This inspection was carried out on 11th September 2006.

The inspector made no statutory requirements on the home as a result of this inspection and there were no outstanding actions from the previous inspection report.

What the care home does well

Southview provides a good standard of care in a homely atmosphere where service users were observed relaxing in the lounges and in their own rooms. The new owners have made significant improvements to the environment in the 6 months since ownership, making the environment safer and more attractive for service users and staff.

What has improved since the last inspection?

The bathroom in the lower hallway has been improved greatly, and to a high standard, and now provides a large shower room adapted for use by service users with impaired mobility. A new bedroom with en suite toilet facilities has been created and an application has been made to the Commission to increase the registered numbers from 18 to 19. The hallway leading from the main entranceway to the annex bedrooms has been redecorated and a new carpet has been fitted. The laundry room has been relocated in a newly rebuilt outbuilding which has improved these facilities considerably. Several bedrooms have been redecorated and fitted with new carpets and new furniture. New dining room furniture, of a type that is easier for service users to move over the carpeted floor, has been purchased.

What the care home could do better:

Considerable progress has been made in ensuring the National Minimum Standards are met and no requirements or recommendations are made at this time. The owners recognise that some of the service users who have lived at Southview for some time and who had a close relationship with the previous owners were finding the changes unsettling. Assurances were given that each service user would be consulted in light of the survey results to overcome these insecurities. The following advisory points were made: - where it has been identified that a service user is suffering from confusion the care plan should be expanded to contain more details as to how this confusion affects the person`s abilities and quality of life and how staff should support the service user in a consistent manner. - Pre-admission assessments should be completed in more detail to demonstrate the information from which a decision regarding the suitability of the home is being made.

The categories of registration are OP and DE(E) The home may accommodate a maximum of 18 older people (over the age of 65) who may also have dementia. 13/03/06 Date of last inspection Brief Description of the Service: Southview is a care home providing accommodation and personal care for 18 older people, aged over 65, who may also have dementia. Nursing care is not provided in this home. It is privately owned by Ashley Residential Care Ltd. The Responsible Individual is Mrs Anna Chapman. The present owners took over the running of the home on 7th March 2006. The home is a single storey detached property situated in the residential area of Lipson, Plymouth. It is close to local amenities and transport links. Southview offers 17 single bedrooms and one double bedroom: eight of the single bedrooms have en-suite toilet facilities. There are two bathrooms and four toilets for communal use. The home has a large lounge room and dining room. Accommodation is centred around an attractive courtyard and garden and all areas are accessible to the service users. There is on street parking available outside the home. The current weekly fees for the home range from £275 to £390 dependant upon individual service user’s care needs. Southview DS0000064360.V311732.R01.S.doc Version 5.2 Page 5 SUMMARY This is an overview of what the inspector found during the inspection. This unannounced inspection took place in the afternoon 11th September 2006. A tour of the premises took place and records relating to care, staff and the home were inspected. All 17 service users were spoken with during the visit, as well as two relatives: all were satisfied with the care they receive. Staff on duty were observed and spoken with in the course of their daily duties. The owner, Mrs Anna Chapman, was present throughout the majority of the inspection. At the time of the visit to the home all residents were provided with a survey form and envelope to allow them to comment anonymously and directly to the Commission regarding their experiences. Following the visit all staff and relatives were sent surveys forms also to allow for comment. Of the 11 sent to relatives, 3 were returned. These relatives were satisfied with the overall care but felt at times there were insufficient care staff on duty. Of the 15 surveys sent to staff, 3 were returned: 2 felt that they did not receive enough support to undertake their role. Ten of the 17 surveys given to service users were returned: 8 were very satisfied with the care and support provided; 4 said that the quality of the food had recently deteriorated and 2 felt they did not receive the support they required and they did not know who to make a complaint to. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 3 The quality in this outcome area is good. This judgement has been made using the available evidence including a visit to the service. Pre-admission assessments ensure the home is suitable to meet the needs of prospective service users. EVIDENCE: One newly admitted service user described how she had been made welcome and how well she had settled in. She said that the staff were kind and polite and she had been provided with information to support her decision to move into Southview. The owner had undertaken a pre-admission assessment: these assessments allow the home to ascertain and prepare for prospective service users individual needs, including religious and cultural needs. The inspector advised that this assessment should be completed in more detail to demonstrate the information from which a decision regarding the suitability of the home is being made. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 7, 8, 9, 10, 11 The quality in this outcome area is good. This judgement has been made using the available evidence including a visit to the service. Service users can feel confident that their personal and health care needs will be met. Service users were treated with dignity and privacy was respected. EVIDENCE: The inspector spoke to all residents to ascertain their views on living at Southview. Whilst a small number of residents expressed concerns that things had changed following the recent change of ownership, all acknowledged that their needs were being met. One service user felt the home wasn’t suitable for her as she wished to return to her own home and the owners were supporting her to have a home assessment to identify any aids and adaptations necessary. Ten service user surveys were returned to the Commission and of these 2 felt that they received inadequate support. As these surveys were returned anonymously, the owners have given assurances that they will consult with each resident to resolve these concerns. Service users had access to health care service services such as doctors, district nurses and the mental health team. The owners were advised that where it has been identified that a service user is suffering from confusion the care plan should be expanded to contain more details as to how this confusion affects the person’s abilities and quality of life and how staff should support the service user in a consistent manner. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 12, 13, 14, 15 The quality in this outcome area is good. This judgement has been made using the available evidence including a visit to the service. The routines in the home are relaxed. Service users can exercise choice over their lifestyle. EVIDENCE: At the time of the visit to the home, despite the unsettling time for some in coming to terms with the change in ownership, the service users spoke positively about the lifestyle in Southview. They said that they chose where they spent their day and the times they got up and went to bed. Four residents said the quality of the food was not as good as it had been. These comments were made anonymously through the use of written surveys and the owners gave assurances that through residents meetings and individual consultation they would consult each service user regarding their preferences. Two visitors said they were made welcome and were kept fully informed by the owners and care staff. Activities were planned on a daily basis although some service users said they did not always choose to join in. Personal interests were encouraged and service users were able to keep pets in the home, subject to management agreement. Since the change in ownership, the residents are now offered the opportunity to visit local places of interest. Service users and their relatives and friends are confident that their complaints will be listened to, taken seriously and acted upon. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 16, 18 The quality in this outcome area is good. This judgement has been made using the available evidence including a visit to the service. Service users can feel confident that any complaints they have will be taken seriously and that their views are listened to and acted on. EVIDENCE: The home had a written complaints procedure. Two service users who returned a survey said they did not know who to make a complaint to. This was discussed with the owners and assurances were given that each service user would be consulted to alleviate any concerns. Neither the home nor the Commission for Social Care Inspection have received any complaints since the last inspection. Three relatives returned surveys and all were satisfied with the overall care provided at Southview. None had made a complaint. The owner confirmed that adult protection training is planned for all staff members. The home kept a visitors book to record dates, times and names of all visitors to the home. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 19, 20, 21, 23, 24, 25,26 The quality in this outcome area is good. This judgement has been made using the available evidence including a visit to the service. Southview provides a pleasant, homely and safe environment that has been much improved over recent months. EVIDENCE: Considerable investment has been made in the environment over the past 6 months. One of the bathrooms that was very small and unusable by service uses requiring assistance has been converted into a shower room suitable for use by those with impaired mobility. The laundry room has been relocated to a newly rebuild out-building proving much improved facilities. The room previous used as the laundry room has been converted into a bedroom with en suite toilet facilities. The hallway leading from the main entranceway to the annex bedrooms has been redecorated and new carpet has been fitted. This gives a much better first impression of the home. The call bell system has been replaced with one that can only be cancelled at the point of call and Southview DS0000064360.V311732.R01.S.doc Version 5.2 Page 15 allows for mobile call bells to be given to service users to use wherever they are in the home, thus giving them more confidence to move around the home knowing that assistance can be summoned at all times. The garden has been tidied and overgrown trees and shrubs have been cut back making this a very pleasant area to sit and also improving the light to those bedroom that overlook the garden. Several bedrooms have been refurbished and new carpets and furniture purchased. The owners have plans to redecorate all remaining areas of the home, including providing a new kitchen and replacing all equipment as well as improving the remaining bathroom. Service users’ needs are met by the numbers and skill mix of staff. Service users are in safe hands at all times. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 27, 28, 29, 30 The quality in this outcome area is good. This judgement has been made using the available evidence including a visit to the service. Service users can be confident that the staff group will have a good understanding of their care needs. EVIDENCE: As noted in the summary, the owners recognise that some of the service users have been unsettled with the change of ownership. A number of new care staff have been appointed and some service users are concerned over the young age of some of the staff: all care staff are over the age of 18 and no staff member under the age of 21 is left in charge of the home. The 3 surveys returned by relatives and 2 of the 10 returned by service users said there were insufficient care staff on duty, however 15 of the 17 service users spoken to by the inspector said that they were assisted promptly and unhurriedly by care staff thus indicating that there were sufficient staff available. Examination of the duty rota indicated that there were 2 care staff on duty throughout the day and evening, and 1 waking and 1 sleeping-in member of staff during the night. Care staff are supported by catering staff everyday from 8am to 2pm and for 5 evenings a week from 4.30pm to 6.30pm. The owner is also in the home most weekdays. Training records indicated that the owners have a positive approach to training and have invested in a training programme that addresses the specific care needs of the residents. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 31, 32, 33, 25, 36, 37, 38 The quality in this outcome area is good. This judgement has been made using the available evidence including a visit to the service. The owners have a good understanding of where the home needs to improve and is working closely with the staff team and service users to ensure the improvements take place. EVIDENCE: Mrs Chapman, the owner will be applying to the Commission to register as the manager for Southview. Throughout the inspection Mrs Chapman spoke to all the service users in a warm, friendly and courteous way. She has given assurances that she will consult formally with all the service users to alleviate their concerns over the changes taking place within the home and to involve them in the planning of further improvements to the services provided. Documentation relating to health and safety issues was well recorded and Mrs Chapman was able to demonstrate her commitment to ensuring Southview is a well managed home that meets service users needs in a safe environment.
