What the care home does well

St Lawrence Lodge provides a good service for vulnerable residents in a pleasant and secure environment. The resident`s are consulted in the day-to-day issues concerning the running of the home. The home cares for the resident`s families and encourages them to remain involved in their relative`s care. There is a stable enthusiastic work force that works well as a team. The carers are respectful towards the residents, their families and also towards each other. The manager is committed to the National Vocational Qualification system. The majority of carers have achieved level two and seniors level three.

What has improved since the last inspection?

The entire wording in the complaints procedure has been amended to provide the residents and their representatives with accurate information. The cook`s hours have been increased to give an improved service during the evening meal.

What the care home could do better:

The communal areas of the home would benefit from redecoration and areas of maintenance should be recorded.

Service users to include up to 20 OP, up to 20 DE (E), up to 7 PD (E) and up to 1 SI (E). 31st August 2005 Date of last inspection Brief Description of the Service: St Lawrence Lodge is a residential care home adapted to meet the needs of 20 service users. Previously a Victorian vicarage, the adaptations now provide accommodation on two floors. In total there are 18 single rooms, three of which have en-suite facilities, and one shared room. The communal area is on the ground floor. The home is open plan in nature, having a large lounge and smaller dining area to the rear of the room. The dining area opens onto the small garden that is well used in the better weather. Adapted baths and toilets are located throughout the home. Car parking is available to the side of the building.The home is situated in a residential area, close to the centre of Denton. There is a shopping centre and market within walking distance. The adjacent towns of Ashton and Stockport are accessible by public transport. The service users who live at St Lawrence Lodge have been assessed as requiring residential care. The Commission for Social Care Inspection has registered the home to provide care for older people who may have dementia or a physical disability. This was an unannounced inspection carried out over five hours. St. Lawrence Lodge provides personal care for up to 20 people over 65 years of age. It is privately owned. The manager is registered with the Commission for Social Care Inspection and was present during the inspection. In addition to teams of carers the home employs domestic, catering and maintenance personnel. In total six residents assisted the inspector with their comments and experiences of life at the home. Their comments in general were positive and it was obvious they had made friends and felt relaxed and comfortable. One resident invited the inspector to look at her room of which she was justifiable proud. The inspector also observed care practices, the serving of the main meal and medication administration. The requirements made at the previous inspection were also assessed for compliance. Comment cards were left for the residents and visitors to complete. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 2 The residents have a copy of the written terms and conditions and complaints policy on admission to the home. Standard 3 was inspected at the previous unannounced inspection (August 2005) when it was judged to meet fully with the intended outcome. Intermediate care (Standard 6) is not provided at St Lawrence Lodge. EVIDENCE: At the previous inspection carried out in August 2005 there was a requirement for the wording in the terms and conditions and accompanying complaints procedure to be amended and brought up to date. The registered manager has now satisfactorily amended the document. St Lawrence`s Lodge DS0000005578.V274878.R01.S.doc Version 5.1 Page 9 Health and Personal Care The intended outcomes for Standards 7 – 11 are: 7. 8. 9. 10. 11. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 7 & 10 The resident’s health and personal care needs are in general well met but not always reflected in the documentation, which could result in staff being inconsistent in their approach. The residents are treated in a dignified manner and their privacy is respected. Standards 8 and 9 were inspected at the previous unannounced inspection where they were judged to meet fully with the intended outcomes. EVIDENCE: A resident who spoke with the inspector stated that he was considering moving into sheltered accommodation. He described how he was being assisted by the staff to regain previous life skills. It was apparent that the home was working very hard (in conjunction with the Social Services Department) to ensure the resident achieved his full potential in order that he could return to the community. This is considered to be very good practice but it was disappointing to note the individual care plan failed to reflect the action the care staff need to take in order to meet specific needs. They clearly described their experiences of life at St Lawrence Lodge and their satisfaction with the service. The residents were certain their privacy was respected. They had made friends at the home and stated they felt safe and secure. A member of staff was observed to provide emotional support and guidance in a quiet but effective manner to a distressed resident. On another occasion a carer was observed to provide unobtrusive assistance during the mealtime. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 14 The residents are enabled to influence their day-to-day life and decisions taken by them are respected and upheld. Standards 12,13,14 and 15 were inspected at the previous unannounced inspection (August 2005) where they were judged to meet fully with the intended outcomes. EVIDENCE: At the last inspection the main meal of the day had been changed to the evening, for a trial period. This situation was reviewed during a residents meeting where it was unanimously agreed to revert to the previous timing of the main meal to lunchtime. The inspector observed this meal. It was served by the carers and apparent that the residents had made a choice from the menu displayed within the home. The meal was a social occasion with the residents chatting one to another and with the carers. Several residents commented on their enjoyment of the meal and thanked the staff. One resident requested not to have his meal at this time but would like it later in the day. This was acknowledged and presented no problem to the staff. The meal was unhurried and assistance was provided in a discreet and considered manner. Service users and their relatives and friends are confident that their complaints will be listened to, taken seriously and acted upon. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 16 & 18 Systems are in place to protect the residents from abuse and ensure complaints are investigated. EVIDENCE: At the previous unannounced inspection (August 2005) there were requirements for the complaints procedure (contained within the terms and conditions) to be amended and the terminology brought up to date. This has now been completed and the registered manger was advised to remove the old copies and replace them with the corrected document. Protection of vulnerable adults training has been arranged through the Stockport Social Services for the senior staff. This will enable them to “cascade” the training to the other staff members. It is understood that all carers who have achieved the National Vocational Qualification (NVQ) at level 2 have also received training in abuse awareness. Service users’ own rooms suit their needs. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 19 & 23 With minor exceptions the resident’s live in a safe and well-maintained environment and the service users rooms’ suit their individual needs. Standards 25 and 26 were inspected at the previous unannounced inspection (August 2005) where they were judged to meet fully with the intended outcomes. EVIDENCE: The home employs a maintenance man to carry out the day-to-day repairs and redecoration. He carries out the repairs at the request of the senior staff but does not complete a record of the jobs he has completed. An officer of the Greater Manchester Fire Authority also observed this during a recent inspection of the home. In order to comply with the fire officer’s comments concerning doors failing to close completely to the rebates the maintenance person had made certain repairs and adjustments. It was observed that the door from the lounge into St Lawrence`s Lodge DS0000005578.V274878.R01.S.doc Version 5.1 Page 14 the bedroom area and the door to the laundry remain as the fire office had observed. The registered manager was advised accordingly. The registered manager stated there are plans to redecorate and recarpet the main communal areas of the home as it was agreed that these areas were beginning to look a little shabby. A resident invited the inspector into her room. The room had been nicely personalised and the resident was obviously very proud of it. The room had been redecorated and new carpet laid to her personal choice. Service users’ needs are met by the numbers and skill mix of staff. Service users are in safe hands at all times. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 27 The home employs staff in numbers to meet the assessed needs of the residents. Standards 28, 29 and 30 were inspected at the previous unannounced inspection (August 2005) where they were judged to meet fully with the intended outcomes. EVIDENCE: The manager was required to review the cooks’ hours to provide cover for all meal times at the previous inspection. The inspector was advised that this had now been achieved thereby releasing the senior staff from catering duties, as had previously been the case. The manager continues to support carers to complete the National Vocational Qualification at levels two and three. She is very skilful in seeking out further and specific training for all levels of staff. During the afternoon of the inspection a large group of staff were involved in fire awareness training carried out by an external trainer. This is a very stable workforce, which serves to provide a continuity of care to the residents. From observation the carers were attentive and responded to the residents in a respectful manner. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 38 Whilst the health, safety and welfare of the residents, staff and visitors is promoted and protected, none compliance with the Greater Manchester fire officers requirements have the potential to put them at risk. Standards 31,33 and 35 and were inspected at the previous unannounced inspection (August 2005) where they were judged to meet fully with the intended outcomes. EVIDENCE: An officer of the Greater Manchester Fire and Rescue Service recently inspected fire precautions at the home. He made observations and submitted his written report concerning the precautions. Included with the report were details of certain doors that required maintenance in order that they closed fully to the rebate. The manager was advised concerning this issue.
