The inspector found there to be outstanding requirements from the previous inspection report but made no statutory requirements on the home.

What the care home does well

Service users live in a clean home with plenty of space and a well-kept garden. The home is adapted to meet the needs of service users. Staff treat service users with respect and have positive relationships with service users. Staff receive training that is relevant to the needs of the service users. Staff have relevant qualifications and experience. A good range of leisure activities is on offer to service users. Service users are supported to go out frequently. Service users` health, safety and welfare are protected. There is a clear procedure if service users or their relatives want to make a complaint. Service users are supported well by staff to take their medication.

What has improved since the last inspection?

A service user has been helped to grow fruit and vegetables in the garden. The quiet lounge has been redecorated.

What the care home could do better:

The hot water needs to be adjusted as it is not hot enough. Staff records need to be kept at the home so that they can be inspected.

Date of last inspection 18/1/05 Brief Description of the Service: 46 Smithies Moor Lane is a care home registered to provide nursing care and accommodation for six adults with learning disabilities. The home is operated by St Anne’s Shelter and Housing Association, a charitable organisation.The property is a large detached bungalow, which stands in its own grounds, overlooking countryside. There are parking facilities at the front of the home and attractive gardens to the rear.The home is located in a residential are on the outskirts of Birstall. Local shops, a post office and community facilities can be found within 1 mile of the home. This inspection was unannounced and took place between 3.30pm and 5.50pm. One inspector conducted this inspection visit. The inspector had the opportunity to talk to two service users during the course of the inspection. The inspector also talked to the manager, deputy manager and a nurse on duty. The inspector examined records, and accessed communal areas of the home. This is a well run home providing good care to six service users with learning disabilities. Service users’ rights are respected and responsibilities recognised in their daily lives. Service users are offered a healthy diet and enjoy their meals and mealtimes. The Commission considers Standards 12, 13, 15, 16 and 17 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 12,13,14 Service users are offered opportunities to access community facilities on a regular basis. A good range of leisure and educational activities is available at this home. EVIDENCE: The manager told the inspector that some of the service users attend day services and a range of college courses. One service user had been at day services on the day of inspection. A range of leisure activities is offered to all service users both in the local community and within the home. On the day of inspection, two service users had gone on a day trip to Bridlington, and two service users went out for their evening meal. The manager reported that all service users have had a holiday this year. The home has its own transport that enables easy access to the community. The inspector saw evidence of available activities within the home including, games, TV and stereo equipment and a computer. One service user was supported to dig up potatoes from the garden that she had grown. A vegetable plot has been developed since the last inspection, and a service user has enjoyed growing fruit and vegetables. Service users receive personal support in the way they prefer and require. Service users’ physical and emotional health needs are met. Service users retain, administer and control their own medication where appropriate, and are protected by the home’s policies and procedures for dealing with medicines. The ageing, illness and death of a service user are handled with respect and as the individual would wish. The Commission considers Standards 18, 19, and 20 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 20 Administration, storage and recording of medicines are well managed at the home. EVIDENCE: Medication records and stock were examined. Medication for one service user did not reconcile with records kept, however the manager was able to identify where the error had occurred and take appropriate steps to rectify this. Medicine management in the home is good. Staff complete a daily check and a further weekly audit is completed of all medicines. Evidence of this was seen in the records. This system allows for any errors to be identified quickly and appropriate action to be taken. Clear guidelines and instructions are in place regarding the administration of prn medication. Medication is stored securely in the home. Service users feel their views are listened to and acted on. Service users are protected from abuse, neglect and self-harm. The Commission considers Standards 22, and 23 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 22,23 The home has a clear complaints procedure that is available to service users and their relatives. Good procedures and care practice at this home protects service users from harm and abuse. EVIDENCE: The manager reported that no complaints have been received at the home in the past twelve months. A satisfactory complaints procedure that includes timescales for dealing with a complaint is in place at the home. Since the last inspection the manager has written to all relatives of service users to remind them of the complaints procedure. This was a recommendation at the last inspection. The inspector was unable to confirm service users understanding of the complaints procedure. A satisfactory adult protection procedure, including whistle blowing, is in place in addition to the Kirklees Vulnerable Adult procedures. A comprehensive physical intervention policy and procedure is available that is in line with Department of Health guidance. The manager and staff told the inspector that agreed physical intervention plans were still in place and continue to be used occasionally, to good effect. The manager and a staff member described deescalation techniques that are used successfully with a service user. Staff have received relevant training in de-escalation and physical intervention techniques. Service users have the specialist equipment they require to maximise their independence. The home is clean and hygienic. The Commission considers Standards 24, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 24,30 Service users live in a clean, comfortable home that has plenty of space. Two service users have single bedrooms and there are two shared rooms each accommodating two service users. Adapted bathing facilities are provided. EVIDENCE: The inspector had the opportunity to look around communal areas of the home. The home is in a good state of repair and is domestic in style. The home is comfortable and is furnished to a satisfactory standard. The quiet lounge has been redecorated since the last inspection. The home provides accommodation to six service users in two single bedrooms and two shared bedrooms. The organisation is exploring how the home might be adapted to provide single accommodation to service users. The shared rooms have been arranged so that separate areas are provided using screening and wardrobes. Adequate communal space is provided including a good-sized lounge with a raised dining area, kitchen and quiet lounge/computer room. An adapted bathroom and shower room are provided. There is a large wellmaintained garden to the rear of the property and a car parking area to the front. There is a small sleeping in room/office for staff. The home was clean and free from offensive odours. Service users are supported and protected by the home’s recruitment policy and practices. Service users’ individual and joint needs are met by appropriately trained staff. Service users benefit from well supported and supervised staff. The Commission considers Standards 35 the key standard to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 32,35 Staff working at the home have relevant qualifications and experience. Relevant training is provided to staff. Staff have good relationships with service users. EVIDENCE: The inspector did not fully assess standard 34, however the manager reported that the organisation is still in discussion with CSCI regarding recruitment records being held centrally. Records required under the Care Homes Regulations are not all available within the home. The requirement made at the last inspection regarding this has therefore been brought forward, and the inspector will assess this in full at the next inspection. Staff at the home have a range of skills and experience. Of six care staff, four have achieved NVQ level 3, one has achieved NVQ level 2 and is currently working towards level 3 and one carer is completing the Learning Disabilities Award Framework foundation training and will then commence NVQ training. There are five qualified nurses (learning disability), including the manager, working at the home. There is a comprehensive training and development plan in place. This is in addition to NVQ training, which is ongoing. New care staff complete the Learning Disability Award Framework induction and foundation training. Care practice observed was positive. Staff communicate effectively and respectfully with service users, and service users appeared to be relaxed in the company of staff. It was evident at the inspection that staff have positive relationships with the service users and know them well. JUDGEMENT – we looked at outcomes for standard(s) 42 The health, safety and welfare of service users are protected at this home. EVIDENCE: Records regarding health and safety matters were in good order. There was evidence in the records that the required checks and maintenance of safety and specialist equipment is carried out. Evidence that regular fire drills are carried out to ensure that service users and staff are aware of the procedure was seen in the records. It was noted that water temperatures are often several degrees below the recommended 43 degrees centigrade and it is recommended that these be adjusted. Risk assessments regarding health and safety matters are up to date and available. All staff receive training in health and safety.
