This inspection was carried out on 26th January 2006.

The inspector made no statutory requirements on the home as a result of this inspection and there were no outstanding actions from the previous inspection report.

What the care home does well

All prospective residents have an in-depth assessment before moving into the home to ensure all individual needs can be met. The care planning system is good, with up to date information provided in order to deliver the care requited to meet the assessed needs. All residents and/or their families are involved in the care planning process. All healthcare needs are met and there is an up to date record of professional healthcare visits and appointments. The administration and recording of medication is of a high standard ensuring the safety of the residents. There is a programme of organised activities and outings both in groups and on a one to one basis. Catering staff make every effort to ensure that a nutritious and varied menu is provided with a choice at each meal.

What has improved since the last inspection?

Installation of a new platform lift is in progress and the building of two new medical rooms is almost complete. Redecoration of the home will continue after the building work is complete.

The service must at all times employ a suitably qualified and experienced manager who is registered with the National Care Standards Commission. A maximum of nineteen older people (19OP) including two people with dementia (2DE(E)). One named older person with a mental disorder (1MD(E)). When single rooms of less than 12 sqm usable floor space become available they must not be used to accommodate wheelchair users, and where existing wheelchair users are in bedrooms of less than 12 sqm they must be given the opportunity to move to a larger room when one becomes available. 18th July 2005 Date of last inspection Brief Description of the Service: Rosehill House is registered to provide care and accommodation for up to 19 older people. The home is owned by Mr and Mrs Hill-Eades, with Mrs Hill-Eades responsible for the day-to-day running of the home. Rosehill House is a detached, older style property set in its own grounds approximately three miles from Whitehaven. Access to the home is by a shared driveway with the nearby Rosehill Theatre. The accommodation for residents is on the ground and first floors with a stair lift giving access to the first floor. There are thirteen single rooms and three doubles, two of which are currently shared and the remaining one used for single occupation. The home has one assisted bathroom, an assisted shower room and toilets close to communal areas. All the bedrooms have en-suite toilet and washbasin facilities. The external areas are pleasant and well maintained with seating areas and ample car parking. This was an unannounced inspection of the home that took place over one morning. It was the second inspection of the year and those standards not assessed on this occasion were inspected and met during the last visit that took place on the 18th July 2005. Time was spent talking with the manager, deputy manager, care staff and residents. Records to do with the running of the home were examined and some parts of the home were looked at. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 1&2 The home’s introduction pack is excellent, providing residents and prospective residents with details of the services the home provides. This information enables an informed decision to be made about admission to the home. EVIDENCE: When first visiting the home, prospective residents and/or their families are given a brochure illustrating, in brief, the facilities offered at Rosehill. On admission a pack containing, a full statement of purpose, resident guide and contract and terms and conditions are issued. There is a copy of the contract held on each resident’s file. The information contained in this pack gives all that is required to ensure the resident is confident that the home is able to meet all the assessed needs. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 9 The healthcare of the residents are understood and well met. Medication and records are extremely well maintained to ensure the protection of the residents. EVIDENCE: During the visit the inspector was able to observe the medication being given to the residents. All medication was administered directly from the blister pack supplied by the pharmacist with the medication record signed at the same time. All medication records were checked and these were found to be well organised and correctly and neatly filled in. The registered manager or deputy checks the records regularly to ensure no errors have occurred. Controlled drug records are kept independently and an audit of these showed no inconsistencies. There is an excellent system for detailing the medication received and returned with the pharmacy signing for all those returned. All care staff have completed safe handling of medication training. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 14 There is a sociable atmosphere in the home and the residents interviewed were happy with the daily routines and the opportunity to choose how to spend their day. EVIDENCE: Although few formal residents meetings are held, informal “chats” often take place in the lounge during morning coffee or afternoon tea times. Communications with family members are good with the manager always ensuring an informative flow of information between the home and relatives. The inspector observed residents moving freely around the home with some in the lounge and those who wished, remaining in their rooms. One resident had just come in from her daily walk and told the inspector how much she liked to go out. Service users and their relatives and friends are confident that their complaints will be listened to, taken seriously and acted upon. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 18 Staff have good knowledge and understanding of adult protection issues, which protects residents from abuse. EVIDENCE: Discussions with the manager evidenced that all staff are aware of adult protection issues as this is discussed during staff interviews and also during the induction programme. The home has purchased a set of training videos and in-house training is organised by the manager and deputy. Those staff who have, or are completing their NVQ also discuss this subject at length during the course. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 23 & 24 The home offers comfortable accommodation and most areas are well maintained. This contributes to a good quality of life for the residents. EVIDENCE: The inspector examined a sample of residents’ rooms during the visit and found them suitable for the stated purpose with except one having en-suite toilet facilities. The rooms were warm and comfortable and personal to the residents with pictures, ornaments and photographs brought from residents’ own homes. Some residents choose to remain in their own room during the day watching their own television or just sitting quietly. EVIDENCE: There is a very low turnover of staff at Rosehill House but the home does have a full recruitment and selection procedure. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. JUDGEMENT – we looked at outcomes for the following standard(s): 34, 35 & 37 There is clear leadership, guidance and direction to staff to ensure residents receive consistent good quality care. Residents are protected by the accounting and financial procedures operating within the home and a full set of policiea and procedures. EVIDENCE: The registered providers work with the bank and their accountants to ensure the financial viability of the home remains stable. There is no residents’ cash held on the premises and accounts for personal spending are prepared and sent to families on a monthly basis. The home has robust policies and procedures in place that are examined and updated on a regular basis.
