This inspection was carried out on 21st November 2005.

What the care home does well

Service users say that their experience at Silver Birches is like living in a small, family owned hotel. People are encouraged to pursue their preferred daily routines and to be as independent as possible. Service users say they get on well together and enjoy each other`s company. All of the staff team are reported to be very friendly and helpful. Service users say they like to hear about the families of the staff and they enjoy having the family pets - dog, cat and parrot, around the house. The house is furnished and decorated to a high standard and everyone has been assisted to personalise their own rooms. Everyone is always full of praise for the quality and choice of meals that are available. The home is well managed, the records are kept up to date and the staff work well together as a team.

Since the last inspection all members of the team have taken part in a variety of training courses.

The house is in a quiet residential area of Mytholmroyd, off the main road but within easy reach of the main bus route and railway station. Shops and other local amenities are a short walk away. In the Statement of Purpose, which gives information about the home and is available on request from Silver Birches, it says that the home caters for people with low to medium dependency levels. Accommodation is provided in single bedrooms, four have en suite facilities. The bedrooms on the first floor can be accessed by a stair lift. On the ground floor there is a comfortably furnished lounge and adjoining conservatory, which overlooks the garden, the dining room is adjacent. The house is decorated and furnished to a high standard throughout. There is ramped access to the house and the well-stocked garden. The proprietors live on the premises and with other members of their extended family they are very much involved in the day to day running of the business. This inspection was unannounced and was conducted over three hours. Discussion took place with the staff on duty and with service users. The staff rota and staff records were examined. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 1,2,3,4,5. All prospective service users have their needs fully assessed prior to admission and visits to the home are arranged for them and their families. EVIDENCE: Many of the people who live at Silver Birches are from Mytholmroyd and the surrounding area. Some people have known each other for many years and chose to move into the home because they knew people who were already living there. The home does not advertise it relies on its good local reputation and personal recommendations. In the literature provided by the home about the service on offer, it states that the home aims to provide a service to people who have low to medium dependency levels. Prospective service users and their families are always invited to visit the home to meet people living there and to view the accommodation, before making a decision about moving in. People are welcome to stay for a meal and to visit as often as they wish; service users confirmed that this was the case. The manager assesses everyone before they move into the home to ensure that their needs can be met. Assessments are well documented and these form the basis of the plan of care. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 10. Service users feel valued. Care and support is delivered with discretion and privacy and dignity is respected. EVIDENCE: The staff address people as each service user prefers, people are asked about this on their first contact with the home. From observation and from talking to service users it is evident that everyone is treated respectfully. Service users say that all the staff are very kind, helpful and always pleasant. Staff were said to be very patient, people did not feel that they were hurried in any way. Staff members said that all personal care is delivered discreetly, in private. Service users confirmed this. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 14,15. Service users are encouraged to exercise control and choice over their lives. Menus offer plenty of choice, meals are of good quality. EVIDENCE: Although all of these standards were assessed at the last inspection, service users mentioned some points again on this visit. Service users say that one of the reasons they like Silver Birches is because each person’s chosen daily routine is respected by the staff. People like to get up at times that suit them and have breakfast when they are ready. Some people like to spend parts of the day reading, watching TV or listening to the radio in their own rooms. The staff understand this and support people accordingly. Service users do not have to fit into a routine that suits the needs of staff. The main meal of the day is in the evening, lunch is a light meal and this arrangement appears to suit everyone. Service users view mealtimes as a social occasion, the majority of people eat together in the dining room. According to service users the staff possess excellent culinary skills and produce meals of good quality. The menu was reported to be varied and the meals made from good quality ingredients. One service user said that if Silver Birches DS0000001012.V265903.R01.S.doc Version 5.0 Page 11 someone had a particular wish for a certain dish or a desire for a particular food, the owner of the home would arrange for this to be made available. EVIDENCE: Since the last inspection, all staff have taken part in adult protection training provided by the Adult Protection Coordinator for Calderdale. Service users’ needs are met by the numbers and skill mix of staff. Service users are in safe hands at all times. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 27,28,29,30. The home is appropriately staffed to meet the needs of the service users. EVIDENCE: Some members of the staff team are related to the owners. The home is currently fully staffed. Examination of the rota showed that all parts of the day and night are adequately staffed to meet the needs of service users. The owners live on the premises and they are available most days, even if they are not on duty. They confirmed that they are available at the home every evening and throughout the night. They cover waking night duties. All but two of the staff have now completed NVQ level 2. The last two staff are near to completion. The owner/manager is currently undertaking NVQ level 4. Recent staff training has included health and safety, food hygiene and the safe handling of medicines. Two staff have recently completed first aid training and all staff are about to embark on a course about nutrition and healthy eating. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 31,33,35,38. The home is well managed. Appropriate systems are in place to safeguard the health and safety of service users. EVIDENCE: The manager/owner lives on the premises and is available most days. She is approachable and welcomes comment on the service provided. Service users have said in the past & on this occasion that they would not hesitate in talking to her if they had any concerns. From speaking to staff it is apparent that they have the interests of service users uppermost in their everyday practice. The home does not hold personal money on behalf of individuals. Service users are usually supported by families and friends in these matters. Silver Birches DS0000001012.V265903.R01.S.doc Version 5.0 Page 16 The home has a comprehensive set of policies and procedures, which are regularly reviewed, and they have support from a specialist health and safety company, which provides advice and annual inspection of related records. Fire alarms and emergency lighting are tested regularly and a full record of the servicing of fire safety equipment was available.
