Conditions of registration: None Date of last inspection 10/1/05 Brief Description of the Service: Silver Birches is registered to provide personal care for ten older people. The house is in a quiet residential area of Mytholmroyd, off the main road but within easy reach of the main bus route and railway station. Shops and other local amenities are a short walk away. In the Statement of Purpose, which gives information about the home and is available on request from Silver Birches, it says that the home caters for people with low to medium dependency levels. Accommodation is provided in single bedrooms, four have en suite facilities. The bedrooms on the first floor can be accessed by a stair lift. On the ground floor there is a comfortably furnished lounge and adjoining conservatory, which overlooks the garden, the dining room is adjacent. The house is decorated and furnished to a high standard throughout. There is ramped access to the house and the well-stocked garden.The proprietors live on the premises and with other members of their extended family they are very much involved in the day to day running of the business. This is an overview of what the inspector found during the inspection. The inspection was carried out over 4.15 hours. Seven residents, one relative and three members of staff were spoken to. Although there have been no concerns about the care of people at Silver Birches, it would be good practice for staff to attend some updated adult protection training. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The intended outcomes for Standards 7 – 11 are: 7. 8. 9. 10. 11. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 7,8,9. The care plans clearly show the action that staff at the home must take to meet the needs of residents. EVIDENCE: There was evidence that service users had been assessed appropriately before moving to Silver Birches to ensure that their needs could be met at the home. The care plans show how the health, personal and social care needs of individuals are to be met. The daily records provide evidence of the care that is delivered. Details are recorded about residents’ preferred daily routines and about their likes and dislikes, interests and hobbies. Each month the records are summarised and the plans are reviewed. Relatives accompany most people when they attend medical appointments; where this is not possible the staff confirmed that they would escort individuals. There are detailed records on each personal file showing the outcomes of all medical appointments. One person chooses to look after her own medication; the arrangements for this are satisfactory. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 12,13,14,15 Residents are encouraged to maintain their independence and they make choices about how they want to spend their time. EVIDENCE: Residents said they liked to choose their own daily routines, some people said they prefer to spend time in their own rooms watching TV, reading or doing crosswords. Other people said they liked to spend time in the lounge where there was more opportunity to chat. Residents said that the staff respected these choices. Everyone said that visitors were always made welcome when they called at the home. The majority of residents said they occasionally go out with their relatives. There was unanimous agreement that the meals were very good. Everyone said there was plenty of variety on the menu and made favourable comments on the quality of the produce and the good home cooking. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 16,18. The complaints procedure is well publicised and residents are confident that complaints would be taken seriously and acted upon. EVIDENCE: The home has a complaints procedure in place; details are included in the Statement of Purpose, which provides information about the home. This is available from Silver Birches on request. Residents said they would not hesitate to mention any concerns they had to the owner and they felt that they would be listened to. Of the people spoken to, no one had needed to make a complaint. There have not been any adult protection issues at Silver Birches. Nevertheless, it would be good practice for staff to attend training in this area to ensure that they are up to date with current procedures. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 19,20,21,22,23,24,25,26. The standard of the environment at Silver Birches is good, providing residents with a well-maintained, attractive and homely place to live. EVIDENCE: All parts of the house are comfortably furnished and pleasantly decorated. Residents spoken to said they were very pleased with the standard of accommodation. All of the bedrooms are decorated differently and each one has colour coordinated accessories. There is evidence that individuals have organised their rooms to suit their own needs and everyone has lots of their own personal possessions around them. The bathrooms are well decorated and inviting, and there are lots pictures, ornaments and well tended plants throughout the house. All parts of the house were clean and odour free, residents said that this is always the case. At the back of the house there is a well-stocked garden, which everyone said they liked very much. Some residents were sitting out in the garden enjoying the good weather on the day of the inspection.
