This inspection was carried out on 14th April 2005.

CSCI has not published a star rating for this report, though using similar criteria we estimate that the report is Adequate. The way we rate inspection reports is consistent for all houses, though please be aware that this may be different from an official CSCI judgement.

The inspector found there to be outstanding requirements from the previous inspection report but made no statutory requirements on the home.

What the care home does well

Good information is provided for potential residents, this includes the views of existing residents on the home. Residents at the home said they were treated with respect, privacy and dignity by staff and were able to make choices about daily routines. Residents were pleased with their environment acknowledging in particular the standard of cleanliness within the home. All areas within the home smelled pleasant and were cleaned to a high standard. The home had a pleasant atmosphere and special occasions such as Christmas and Easter were celebrated. The residents said visitors could visit the home at any reasonable time and that they had a good relationship with the staff, staff worked hard and provided a good service and relatives stated that they were able to participate in the ongoing care of residents. Residents and relatives were very happy with the meals provided and medication was managed well.

What has improved since the last inspection?

The service user guide has improved with the addition of resident`s views of the home. The environment has improved following some redecoration and replacement of a carpet and some furniture. There have been efforts made to carry out some training for care staff in important areas such as adult protection. Some recording keeping in relation to health and safety and reporting of accidents and incidents has improved. Most of the previous requirements had been met.

What the care home could do better:

Some residents and staff stated that they thought more activities and outings could be offered for residents to encourage them to participate. More staff need to attend adult protection training, and be more aware of the local adult protection procedures. A plan of training should be developed which should take into account the needs of individual staff. Health and safety systems were in place, staff stated that they had received fire training however records were not available and a container of paint was found unattended in a bathroom. One bathroom was used to store equipment, this did not allow access to the bath and could be a risk to residents. A more appropriate storage area needs to be found. Some staff and one resident stated that recently there had not been enough staff on duty, the rota did not clearly state in what capacity staff were working in. The information provided when recruiting staff needed to improve to further protect residents who lived at the home. Supervision of staff did take place but this needed more detail and discussion with the staff themselves to ensure they knew what supervision was for. More information could be included on resident`s personal money account sheets to show that residents are aware of how their money is used, including two signatures wherever possible one being the resident.

Persons accommodated shall be aged 60 years and above 2. Of the 34 registered beds, 30 can be used as nursing care (N) or personal care and 4 can be used as personal care (PC) only. Date of last inspection 5th October 2004 Brief Description of the Service: Rockley Dene is a purpose built home providing nursing and personal care. Accommodation is provided for 34 service users over the age of 60. The home is owned by Angel Care Ltd and is situated at Worsbrough Bridge. The home is on a main bus route into Barnsley town centre and there are local shops nearby. The accommodation is provided on two floors, a lift and stairs access the upper floor. Resident accommodation is provided in 9 double and 16 single bedrooms of which 10 have an en-suite facility. There is one large lounge area and another lounge area within the conservatory. One lounge area is nonsmoking. The dining room opens on to a patio area and there is a pleasant garden area. It should be noted on the same site, but not attached, is Rockley Dene ‘Personal Care’ Home. This is registered separately and is not covered by this inspection report. This unannounced inspection took place over approximately eight hours from 9:15 to 17:05. The inspection process included a partial tour of the premises, inspection of a sample of records and policies, discussions with staff, residents and relatives and observation of staff carrying out their duties. The majority of residents and staff were seen during the inspection and the inspector had the opportunity to speak to six staff, six residents and two relatives in some detail. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 1, Standard 6 is not applicable at the home. The service user guide contained appropriate information, including residents views of the home, to allow prospective residents and relatives to make an informed choice about the home. EVIDENCE: Copies of the service user guide, statement of purpose and a brochure were available in the reception area of the home. There was also information collated from a survey of service users available. One relative stated that the family had visited the home several times to enable them to make a choice, she stated that the family had been given a lot of information about the home including a copy of the last inspection report. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 8 and 9 The health needs of residents are met. Adequate procedures are in place for the safe management of medication. Individual risk assessments allow some residents to self-administer some medication in a safe way, promoting their safety and independence. EVIDENCE: Three residents care plans all contained risk assessments relating to development of pressure sores, in one care plan there was information relating to the treatment of a pressure sores including information from tissue viability specialists. Staff stated that enough equipment was available to assist in the prevention of pressure sores, which leads to a low incidence of pressure sores within the home. In one care plan a risk assessment had been updated following an accident, which occurred recently, this included detailed action to reduce the risk of subsequent falls and included the signature of a relative. A detailed plan of treatment had also been updated and completed in relation to the same accident. Residents and relatives spoken to confirmed that they had been involved in the choice of their GP. Resident’s health care needs are met. Rockley Dene Nursing Home J51 S6489 Rockley Dene N V218837 14.04.05 UI Stage 4.doc Version 1.20 Page 10 In the three care plans information was recorded detailing the support needed to ensure safe and appropriate administration of medication, one resident was able to self administer inhalers and a nebuliser. A lockable space was provided for storage of medication. A qualified nurse administers medication to residents, when observed administering medication the nurse was sensitive to the needs of the residents. Medication, including controlled drugs was stored securely and appropriately and records checked were fully completed promoting the protection of residents. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 12, 13, 14 and 15 Some activities and outings are provided but more activities and outings could be offered to encourage the participation of more of the residents. Relatives and friends are encouraged to participate in some aspects of resident care enhancing the quality of life for all concerned. Residents are given choice in many aspects of their lives, allowing them to maintain their independence. Residents receive a choice of food, which is of good quality and can eat in a pleasant dining room or their room, maximising their choice and comfort. EVIDENCE: Residents stated that they are able to choose what time they get up and go to bed, what food they eat and where they eat it, what they wear and where they spend time within the home during the day. Relatives and friends of residents were seen visiting throughout the day and one relative said that she continued to be involved in some aspects of her relatives care. An activities programme was displayed at the home, staff stated that they try and involve residents in an activity in the early afternoon however it was sometimes difficult to get residents to participate in activities. A game of hoop-la was played on the afternoon of the inspection. Staff stated that a visitor runs a weekly bingo session, which the residents enjoy and parties are held at the home at significant times of the year for example, Christmas and Easter when entertainment is provided. Some staff felt that more activities should be available for residents. One resident stated he went out regularly with family and friends, he stated that he was happy and that he had paid to have “sky” TV in his room to enable him to watch what he wanted, and that he was able to eat in his room if he wished. Some residents stated that they would like to go out more. Information was contained in two of three care plans relating to the management of the finances of residents. Residents spoken to stated that they were happy with the current arrangements for their finances. They stated that if they had money in the safe at the home that could get it when they wanted and that the manager kept them informed as to how much money they had in the safe. All residents spoken to stated that they had been able to bring personal possessions into the home, these included items of furniture, photographs and other personal effects. Time was spent observing in the dining room at lunchtime. The dining room was well presented and meals were served in an appropriate unhurried manner. Staff assisting residents to eat did so in a discreet and sensitive way. Some residents had lunch in their rooms. A choice of meals was provided, one resident was provided with soup on request when she stated that she did not want a cooked meal. Another resident was given a larger portion on request. Residents spoken to stated that the food was good and that they had “no grumbles”. One relative stated that she had eaten at the home and the quality of the food was good. The inspector had lunch, the main course was served hot and was of good quality, and the dessert was home made and was of a high quality. The cook stated that she made a lot of the desserts, as she did not use sugar, enabling residents on some special diets to enjoy the same desserts as other residents. One resident stated that she always had a salad at teatime; other residents had the choice of a hot meal at teatime as well. Service users and their relatives and friends are confident that their complaints will be listened to, taken seriously and acted upon. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 16 and 18 The complaints procedure was available, people were aware how to complain and thought that their complaints would be listened to and dealt with. Procedures are in place to protect residents from abuse, training and a raised awareness of some procedures is needed to increase the level of protection. EVIDENCE: The complaints procedure was included in the service user guide; this was available in the foyer of the home and in individual residents rooms checked. One relative stated that this information had been given prior to the family making the choice of home. The procedure states the timescale for response but needs updating to include the correct title for the Commission for Social Care Inspection. Residents stated that if they needed to complain that they would speak to the manager. All residents stated that they thought they would be listened to. One resident stated that if she was not satisfied she would contact social services. Relatives stated that they thought that the manager would listen and resolve any problems. Residents and relatives stated that they felt that the home was safe. A local procedure was in place for the reporting of allegations and incidents of abuse, including whistle blowing. A copy of the Barnsley Local Authority adult protection procedures was available but staff were unaware of it. Another local authority placed one resident at the home; the home did not have the adult protection procedures of that authority. The Deputy Manager was aware of what to do in the event of an allegation of abuse and was aware of most of the people to be contacted but did not mention contacting the adult protection team. However all staff stated that they would report any concerns relating to suspected abuse to the nurse in charge or the manager, they stated that if it was the nurse or the manager involved they would contact social services or the inspector. Residents and relatives spoken to all stated that staff were kind. Policies and procedures were in place regarding the handling of resident’s money. One service user spoken to stated that the manager regularly told her how much money she had in the safe at the home. Service users live in a safe, well-maintained environment. Service users have access to safe and comfortable indoor and outdoor communal facilities. Service users have sufficient and suitable lavatories and washing facilities. Service users have the specialist equipment they require to maximise their independence. Service users’ own rooms suit their needs. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 19, 20, 21, 24 and 26 Aspects of the environment had been improved by some redecoration and replacement of some carpets and furniture. Most areas of the home were accessible, however one bathroom was being used for inappropriate storage. The home was clean and hygienic. EVIDENCE: The home had carried out some maintenance, and redecoration. Skirting boards and doorframes on corridors had been repaired and protected against future damage. This work is to be carried out throughout the building. The carpet and chairs in the conservatory had been replaced. Staff stated that new curtains and some new bedding had recently been purchased. Areas of the home checked were clean, odour free and decorated to a reasonable standard, resulting in a mainly safe and well-maintained environment. Rockley Dene Nursing Home J51 S6489 Rockley Dene N V218837 14.04.05 UI Stage 4.doc Version 1.20 Page 16 Residents and relatives spoken to stated that the home was always very clean and that they were happy and comfortable in their rooms, resulting in a pleasant environment. Laundry facilities are appropriately sited within the home. The laundry was clean and tidy. Staff had access to policies and procedures for control of infection including, dealing with spillages, the use of protective clothing and hand washing. Staff working in the laundry were able to explain the procedures in relation to the washing of clothing and linen and stated that they had sufficient and appropriate equipment to provide an appropriate laundry service to residents. Staff stated that if equipment required maintenance this was carried out in a timely manner. Residents spoken to stated that in the main the laundry service they received was good. Care staff were aware of the control of infection procedures and were able to discuss good practice and how they applied this in a practical way. Senior staff stated that new staff were given the information about control of infection as part of their induction, reducing the risk of infection. Service users needs are met by the numbers and skill mix of staff. Service users are in safe hands at all times. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission considers Standards 27, 29, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 27, 29, and 30 The numbers and skill mix of staff do not consistently meet requirements. Recruitment policies and procedures need to be more robust. Induction training provided is comprehensive, staff receive further training to assist them in meeting the needs of the residents, but do not have individual training plans. EVIDENCE: On the morning of the inspection there was one nurse and four care staff on duty. The rota showed that on a number of occasions over the last two weeks the number of staff on duty was less than required in the staffing notice. The deputy manager stated that ancillary staff were used to supplement care staff numbers when required, this was not clear on the rota. Shortage of staff was more of a problem in the daytime reducing the time staff could spend with residents; adequate staffing was maintained at night. The nurse in charge stated that there was usually two nurses on duty in the morning but the second nurse is usually the Manager and she was on leave. One resident stated that she did not think there was enough staff on duty. Other residents stated that if they need a member of staff they pull the cord and someone would come. Relatives stated that the staff worked hard and that you could always find a member of staff when needed. Some staff stated that at times there was not enough staff, they stated that this has only happened recently because some staff had left and staff had been Rockley Dene Nursing Home J51 S6489 Rockley Dene N V218837 14.04.05 UI Stage 4.doc Version 1.20 Page 18 ill. Staff stated that they did not think this had affected the service received by the residents. The Deputy Manager stated that attempts were made to cover the rota when numbers were short, including the use of agency staff. The number of care staff was less on the day of the inspection because a member of care staff needed emergency leave at short notice. Domestic staff, kitchen staff and a handyman were also on duty. The home does not have any staff under the age of 18 and a qualified nurse is in charge of the building at all times. The use of ancillary staff to complement the numbers of care staff at times has not compromised the cleanliness and pleasant environment. Three staff files were checked, two of the files included, two written references a full employment history and evidence of a CRB disclosure. Two of the three files also included a statement of terms and conditions. Residents are not fully protected by recruitment policies and procedure as a member of staff had recently been employed without written references or evidence of a CRB disclosure this was reflected in the lack of information the staff members file, the Deputy Manager stated that the member of staff was being closely supervised until references were obtained. The Deputy Manager was unsure of the progress of the development of a training plan for the home, staff stated that they had received moving and handling training, fire training and other training relating to the specific needs of the residents. One member of staff stated she could not recall all the training she had been on as she had attended a lot. Staff working in the laundry stated that they had attended COSHH training and had been involved in training related to the care of the residents. One member of staff stated that her induction training had included fire, moving and handling, first aid, and health and hygiene and abuse awareness. Staff were not clear if they had an individual training plan. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 33, 35, 36, 37and 38 Effective quality monitoring systems were in place. Financial systems and procedures are in place to safeguard residents. Staff do not receive regular formal supervision. There were some gaps in record keeping which could affect safety of staff and residents. In the main the health safety and welfare of residents and staff were maintained, some gaps in training records may compromise safety. EVIDENCE: The results of the residents questionnaires had been published and were available along with the service user guide in the foyer and in all residents’ rooms checked. The home has a professionally recognised quality assurance system and is independently assessed against ISO 9002 standards, allowing residents a say in the running of the home. Relatives stated their views had not been formally sought however they could discuss any issues with the manager at anytime and that she would try and resolve them. There was a policy and procedure for the handling of resident’s monies including written transactions for each individual resident whose money was kept in the safe, safeguarding residents financial interests, however written transactions only had one signature. Staff spoken to stated that they did not receive individual supervision on a regular basis. Staff were observed moving residents in an appropriate way and using the hoist in a safe and competent manner. All staff spoken to stated that they had received moving and handling training. Records were available to confirm that the hoists, specialist baths and the passenger lift are serviced regularly. Fire drills take place on a regular basis and staff asked stated that they had received fire training. Records were not available to confirm that all staff had received fire training. Fire appliances had been serviced. In the main hazardous substances were stored securely, however a plastic container of paint was found unattended in a bathroom. Records were in place stating that electrical systems and equipment had been maintained. Hot water checked was close to 43 degrees centigrade to reduce the risk of scalding. The home had a robust accident reporting system and notified the CSCI appropriately of any incident occurring at the home. A recent incident had been reported under RIDDOR. Residents and relatives spoken to stated that residents were kept safe in the home.
