What the care home does well

Two Service Users said that the staff do every thing they can to help them. "I just ring my bell and someone comes straight away". "I like the dinners, I can choose something else if I want to but the meals are all very good". The individual Service User assessments seen were very well detailed and clearly indicate needs and abilities and give staff clear directions on what assistance to provide. The Registered Manager has written and organised records, policies and procedures to be easily understood, accessible and confidential where required.

What has improved since the last inspection?

The Registered Manager stated that he has almost completed his NVQ4.

What the care home could do better:

Medication administration does not meet National Minimum Standards as described in the body of the report. A requirement was made for this. Two staff left the employ of the home in the previous year and this had compromised the home obtaining a 50% NVQ trained staff complement. The Registered Manager said that training is not yet completed to meet all mandatory courses in the National Minimum Standards although some had been arranged. A requirement was made for this. The role of Registered Manager is demanding. The National Minimum Standards does not determine the numbers of hours a Registered Manager should work, but the Registered Manager must be able to demonstrate how he will manage the home effectively. The Registered Manager has to work in the home daily for long hours. A recommendation is made for dedicated management support to be given to allow time for best practices to be undertaken.

The Manager to have completed NVQ 4 in Management and Care by 2005. 18th October 2005 Date of last inspection Brief Description of the Service: Roselawn residential home provides care for up to 13 older people who require varying degrees of assistance. Rose Lawn has access to specialist services within the community. Staffing within the home comprises of a Registered Manager, care and ancillary staff. The home is a family run business with the owners and their family members having a high level of input into the home. There are two double bedrooms in the home. All but one bedroom has en-suite facilities. The home is located within short walking distance to Margate town centre and sea front. There is a small parking area to the front of the property. Fees are in the range of £290-00p - £350.00p . There were 9 Service Users living at the home at the time of the unannounced inspection. The Registered Manager, the Registered Providers, a carer and a catering assistant were on duty at the time. The carer was the only person not a member of the family. Some records were viewed and an accompanied tour of the premises was undertaken. Some Service Users, the Registered Manager and Registered Providers spoke to the Inspector during the visit. Records seen were generally of a very good standard and had been clearly and concisely written by the Registered Manager. Medication administration was seen to include ‘potting up’ tablets and did not meet the National Minimum Standards. The Registered Manager said he knew it was not correct but it was to save time. The Registered Manager stated that not all staff has undertaken appropriate medication administration training but he was currently seeking a suitable training course. Medication administration is required to meet the guidelines of the Royal Pharmaceutical Society of Great Britain. The deputy manager has left the employ of the home since the previous report and the Registered Manager works in the home every weekday from 7-5 and two split shifts every Saturday and Sunday. The Registered Manager must be given adequate assistance to support his duties to meet the health and welfare needs of the Service Users. Service Users commented most favourably on the care they receive and the attention given to their needs. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 3,6. Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. Prospective Service Users know they will have their needs assessed before moving into the home. Intermediate care is not undertaken. EVIDENCE: A Service User said that she had been able to visit the home prior to making a final decision to live there permanently. She added that all her needs were catered for. The Registered Manager said he and a member of staff would visit a prospective Service User in their own home to make an initial assessment. Evidence of a written assessment was seen. Care management reviews are also taken into consideration when making a decision about whether the home can meet individual needs. Service Users are given the opportunity to visit the home to ensure their needs will be met. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 7,8,9,10. Quality in this outcome area is adequate. This judgement has been made using available evidence including a visit to this service. Individual plans of care are undertaken. Service Users health needs are met. Care staff lack medication training and medication practice is unsafe. Service Users stated they are treated well and with respect. EVIDENCE: Individual care plans contain very good detailed assessments of different aspects of Service Users needs and abilities. Plans also include for example and where applicable, a brief history of any falls and an assessment of health and social needs regarding such falls. This is good practice. There is evidence in the care plans of health care treatment and intervention, Rose Lawn Residential Home DS0000023534.V326509.R02.S.doc Version 5.2 Page 11 Service Users said that the staff were kind and eager to help them and that “nothing is too much trouble”. Medication administration does not meet National Minimum Standards. The Registered Manager was able to discuss with clarity the requirements of the Standards but stated that due to time constraints had potted up medication in readiness for administration. He also stated that he is currently in the process of changing pharmacy and is discussing the homes needs with a different dispensing pharmacist. Not all staff had received suitable training to administer medication and the Registered Manager said he was looking for suitable courses. A requirement is made for the National Minimum Standards to be met. The Registered Manager agreed to this. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 12,13,14,15. Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. Service Users know their lifestyle can meet their preferences. Family and social contact is encouraged. Service Users are helped exercise control over their lives. Service Users receive a varied diet and special diets are catered for. EVIDENCE: An entertainer was in the home at the time of the inspection and Service Users confirmed that regular entertainment takes place. Kent County Council library provides large print books, videos and a resource for reminiscence materials. A Service User said that some people have their families visit to take them out. The home has a people carrier and occasional visits have been made to local theatres, shopping and restaurants. The Registered Manager stated the home does not take responsibility for any Service Users monies except if occasionally a family member will give some money for the home to pay for example, hairdressing. Service Users described the meals as very nice and that there is always a choice if they requested it. Food is considered by the Registered Provider to be a highly important part of the day and some Service Users confirmed this. Food being prepared for dinner that day, including special diets, was all fresh produce. Specialist diets are prepared to look as similar to the main menu as possible. The home has been awarded a gold award from Thanet District council for ‘food excellence’. There was evidence of a variety of food in the storage area and the kitchen was well organised and tidy. A cleaning schedule was provided but no evidence of a rota or signature confirming this had been undertaken. The Registered Manager said he would provide a format for staff to sign as and when cleaning is undertaken. Service Users are protected from abuse. EVIDENCE: The home has a complaints policy and procedure. A notice advises who to contact in the event of a concern. A Service User said she was aware of who to speak to if ever she should have a concern or complaint and that she felt she could speak to the owners and Registered Manager. Certificates were seen for some staff attendance at adult abuse awareness courses. Adult abuse training is undertaken through induction and will be followed up by an external training course. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 19,24,26. Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. Service Users live in a safe well-maintained environment. Service Users bedrooms are comfortable and personalised. Service Users live in an environment that is clean, safe and hygienic. EVIDENCE: The home was clean and hygienic and the laundry facilities have machines that reach the required high temperatures for infection control. Maintenance certificates for gas, electricity, lift, hoist and fire safety were in date. There are two double bedrooms in the home one is currently used as a single room and the other is vacant. The Registered Manager confirmed that he was looking for another single occupant, as the room is small. They have the choice to bring small personal items of furniture into the home and many bedrooms seen were individualised with personal effects. Service users’ needs are met by the numbers and skill mix of staff. Service users are in safe hands at all times. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 27,29,30. Quality in this outcome area is adequate. This judgement has been made using available evidence including a visit to this service. Service Users needs are met by adequate numbers of care staff. Recruitment practices support Service Users. Service Users will benefit by all care staff completing mandatory training. EVIDENCE: Care staffing rotas try to take into account the times of high and low activity. Service Users said that the staff were very kind and helpful and would answer their call bell very quickly. One said she had confidence in the staff to look after her. The Registered Manager recognises the importance of training, and delivers where possible a programme that meets any statutory requirements, but there are still some areas which need attention. Because two staff have left in the past year the 50 NVQ level has been compromised. Mandatory training has started; but not courses all have been completed by some staff. A requirement is made for training appropriate to the work they are to perform to be arranged and completed according to the National Minimum Standards. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 31,33,35,38. Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. The manager has almost completed the required qualifications and would benefit from dedicated management support. The home is run in the best interests of the Service Users. Service Users financial interests are safeguarded. The health, safety and welfare of Service Users and staff are promoted and protected. He is currently 2/3rds of the way through his NVQ4. A Service User said that the Registered Manager is very approachable. Service Users and relatives questionnaires are used as quality assurance. Some of these were viewed and contained positive feedback including ‘care staff are exceptional’, have only seen kindness and care for all’. There is also information that the last inspection report is available for viewing. The home also uses a quality assurance document from the Kent Care Homes Association. The Registered Manager confirmed that the home does not have any involvement in Service Users monies. A Service User said that her family dealt with her finances. Health and safety records seen indicated that regular safety checks are undertaken by outside agencies. Fire records include regular checks and drills. The Registered Manager was able to express a clear understanding of the different needs of Service Users and of procedures and practices. He was aware of the requirements to meet staff training and medication administration standards.
