Conditions of registration: None Date of last inspection 3rd February 2005 Brief Description of the Service: This home has two stories and was purpose built. It provides both personal care, and nursing, for up to 38 elderly persons, all within single rooms. There is a passenger lift to give access to the first floor. The home has recently undergone internal alteration to improve the layout. Although the grounds are not extensive they are adjacent to the local church, this and the absence of a boundary fence, gives them a spacious feel. A sheltered patio area is located at the rear of the home. The home is in a quiet residential area of Washington, close to the main North / South highway (A182). There are some local shops and facilities within walking distance, and a large retail development (The Galleries) is only a few minutes away by motor transport. This is an overview of what the inspector found during the inspection. This unannounced inspection took place over one day on 6 July 2005. One inspector carried out the inspection. The manager was present at the time of inspection. The views of people living at the home were gathered during the day of inspection, by talking to them, their visitors and by inspecting survey documents. Positive feedback was received from those who [contributed to the inspection]. the inspector spoke to - avoid. The manager was interviewed and documents were sampled including care plans. The morale in the home has improved and the home is better managed. Inappropriate door handles have been replaced, and fire precautions have improved.

The intended outcomes for Standards 1 – 6 are: 1. 2. 3. 4. 5. 6. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 1, 2, 3 The home provides comprehensive information to prospective service users to enable them to make an informed choice about living there. Service users do have contracts and a copy of terms and conditions. A comprehensive assessment is carried out to ensure the home can meet service user’s needs. EVIDENCE: The home provides a comprehensive guide for service users, written in easily understood language. The service users relatives, who were spoken to, confirm they were given adequate information and were able to visit the home before their relatives were admitted. The manager confirms that an assessment of service users needs is carried out before people come into the home. Care plans contain comprehensive assessments of service user’s needs. Examples of service users contracts were seen and the manager confirms that all service users receive them. The intended outcomes for Standards 7 – 11 are: 7. 8. 9. 10. 11. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 7, 8, 9, 10, 11 The home carries out comprehensive care planning that takes into account service user’s health, social, spiritual and cultural needs, and ensures these can be met. Medication policies and procedures are appropriate and protect service users. Service users are respected and their dignity is maintained by staff. EVIDENCE: The assessments and care plans are comprehensive and well written. They include nutritional assessments, pressure damage risk and moving and handling. There are good risk assessments relating to the deployment of bed rails. There is written evidence that clinical specialists are involved in service user’s care. The Chiropodist was visiting the home on the day of inspection and service users were receiving treatment in the privacy of their own rooms. Social and cultural needs are also included in what? as well as more sensitive issues such as service users wishes and attitudes towards end-of-life decisions and resuscitation. Service users comments such as, “When I was poorly, they looked after me well,” and, “the nurse on this floor is lovely with the residents,” tend to confirm that they feel cared for by staff. Visitors and relative echo these sentiments,

Nursing staff told the inspector confirmed that drugs to aid mobility or pain relief were given to service users before they were helped out of bed and records confirmed that timings were appropriate. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 12, 13, 14, 15 Service users health care needs are met and the home has most of the equipment and facilities it needs. The home has good community links and encourages contact with service users, from friends and families. Service users have access to all healthcare services that they need. Service users are assisted to make choices and exercise control over how they live. The home provides appetising meals and appropriate choice, and support, to ensure service users receive a good diet. EVIDENCE: The home is well equipped with assisted bathing equipment, mobility aids and hoists. A new call system is in place to ensure more rapid response from staff. Unfortunately the home has no “loop” system for hearing aid users. Some service user’s told inspectors confirmed that they could choose where to spend their time and what they enjoyed doing. One service user had their own refrigerator, as well as radio, TV and other items of interest in their room. The manager stated that service users are assisted with recreational pursuits by a recreational therapist. There is evidence of art and craft work in service users rooms and people are pleased with what they have achieved. One
St Georges Nursing Home B52-B02 S18208 St Georges V217731 5 Jul 05 Stage 4.doc Version 1.40 Page 12 service users told the inspector that, “ There is a man, who is very patient with us, who does art with us.” Other service users said that they were able to go out into the garden or visit local facilities such as shops, and that staff helped them if they could not go out on their own. The manager confirmed the home maintained links with the church that is adjacent to the home. A number of visitors were seen during the time of the inspection and visitors confirmed that they are welcomed into the home at any time. The meal that was seen was well presented and looked appetising. The dining tables were nicely presented with tablecloths, condiments and napkins. Menus were available and offered a variety of meals. The kitchens were clean, well organised and staff were enthusiastic about their role in the home. Service users needing help to eat were assisted appropriately. Service users commented positively on the quality of the food. Comments such as, “There’s always good food here”, were typical of many offered by service users. Service users’ legal rights are protected. Service users are protected from abuse. JUDGEMENT – we looked at outcomes for standard(s) 16, 18 Service users and relatives are able to raise issues with staff and the manager and they are sure that these will be acted upon. The home has appropriate training and systems to ensure that service users are protected from abuse EVIDENCE: Complaints are recorded well and the manager states that she has an open door policy. Relatives confirm that they would be happy to raise issues with staff or the manager if they had concerns. Service users have access to safe and comfortable indoor and outdoor communal facilities. Service users have sufficient and suitable lavatories and washing facilities. Service users have the specialist equipment they require to maximise their independence. Service users’ own rooms suit their needs. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 19, 20, 21, 22, 23, 24, 25, 26 The environment is well maintained to ensure service user’s safety and is clean, hygienic and pleasant to be in. Communal facilities are easily accessible inside and outside of the home. Service users have most of the equipment they need to meet their needs. Bedrooms are safe, comfortable, well-furnished and meet service users needs. EVIDENCE: The home is in the process of converting surplus space into a dedicated hairdressing facility and a new call system has been installed to ensure staff can reach service users quickly and efficiently. The home was warm, well ventilated and well decorated. Heating is provided from a safe, low surface-temperature source. No extremes of temperatures were experienced during the inspection. The home has good levels of natural and artificial light. The home has ramped access and appropriate lifts. There is ample space in communal areas and service users have a variety of lounge, dining and sitting space to choose from. Unfortunately there was no loop system in any area to help service users with hearing problems.

Bedrooms are reasonably well furnished and contain personal items belonging to the occupants. The soft furnishings match the décor and the overall effect is homely. A number of service users have been provided with adjustable beds. There are touch-pad switches at wheelchair user’s height. There are adequate numbers of toilet and bathing facilities and these are fitted with appropriate handrails, hoists or other mobility aids. The home now has suitable disposal facilities in the sluices on each floor and the home is kept clean and tidy in all areas. There were no unpleasant odours in the home at any time. It was noted that one staff member did not have a uniform or protective clothes and the necessity of this was discussed with the manager. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission considers Standards 27, 29, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 27, 28, 29, 30 The home has an appropriate number of staff, with the right skills, to meet service users needs Recruitment processes are appropriate and carried out in a way that protects service users. Service users needs are met by a staff team who receive appropriate training and display competency when carrying out their work. EVIDENCE: Duty rotas were examined and they indicated that there are appropriate numbers of staff employed. The manager confirmed this and the number of staff in the home of the day of the inspection was appropriate. The manager states that she adjusts staff to meet the dependency of service users. The manager and staff confirm that induction and training is carried out and records support this. Service users confirm that they feel secure and are attended to quickly, one stated that, “All the lasses are there for me.” It was observed that call buzzers were responded to promptly. As noted in an earlier section, it was observed that drug doses are given with thought and consideration to the effect they will have on service user’s day, not simply as part of a routine. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 31, 32, 33, 38 The home is run by an experienced and competent manager whose leadership and approach is benefiting service users. The health and welfare of service users is being protected by appropriate management. EVIDENCE: The manager is an experienced clinician who has managed care homes in other parts of the North East. There appears to be an improvement in morale since the last inspection. The manager states that the staff are working as a more cohesive team and helping each other out when necessary. Staff confirm this. One person said that, “We now have a leader.” Another confirmed that the manager is “approachable” but would, “stand no nonsense.” Service users and relatives both confirm that the manager is approachable and that they would be happy to raise problems if they had any. Outstanding health and safety issues from the previous report have been dealt with.
