What the care home does well

What has improved since the last inspection?

The staff team are being continually developing and empowered through training, coaching and positive role modelling. The home is now having to use less agency staff which has improved the consistency and continuity of care. Building work continues and is nearing completion.

What the care home could do better:

Residents believe that Roselawn cannot be improved. On this occasion the inspector has not required or recommended any changes to current practice.

The home is owned by Key Change, a not for profit organisation and has a Christian and spiritual ethos. The home currently provides personal care together and accommodation for up to 23 older people. However, building work is under way which will result in the home providing 6 new ensuite bedrooms, an extra dining room, a quiet room, a conservatory and extra staff and meeting rooms. The home is set back from the road and has level access throughout the ground floor. Bedroom accommodation is provided on the ground and first floor of the home, and floors are linked by a passenger lift. All bedrooms have en suite facilities. The kitchen is equipped to a very high standard. The gardens are level, secure and well tended. This short unannounced inspection took place as part of the normal programme of inspection between 1.30pm and 3.30pm. Residents were just finishing lunch with coffee or tea as desired. The inspector looked around the home, spoke with seven residents, two visitors, two members of staff and with the manager. No written records were inspected. Building works continues to provide 6 new ensuite bedrooms and extended communal and office space continues. It is anticipated that this will be completed by Autumn 2005. What the service does well: Roselawn has a strong and well-developed ethos of person centred care. Each resident is treated as an individual who has rights and responsibilities and who should be supported to live the life they wish to live. Health needs are well met with evidence of good multi disciplinary working. Personal support is provided in a way that promotes privacy, dignity and independence. Residents who receive ‘bad news’ are well supported to live full and fulfilling lives. Residents’ lives are enriched by their lifestyle and the links with the local community. Residents carry on with their lives receiving the support they need to do so, for example visiting with friends, going out for walks, going to church and going shopping. Staff are described as always giving that little extra. During this inspection a carer arrived at the home on her day off to take one resident into town as it was such a nice day. What was particularly person centred about this was that the resident was asked. It was not assumed that she would be ‘taken’, that she would go or that she would be grateful. The inspector saw the resident in the town later and she was obviously really enjoying herself. Other residents had visited the local gardens that morning and some were preparing to receive visitors. The food and chef are described as ‘exceptional’. There is wide choice, extensive variety and all meals are beautifully presented as social events and occasions. Roselawn is staffed by a team who follow the ethos of the home – promoting independence and person-centred care and personhood. They are described by residents as having the ability to anticipate and meet needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 8 and 10. The health needs of residents are well met with evidence of good multidisciplinary working taking place on a regular basis. Personal support is offered in such a way as to promote and protect residents privacy, dignity and independence. EVIDENCE: All residents spoken to said that staff are very quick to make referrals to (for example) doctors and nurses as needed. Speaking with staff demonstrated that any information given by the medical team is reinforced by staff, and that residents who receive ‘bad news’ are well supported to live full and fulfilling lives. Residents said that all their personal care needs are dealt with in private and that staff behave as (welcome) visitors in their home. Staff were observed knocking on bedroom doors and treating residents with discretion and respect. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 12, 13, 14 and 15. Residents are enriched by the lifestyle experienced and links with the community are excellent, supporting and enhancing residents’ social opportunities. The meals in this home are excellent offering both choice and variety and catering for special dietary needs. EVIDENCE: Residents at Roselawn choose their daily routine, for example what time to get up and go to bed, what they wear and eat. One said that living at Roselawn means she can carry on with her life and not have to ‘start a new life in a home’. Many residents enjoy walking, or being escorted, into Sidmouth for coffee, shopping, walks and meeting with friends. One resident entertains his visitors for lunch once a week and is taken and collected by staff twice weekly to visit with his relatives. Staff were seen making time for and talking with visitors. Visitors get to know other residents creating a real ‘social’ feel. Without fail, residents at Roselawn are exceptionally complimentary about the chef and meals. They say the standard of presentation, variety and choice are excellent. Individual preferences are always taken into account. For example, when fish is on the menu – residents can choose how they would each like it cooked (for example poached, baked etc). Sweets are another favourite with residents when there is usually a choice of between five and nine deserts from the sweet trolley. The chef visits with residents in the afternoon to find out what they would like to eat for supper and according to residents there is little or no restriction on choice. Tables are always dressed and meal times are social occasions. Service users and their relatives and friends are confident that their complaints will be listened to, taken seriously and acted upon. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 16. The home has a satisfactory complaints procedure with evidence that residents believe their views are listened to and acted upon. EVIDENCE: Roselawn has a clear complaints policy. Residents said they had no complaints and that their suggestions are always acted upon. During the inspection one resident made a request. The manager dealt with this immediately, not just providing the item requesting but promoting choice and independence and empowering the resident by asking her to choose the item herself which the manager would then arrange to purchase. Neither the home or CSCI have received any formal complaints. Staff are well matched to meet residents needs, providing good standards of consistent care within the ethos of the home. EVIDENCE: All residents were extremely complimentary about staff. They said ‘they can’t do enough’, ‘they make me feel safe’ and ‘they couldn’t be better’. It is clear that staff treat residents as equals and with respect. Great care is taken to meet identified needs and to anticipate needs. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 31 and 32. The manager provides strong leadership and has a clear vision for the home which has been effectively communicated. The manager is well supported and staff demonstrate an excellent understanding of their roles and responsibilities. EVIDENCE: Many of the residents described the manager’s drive and commitment. They believe that the extensive building works would not have taken place or been so well managed without her leadership and management skills. Residents say they ‘are blessed’. Staff and management have easy, relaxed and respectful relationships with each other and with residents. The team is continually being developed and empowered through training, coaching and positive role modelling. Residents have great faith in the staff team. One resident said ‘they really care for us’.
