The inspector made no statutory requirements on the home as a result of this inspection and there were no outstanding actions from the previous inspection report.

What the care home does well

The home provides generous sized accommodation for all the service users both in terms of individual rooms and communal space. Information about people is sought in advance to ensure that the home is appropriate for his or her needs and the care plans outline this carefully. The service users with whom the inspector spoke expressed satisfaction with the care provided and the way in which it was delivered. Service users who had chosen to spend their day in their rooms were not met on this occasion so their views could not be solicited.

What has improved since the last inspection?

Staffing levels have been improved by the employment of two non UK nationals who appear to have been well received by service users. Housekeeping staff have been employed to deal with domestic routines especial in service users` own rooms.

What the care home could do better:

The main area needing attention relates to staffing records. Training details need to be filed. Appraisals and supervision with staff need to start. The registered manager is aware of this and she needs to ensure that she has sufficient time set aside in her working week to deal with all administrative tasks.

It is situated in the village of Whitston which is midway between Launceston, Bude & Holsworthy. The home, which is privately owned, is relatively new having been created from redundant farm buildings. It provides rooms of a good size, all with ensuite facilities for the service users. There are large communal areas in different parts of the home for the use of all. Externally the courtyard around which the home is bult is tarmacd with a water feature, although at the time of this inspection, it was not working. There is good car parking available. This was an unannounced inspection between the hours of 9.15am and 1.15pm. The inspector looked at records, care plans and medication. Staff recruitment and training was discussed with the registered manager. The home had one vacancy. Nine service users were met and they commented positively on their experiences in the home. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 1,2,3,4,5 Service users are provided with information about the home and details are sought in advance to ensure that their needs can be met. EVIDENCE: The statement of purpose has been reviewed and felt still to reflect the services offered. All service users receive a copy of the home’s brochure, which includes the service users guide. The registered manager said that all service users are provided with a copy of the home’s contract setting out the terms and conditions of residency at St Anne’s. New service users are visited by the registered manager prior to offering accommodation to ensure that a person’s needs and the home’s abilities are compatible. Information is sought from other professionals and agencies, both before admission and where necessary, once a person is living in the home. The home offers limited day care to people living locally or others who are able to provide their own transport. Similarly respite could be offered if a vacancy was available. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 7,8,9,11 Service users’ needs are identified and addressed. This is supported by documentation. EVIDENCE: All service users had full care plans in place noting problems and the action required to address these. Care plans were being reviewed regularly and complimented by daily entries from the care staff. Advice from health care professionals and other agencies is sought when required. Risk assessments and moving & handling assessments are in place as needed. Staff are about to start an infection control course. Medication is stored in a locked and secure trolley. Staff who administer it have received training (safe handling of medication) that adheres to the home’s written policies and procedures. A further course is being arranged. A new list of staff signatures of who may administer medication is needed, the present one being out of date. This practice will be passed to the Pharmacy Inspector of the Commission for Social Care Inspection for comment. The way in which the home cares for people who are ill and dying was discussed. A member of staff will sit with the person if a family member is not available. The registered manager said that the home has received positive comments from families about the care their relative had received. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 12,14, The home provides opportunities to maximise service users’ choice and provide a varied programme of stimulation. EVIDENCE: The registered manager listed the activities that are available for service users to join in with if they choose. There are activities which all can join in, such as church services, bingo and quizzes etc and other more individual past times such as Scrabble or going out for a drive. A video afternoon and music/sing songs are arranged periodically. The registered person’s mother visits the home regularly and entertains on the piano. Service users are encouraged to exercise choice to the level that they are able. This may be choosing the clothes they wish to wear, where to sit and whether or not to join in activities. Food also offers opportunities for choice as service users can have an alternative to the main meal if they ask. The kitchen and menus were not inspected but service users said the food was good. The kitchen staff and some carers have got their basic food hygiene certificates. Service users and their relatives and friends are confident that their complaints will be listened to, taken seriously and acted upon. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 16,17,18 The home’s complaints and PoVA policies have not yet been needed. Service users’ can receive help in maintaining their legal rights. EVIDENCE: The complaints procedure is included in the service users guide. As many service users may not remember where they have put their copy, the home should consider displaying the complaints procedure in the home for reference. No complaints have been made to the home or received by the Commission for Social Care Inspection. How a complaint would be recorded was discussed. It is suggested that a book in which compliments, concerns and complaints are recorded should be kept. The registered manager said that carers had completed a module on abuse as part of their National Vocational Qualifications course. She herself had a good awareness of the procedure to be followed in a case of suspected abuse and she intends to go to one of the Social Service’s seminars on Whistle Blowing & PoVA. Postal votes were made available for all service users who wanted them. Age Concern would be approached for anyone needing advocacy help who did not have other suitable contacts. Service users needs are met by the numbers and skill mix of staff. Service users are in safe hands at all times. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission considers Standards 27, 29, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 27,28,29,30 The service users are cared for by staff in sufficient numbers and who receive training appropriate for their work. The training that is received needs to be better documented. EVIDENCE: Staffing has improved with the employment of two non-UK nationals in the home. The rota indicated that there were sufficient staff on each shift at the time of this inspection. The registered manager said that the home employed juniors in the evenings who helped make tea, clear away dishes and generally tidy up. She was aware that they could not undertake caring duties. The home employs housekeeping staff to attend to cleaning and domestic jobs. Recruitment follows good practice with written references, Criminal Records Bureau & PoVA checks being obtained. National Vocational Qualifications are now held by 11 members of staff at level ll & lll. There was a discussion about training, both induction and on going. Induction is based on TOPSS recommendations. The registered manager said that new staff were undertaking this but staff files showed the check sheets were not being returned to indicate that these had been fully accomplished. A copy of the new standards was left for the registered manager’s information. The benefit of courses such as bereavement and Alzheimer’s Disease was discussed. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 31,32,33,34,35,36,37,38 The registered manager is qualified and whilst she safeguards the service users’ interests, more time needs to be dedicated to administration tasks. EVIDENCE: The registered manager has achieved her National Vocational Qualifications level lV & registered manager’s award. She is about to start a further qualification relating to medication. Minuted staff meetings take place quarterly but these are not always well attended. At present the home has a system of ‘supervisors’ who are the more qualified and experienced staff who direct others. The registered manager said that as the supervisors achieve level lll National Vocational Qualifications, they may be designated as senior staff. St Annes (Holsworthy) D52-D04 S9221 St Annes (Whitstone) V236319 250805 Stage 4.doc Version 1.40 Page 17 St Anne’s is planning on giving service users and relatives a questionnaire to formally ascertain their views on the quality of services provided by the home. At present staff receive verbal comments from service users and families on a casual/ad hoc basis. With regard to finances, the home’s insurance certificate was displayed. The registered person will be asked to provide a statement from their auditor/accountant detailing the home’s financial viability (as per standard 34). The home does not involve itself with service users’ money, preferring that family or a trusted friend undertakes this. Any extra expenses are billed on a monthly account but savings can be held in safekeeping if needed. It is suggested that the home determines on admission whether or not a service user has appointed someone to act as Power of Attorney for them. Staff supervision and appraisals have not yet started. The registered manager is aware of the need to do this but has not, until recently, had time to address this as she has been involved in ‘hands on’ work due to the general staffing problems. She said that this had now eased and she knew that it was a priority. It is important the registered manager has sufficient dedicated time for administration in view of the increasing but essential recordkeeping and paper work. The records required by statute were seen. Staff have recently had fire training; the accident book complies with the requirements of the Data Protection Act 1998; the premises were inspected by the Environmental Health Officer (Health & Safety) in February 2005 and risk assessments on the property have been documented. Most carers have their first aid qualification and moving & handling training but some new staff have yet to do these courses. Compliance with hot water surfaces and outlets were incorporated into the home as it was built. Generally St Anne’s appeared to adhere to safe working practices. STATUTORY REQUIREMENTS This section sets out the actions which must be taken so that the registered person/s meets the Care Standards Act 2000, Care Homes Regulations 2001 and the National Minimum Standards. The Registered Provider(s) must comply with the given timescales. No. 1. Standard OP36 Regulation 18(1)(2) Requirement The registered provider must provide the Commission with a detailed plan setting out the homes proposed timescales for the implementation of staff supervsion, appraisals and the completion of staff training records. The registered manager should ensure that she has sufficient dedicated time to undertake all the administrative tasks associated with her duties. An objective assessment of the homes effectiveness and quaility should be undertaken.
