Date of last inspection Brief Description of the Service: St Georges Nursing Home 9one of the BUPA homes), is situated in a rural setting on the outskirts of Cobham, close to the renowned Wisley Gardens. St Georges is a single storey, purpose built home designed specifically to provide a safe and secure environment for service users requiring dementia-nursing care. The carefully planned layout of the home provides a safe environment for service users to walk around freely. St Georges has a number of reception rooms, providing a central meeting place where service users can get together to chat, watch television or read. A feature of the home is the pleasant central secure courtyard garden, where service users can spend strolling or relaxing on the patios. All bedrooms are furnished to a high standard with en-suite facilities, and are equipped with a television and telephone point, allowing each service user to stay in touch with life outside the home. Modern safety features fitted in each bedroom include 24-hour nurse call system, smoke detectors and thermostatically controlled radiators. Meals are prepared daily by the Chef, using fresh produce and are served in the home’s dining room or in the service users bedroom if required. A varied menu provides a nutritious diet, with frequent snacks and light meals served throughout the day. The activity suite is well staffed and service users are encouraged to participate in the programme of activities, which include a varied, and entertaining social calendar. This is an overview of what the inspector found during the inspection. This was the second inspection to be undertaken in the Commission for Social Care Inspection year April 2005 to March 2006. This inspection was announced and was carried out on the 6th October 2005, by two inspectors and lasted for four hours and twenty minutes. The inspection date was posted around the home, with a request that the inspector would be available to meet and speak with service users relatives. On the day of inspection no relatives were available to speak with the inspector, but the inspector had received a very good response to the relatives questionnaire sent out prior to the inspection. Comments made by the relatives were favourable, praising the staff for the hard work they were doing with their relatives. The first part of the inspection was spent with the newly appointed manager, discussing the format of the inspection, reviewing service users files and care staff files. Only one service user in this home has the ability to speak with the inspectors, therefore the information obtained for this report is sought from documented records found in the service users files, and from information gained from speaking to staff working at the home. The second part of the inspection was spent observing service user and care workers interactions, speaking to service users, and touring the home. This was followed by sharing of information with the manager about the findings of the inspection. The home has made some improvements to its décor, to facilities to enhance the lives of service users and staff since the last inspection. The new addition to the home is now completed and the two bedrooms that were out of use are now functional. The new manager Mrs Carol Purdy has been in post since 12th September 2005. What the service does well:
Care workers at this home say they are knowledgeable about the needs of the service users. Information gathered from the Relatives satisfaction survey showed that relatives rated the attention service users received from care staff as high. They also documented the cleanliness and maintenance of the home as very good. The grounds and gardens were also rated highly. There was

The manager informed the inspector that she plans to restart the monthly meetings with service users and their relatives, to improve communication within the home and service users and their relatives by the introduction of a monthly newsletter. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 1 Service users and their relatives are given good information about the home, to enable them to make an informed choice. EVIDENCE: The home has an up to date service user guide and statement of purpose, which outlines in plain English the rules and regulations of the home and the rights of the service users. The intended outcomes for Standards 7 – 11 are: 7. 8. 9. 10. 11. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 9 10 The homes policy and procedure on administering medication is followed. Service users are treated with dignity and respect. EVIDENCE: None of the current service users at this home are risked assessed as capable to self-administer their medication. All medication to service users are administered by a Registered nurse under the homes drug administration policy and procedure and the Nursing and Midwifery Council guidelines on administration of medicines. Service users views were not obtained on this standard due to their medical condition. However, in discussion with care workers and from evidence seen in the service users file and responses made by relatives on the pre inspection questionnaires, the inspector formed the opinion that service users are treated with respect, and dignity and their privacy in upheld and maintained. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 14 15 Service users are consulted on a regular basis about choices in their daily lives. Meals are nutritious and are served in a pleasant dining room. EVIDENCE: Making the right choices is sometimes very difficult for the service users at this home. However, the inspector has observed care workers taking time to discuss choices with the service users and encouraging them to make a decision even if the decision is not to comply. The inspector noted that the service user’s decision was accepted. An alternative was offered and the negotiation started all over again. It was good to see how attentive the care workers were to the needs of the service users. A four week menu was reviewed which included two weeks prior to the inspection the week of the inspection and the week following the inspection. It was noted that service users had a choice of hot meals, that appeared nutritious, and that they had three main meals during the day with hot and cold beverages and light snacks in between. On the day of inspection the inspectors sampled the mid-day meal. The presentation was very good, the meat tender and tasty, the vegetables were perfect even for service users without teeth. Service users spoken to about the meal said it was lovely. The

JUDGEMENT – we looked at outcomes for the following standard(s): 16 The home has a satisfactory complaints system with some evidence that service users welfare are taken seriously and acted upon EVIDENCE: The Pre Inspection Questionnaire stated two complaints were received and have been dealt with within the home’s policy timescale. The new manager has been in post since the 12th September 2005 and she has informed the inspector no complaints have been received by the home since her starting date. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 26 Recent investment has improved the appearance of this home maintaining a safe and comfortable environment for those living and visiting the home. EVIDENCE: The home has bought a number of beds, armchairs, fitted new carpets, and curtains for the use and comfort of the service users. The reception area has had a new desk, which enable more information about the home to be more visible to the visitors to the home. During the tour of the home it was noted that discrete white boxes containing non-toxic deodorisers were placed high on the walls in the corridors to help minimise offensive odours. JUDGEMENT – we looked at outcomes for the following standard(s): 30 Staff morale is high resulting in an enthusiastic work force that works positively with service users to improve their whole quality of life. EVIDENCE: Discussions with care workers and review of a random sample of their files and the homes training record revealed that care workers are trained to do the work they are asked to perform. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 31 33 35 The manager is well supported by the senior staff of the company and the home in providing clear leadership throughput the home. EVIDENCE: The new manager is only recently appointed to this home but has a long record of managing BUPA homes. From observations made by the inspectors and from feedback from care workers, the home continues to operate in the best interests of the service users. The home manages the pocket monies of those service users who requests this. Their relatives, Power of Attorneys, or social services manage Service users main finances.
