Total number of service users not to exceed a maximum of 30 Date of last inspection 15th June 2005 Brief Description of the Service: Stonehaven Healthcare Ltd; is registered to provide accommodation and personal care for up to 30 older persons. St. Petrocs care home is situated in Bodmin close to the shops, public transport and community facilities. It is a detached house, set back from the road within large well-maintained grounds and ample parking space provided to the front of the property. St. Petrocs provides bedrooms on the ground and first floors with a passenger lift and stair lift provided to assist service users. This is an overview of what the inspector found during the inspection. This was an unannounced inspection to review the standards of care provided at the home when the registered persons were unaware it was planned. I arrived at 11.30 am and left at 3.30 pm. During this time I met service users, care staff and the registered manager. The registered manager assisted me in looking at records kept to monitor the welfare of service users and discussed developments at the home. I met the majority of service users either in the communal areas or in the dining room where the main lunchtime meal was served. The intended outcomes for Standards 1 – 6 are: 1. 2. 3. 4. 5. 6. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 1,2,3 and 4. The home is providing appropriate information to assist service users and advocates in making an informed choice about where to live. EVIDENCE: Stonehaven Healthcare have been registered in respect of this home since November 2003. Their statement of purpose is available within the home and a service users guide has been issued to each service user. These documents explain the services and facilities available within the home as part of the contract and that transport arrangements, hairdressing, chiropody and newspapers can be provided at the home and are at the service users expense. The registered manager and a senior carer will visit the prospective service users at their own home or in hospital prior to admission to complete an assessment process. These records include information received from other agencies to assist in appropriate admissions to the home. The intended outcomes for Standards 7 – 11 are: 7. 8. 9. 10. 11. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 7,8,9 and 10. Key staff maintain appropriate care plans for service users, that ensure the health needs of service users are met, with evidence of multi disciplinary work occurring to provide for service users welfare. EVIDENCE: Detailed assessments and plans of care highlight service users needs with the appropriate action and review procedures that include service users. Senior staff and carers are involved in daily handover procedures to facilitate the necessary communication of the changing care needs and support required by service users. The registered manager and staff have been involved with the Osteoporosis Society in the assessment and prevention of falls training. This has resulted in a reduction of falls occurring at the home. The homes accident records and the procedures to report important events to the Commission in reference to regulation 37 are appropriately maintained. A range of pressure relieving equipment is provided for service users as required and appropriate liaison occurs with the community nursing service to provide for the welfare of service users. Service users are registered locally with general practitioners at the Stillmore House Surgery and at the Carnewater Practice. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 12,13,14 and 15. Service users are receiving good meals at the home, in comfortable surroundings and consultation occurs regarding choice and preferences. EVIDENCE: Service users expressed approval of the visiting arrangements and stated that relatives are made to feel welcome at the home. Service users are mainly dependent on relatives and friends to access community facilities. The registered manager stated that occasional summer trips are organised and a programme of in house activities is maintained. I met service users in the dining room as the main meal of beef stew, dumplings and vegetables was served. A choice of main meal was available and cherry pie; yoghurt and fresh fruit were also served. Service users expressed approval of the meals provided and confirmed that the home had provided preferences as requested at the service users meetings. The dining room presented as a comfortable environment with linen tablecloths, serviettes and flowers on each table. The registered manager stated that she assists with serving meals on a daily basis to maintain contact and as a quality assurance measure. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 16 and 18. The registered manager uses the policy and procedures at the home to assist in the protection of service users. EVIDENCE: The complaints procedure containing the Commissions contact number and address was displayed in the home and is detailed in the service users guide. The registered manager maintains a record of complaints received and how these have been dealt with appropriately at the home. The registered manager has previously instigated appropriate POVA procedures to provide for the welfare of service users at the home. There is a policy at the home regarding staff accepting gifts from service users and precluding staff involvement in the making or benefiting from service users wills. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 19, 20, 21, 22, and 26. Stonehaven Healthcare is providing a good standard of décor and maintenance to ensure service users live in a safe and comfortable environment. The planned and necessary structural work to the main lounge is taking longer than expected; leaving service users without appropriate communal space. EVIDENCE: The 2 communal lounge areas are temporarily unavailable to service users. Stonehaven have informed the service users of the timescales involved. There is a separate dining room and a conservatory at the front of the home offering a seating area overlooking the garden. Service users have sufficient bathroom, shower and toilet facilities. The home provides a call bell system, a passenger lift, stair lift, specialist baths, bath hoists, mobile hoists, adjustable beds, grab rails and pressure relieving equipment. Single rooms are provided apart from 3 double rooms. The home presented as clean, pleasant and hygienic. Carpet repairs or replacement are required in the conservatory area and 2 potential tripping hazards were discussed with the registered manager. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 27, 28, 29 and 30. The registered manager is providing suitably trained staff, in appropriate numbers to provide for the welfare of service users. EVIDENCE: There is a minimum of 2 carers at all times with 4 carers at peak times of activity, in addition to the registered person and ancillary staff. One waking night carer and an on call (sleeper) are provided for service users that require attention at night. The registered manager ensures there are 3 carers provided during the evening until 830pm. The recruitment procedures and records were reviewed for staff and the registered manager was able to provide appropriate references and records regarding this process. The registered provider has a commitment to providing a trained workforce and individual staff files contain training schedules and opportunities. Appropriate training has occurred at NVQ level 2, 3 and 4, food hygiene, healthy eating, activities in care settings, moving and handling, first-aid, infection control and health and safety. The induction records for individual staff are signed and dated by the staff member and senior staff completing the process. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 31, 33, 35, 36 and 38. The registered manager is providing clear leadership to maintain a safe and comfortable home for service users. EVIDENCE: The registered manager is a qualified nurse with appropriate experience and has completed the ‘Registered Managers Award’ in June 2004. The registered manager is continuing to develop the processes at the home to comply with good practice, standards and regulations. The registered manager has developed supervision systems and staff training profiles include fire precautions and safe practice regarding health and safety. The managing director is the responsible individual acting on behalf of the company and visits the home to provide monthly reports to this Commission as part of the quality assessment review process. A recommendation is made regarding these reports. Professional accountants are used to support the management of the business. The registered manager is receiving appropriate support from the registered provider, the appraisal record for 25/10/05 was available.
