users to speak with friends and family in private. Mail is delivered unopened and service users are addressed by the name of their choice. Staff were observed to knock on doors and to wait for an answer prior to entering. Screens are provided in shared bedrooms to ensure that privacy and dignity are protected. The home is now using the ‘Integrated Pathway for the dying’ to keep comprehensive records for those service users who are accommodated in the end stages of their illness. This document involves the recording of a more specific plan of care and is completed by the staff at the home, doctors and any other care professional involved. The home has prepared a very tasteful booklet to give information to families following bereavement. The booklet is called ‘At this sad time’ and provides full information on persons to contact and actions to be taken. It also provides essential contact numbers and addresses to assist the families. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 12, 13, 14, 15 A high level of social activity takes place to provide service users with stimulation and social interaction to enrich their lives. EVIDENCE: Service users spoken to at the time of the inspection spoke highly of the home, the staff and the care given to them. Two service users showed the inspector the table centres they had been making in the activities sessions. A full programme of activities is arranged and is displayed in the home. The activities co-ordinator works with individual service users and groups to provide a high level of activities and social stimulation. A group of service users enjoyed a trip around Wirral prior to Christmas to see the lights on the houses. The owners and managers produced an ‘Old time music hall’ night or the service users and their families which was enjoyed by all those who attended. Entertainers visit the home on a regular basis and the service users held their own Christmas Fair. Relatives are encouraged to visit the home as often as they wish and are welcomed by the staff. One service user is taken shopping by the staff and one service user chooses to use the services of a local barber. A hairdresser visits the home each week and provides a full hairdressing service for all service users as they wish. The home does not deal with any of the service users finances and so family members or an advocate are advised to act on their behalf. Service users are encouraged to bring in personal items and bedrooms were seen to contain pictures, photographs and items of memorabilia. It was evident that the staff strive to provide a homely environment for the service users. Service users and their relatives and friends are confident that their complaints will be listened to, taken seriously and acted upon. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 16, 18 The home has a satisfactory complaints system with evidence that the views of service users and relatives were listened to. EVIDENCE: A complaints procedure is in place and information on how to make a complaint is detailed in the Statement of Purpose. This information is also displayed on the notice board in the entrance area. No complaints have been received by the home since the last inspection. The home give all service users, relatives and visitors the opportunity to compliment the home as well as to complain and a high number of complimentary cards have been received and are displayed. Service users and relatives spoken to confirmed that they had never had cause for complaint, but would be confident in speaking to the manager or owner in the knowledge that their concerns would be addressed. Policies and procedures are in place to ensure the protection of the service users. All staff are required to sign to indicate that they have read, understand and will comply with these. All service users are listed on the electoral register to enable them to cast their vote at elections. The home is not affiliated to any political party. Riversdale Nursing Home DS0000020919.V281973.R01.S.doc Version 5.1 Page 15 Training has been given to all staff on the different types of abuse and of the action to be taken in the event of it being suspected. Evidence of this training is held on the staff files. Qualified nurses have been given training on the Nursing and Midwifery Councils’ Code of Professional Conduct. The owner, acting manager and staff spoken to at the time of the inspection were able to demonstrate their knowledge of Adult Protection issues. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 19, 26 Ongoing investment continues to improve the appearance of this home creating a comfortable and safe environment for those living there and visiting. EVIDENCE: Improvements continue to be made within the home. Two new single bedrooms have now been built which overlook the garden. These rooms are bright and are furnished and decorated to a high standard. An application to register these rooms is now being processed by CSCI. The owners have also made application to register two of the large bedrooms for use as double rooms to enable married couples to be accommodated. These rooms may be used for shared occupancy on request as they are currently for single occupancy only. All bedrooms were seen to be decorated and furnished to a high standard. Arrangements were in place to redecorate the lounge and dining room in the week following the inspection and the wallpaper had been purchased for this purpose. New armchairs, dining chairs, curtains and table linen had been Riversdale Nursing Home DS0000020919.V281973.R01.S.doc Version 5.1 Page 17 ordered for the redecorated rooms and their delivery was awaited. The corridors were being inspected at the time of the inspection and it was evident that the service users had been taken into consideration whilst this work was taking place, resulting in minimal disturbance to them. The lounge and dining room will be redecorated overnight to avoid disruption to the service users. Three additional bedrooms have been redecorated since the last inspection and were bright and welcoming. The new laundry is now fully operational and attends to all linens and service users personal clothing. It was evident that great care is given when laundering service users clothing. The home was clean throughout and is maintained to a high standard. Infection control policies are in place and it was evident that staff follow all procedures. Service users’ needs are met by the numbers and skill mix of staff. Service users are in safe hands at all times. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 27, 28, 29, 30 Staff morale is high resulting in an enthusiastic workforce that works positively with service users to improve their whole quality of life. EVIDENCE: The staff rota provides evidence that the home employs and deploys staff in sufficient numbers to meet the needs of the service users. A qualified nurse is on duty at all times, supported by care staff who have all been given training in the care of elderly people. Additional staff are employed during the busy morning period to enable service users to rise and bathe at a time of their choosing. All prospective staff are required to complete an application form prior to being called for interview. Two references are taken and checks are made through the Criminal Records Bureau and the Protection of Vulnerable Adults register to ensure the protection of service users. All new staff are required to complete a comprehensive induction training programme which is signed for by both the manager and the member of staff. In house training is given to all staff as well as attendance at external training courses. Several of the staff have attended a training course on an Introduction to Care Standards to give them greater insight into the standards and the reasons for the implementation of these. A record is held on staff files of all training undertaken. Specific packs have been prepared for qualified nurses, care, domestic and catering staff. This is to ensure that all staff are fully appraised of their role and responsibility within the home to further protect the service users. Staff records inspected were found to contain all necessary information together with evidence that appropriate checks have been made on them. Supervision is given to all staff on a regular basis and annual appraisals take place. These meetings provide the opportunity for staff to speak with the manager on a one to one basis to discuss career development and to identify with any training needs and opportunities. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 31, 33, 35, 38 The systems for service users consultation are good with a variety of evidence that indicates that service users’ views are both sought and acted upon. EVIDENCE: The acting manager of the home and one of the owners have now achieved an NVQ in management at level 4. The acting manager is a qualified nurse who also holds qualifications in the care of older people, palliative care and in training and assessing. An application to register the acting manager has been submitted to the Commission for Social Care Inspection and is being processed. The acting manager has an open door policy and is available to service users, relatives and staff. Service users and staff spoken to during the inspection confirmed that the both the owner and acting manager are approachable and Riversdale Nursing Home DS0000020919.V281973.R01.S.doc Version 5.1 Page 21 accessible on a daily basis and have the opportunity to meet with them in private whenever they wish. Staff meetings are held regularly and minutes are taken. Group staff meetings are held and also specific staff meetings i.e. qualified staff, care staff, ancillary staff. Supervision is given to all staff every two months and annual appraisals are undertaken to provide staff with the opportunity to review their own professional and career development and to identify training needs. The Registered Person has implemented the ‘Blue Cross’ quality monitoring standard and audits are regularly undertaken. Recent audits have been made on care, catering, administration and housekeeping. The audits involve gathering the views of service users, staff, relatives and care professionals. The findings are summarised in the service user guide. The home has recently achieved ‘Investors In People’ quality assurance standard. Policies and procedures are in place to protect the interests of each service user and these have recently been reviewed and updated. The home does not deal with any of the service users finances. Secure facilities are provided to store valuables and accurate records are held of these. All staff have been given training on health and safety and are required to read the policies and procedures relating to these. Safety certificates inspected were seen to be in place and up to date.
