Date of last inspection Brief Description of the Service: The Gables is new build home, consisting of 8 apartments over three floors each with their own bathroom, bedroom, kitchen and lounge area. Five apartments provide two-bedroomed accommodation. There is also a kitchen/lounge area on the ground floor which any of the people living there can access. The home is situated near the town centre of Huddersfield, close to all local amenities and public transport. There is adequate parking to the front of the building. The Gables provides personal care for up to 13 people who are younger adults with a learning disability. The Provider informed the Commission for Social Care Inspection on 6 September 2007 that the fees range from £310.06 to £1,512.91 per week. This is an overview of what the inspector found during the inspection. As part of this full inspection, a visit to the home took place. The inspector, Helen Battle, visited the home unannounced from 09.40hrs to 10.15hrs, and 15.15hrs to 19.00hrs. Whilst at the home, key documents such as care assessments, care plans, daily records and staff records were looked at, and so were the apartments. Six members of staff were spoken with, along with the registered manager. Four people living at the home were spoken with. The manager had been asked to complete a questionnaire with information about the service, the people who live at The Gables, and the staff working at the home. This was returned to the Commission prior to the visit taking place. Surveys were sent to ten people living at the home, their families and GPs. Four surveys were returned from relatives. These were generally positive. Comments included: “Staff are very good, caring and helpful”. “I have every confidence in the care my relative receives”. Other comments were included which reflected the difficulties experienced by people living in the home and the staff due to the period of change around the move and that people were still settling in. One health professional responded to the survey and raised issues regarding safeguarding and staffing levels. These areas were looked into during this visit. However, although it is acknowledged that there have been some concerns previously, there was evidence during this inspection that action has been taken to address these issues and improve practice. Six responses were received from people living in the home. All were positive with a comment “I am happy here”. The inspector would like to thank the people living at the home, the staff and manager for their warm welcome and hospitality on the day of the visit. What the service does well:
The staff at the home have worked hard to ensure that people moving to The Gables have been well supported before, during and after the move. People have been given information and have been supported in visiting the home a number of times before the move to enable them to make choices about their apartments.

Care records are of a satisfactory standard and medication is managed to ensure the safety of people living in the home. Staff recruitment policy and procedure protects the people living in the home. Staff are employed in such numbers and are adequately trained to meet the needs of the people living in the home. What has improved since the last inspection? Prospective users’ individual aspirations and needs are assessed. Prospective service users know that the home that they will choose will meet their needs and aspirations. Prospective service users have an opportunity to visit and to “test drive” the home. Each service user has an individual written contract or statement of terms and conditions with the home. The Commission consider Standard 2 the key standard to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 2. Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. People are properly assessed before moving into the home with the assurance that their needs will be met. EVIDENCE: A comprehensive Statement of Purpose and Service User Guide is available for prospective service users. The Service User Guide is also available in Widget form. The people currently living at The Gables transferred from two homes which were closed. People were consulted about the move and were able to visit the accommodation several times prior to moving in. Each person was reassessed to ensure that their needs could be met in their new home. Both relatives and people living in the home confirmed that they had been consulted prior to the move and that they had been able to visit The Gables before moving there. The Commission considers Standards 6, 7 and 9 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 6, 7, 9. Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. Plans of care are in place which reflect the current needs of the people living in the home. People make decisions about their lives and take risks as part of a more independent lifestyle where possible. EVIDENCE: The care plans of two people living at the home were examined. These were generally well detailed and did reflect the current needs of the individuals. Specific detail was in place regarding the management of risk and challenging behaviour. Entries in the daily records are of a satisfactory standard and give an account of the care and support provided and where and how people have spent their day.

All stated that the staff are very good to them and that they are kind and patient. Responses from surveys indicated that relatives, health professionals and people living in the home are all very satisfied with the service provided. The support given by the staff working at the home to enable people to make choices and take risks was good. This was evident on the day of the visit in observing staff interaction and activities with people living in the home. People living in the home stated that they were given choices and consulted about life in the home. JUDGEMENT – we looked at outcomes for the following standard(s): 12, 13, 15, 16, 17. Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. People living in the home take part in appropriate activities and are part of the local community. Appropriate relationships are maintained and people’s rights and responsibilities are respected. Meals provided at the home are of a good standard. EVIDENCE: People living at the home take part in a variety of activities and attend day centres suitable for their needs. These include day centres, going out to do their own shopping, going to local groups and clubs. The people living in the home have chosen to go on holiday to Bridlington and, during this visit, some were busy packing in preparation for this. The local library is also accessed by people living in the home. On the day of this visit, one person was supported to have their hair cut at a local barber shop and another person spoke about how they had been out shopping to town independently. People were seen to be treated with respect and their privacy and dignity maintained by staff during this visit. People living in the home are supported to maintain links and relationships with friends and family where possible. People spoken to stated that they are allowed to see visitors at any time and the home has evidently worked hard with people to support them with maintaining relationships. The meals provision at the home is of a good standard. Feedback from people living in the home, and visitors, was very positive and the meal served on the day of the visit was good. The intended outcomes for Standards 18 - 21 are: 18. 19. 20. 21. Service users receive personal support in the way they prefer and require. Service users’ physical and emotional health needs are met. Service users retain, administer and control their own medication where appropriate, and are protected by the home’s policies and procedures for dealing with medicines. The ageing, illness and death of a service user are handled with respect and as the individual would wish. The Commission considers Standards 18, 19, and 20 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 18, 19, 20. Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. People receive support in a way they prefer and require with their emotional and health needs being met. People living in the home are protected by the home’s policy and procedure for dealing with medication. EVIDENCE: People spoken to stated that the staff are kind and provide personal assistance in a discreet manner which promotes privacy and dignity. People living in the home stated that they are able to choose what times they get up and go to bed. It was evident that people choose their own clothes, hairstyles and makeup. There was evidence in people’s records that support is also provided from specialist nurses and support from the hospital. Care must be taken to ensure that there are no signature omissions in the records held. The manager of the home stated that a full audit was to be undertaken the day following the visit. Procedures are in place to safeguard people living in the home regarding medication. The manager is planning to carry out a weekly audit of all medication held in the home to ensure the safety of people living there. The Commission considers Standards 22, and 23 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 22, 23. Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. Generally, people feel their views are listened to and acted on. People are protected from abuse, neglect and self-harm. EVIDENCE: People spoken to felt that they could speak to any member of staff and that they would be listened to. There have been no complaints made to the home since the last inspection. The complaints procedure is available in symbol form for people living in the home. Safeguarding (Adult Protection) training has been given to all staff. Staff spoken to during the visit demonstrated a good understanding of safeguarding (adult protection) issues and procedures. Prior to this visit, there have been two safeguarding referrals made in relation to risk assessment personal care and investigations have taken place. Issues were raised regarding staff understanding around these issues, however the staff have evidently learned from this and have received up to date training. The senior management of Bridgewood Trust have also taken action regarding improving practice in relation to safeguarding issues. The personal monies of two people were checked and were found to reconcile with the records held.

Shared spaces complement and supplement service users’ individual rooms. Service users have the specialist equipment they require to maximise their independence. The home is clean and hygienic. The Commission considers Standards 24, and 30 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 24, 30. Quality in this outcome area is excellent. This judgement has been made using available evidence including a visit to this service. The people live in a comfortable and safe environment and the home is clean and hygienic. EVIDENCE: The home is newly purpose built and, as such, the accommodation is of an excellent standard. People living in the home have been encouraged to take their own items into the home to personalise their apartments and people also said that they had been involved in choosing furniture and other items. The home was clean and free from any offensive odours on this visit. People living in the home seemed pleased to show their apartments and were evidently proud of their new home. JUDGEMENT – we looked at outcomes for the following standard(s): 32, 34, 35. Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. People are supported by competent and qualified staff, and are protected by the home’s recruitment practices. Staff are receiving mandatory training within recommended timescales. EVIDENCE: All the staff working at the home have received movement and handling training within the last 12 months. All staff have also had updated safeguarding training (adult protection) in the recent weeks. Training records indicated that staff have also had Health and Safety training, Fire safety training and medication training. Staff spoken to were clear in their understanding of safeguarding issues and policy, and regarding fire procedures. Staff also complete training regarding supporting people who have a learning disability. All three of these records were complete. Staffing levels are provided to meet the needs of the people living in the home although it was acknowledged that, due to the layout of the home, there had been some difficulty in adjusting to new ways of working since the move to The Gables. Service users benefit from competent and accountable management of the service. The Commission considers Standards 37, 39, and 42 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 37, 39, 42. Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. People benefit from a well run home, and their views underpin development at the home. The health, safety and welfare of people are protected. EVIDENCE: The manager of the home has experience in looking after people with a learning disability. She has achieved the NVQ level 4 award in care management and is awaiting confirmation that she has achieved her Registered Managers’ Award. The manager has only recently taken up the post of manager at The Gables and already has improved systems and most importantly supported the staff team after a difficult period of change. This has been particularly apparent in people choosing their holiday destination and in planning the move to The Gables. Monthly management visits are carried out. Quality assurance processes are in place, and the manager is looking to improve these systems. Feedback from staff regarding the manager was positive. All the staff spoken to felt that they were well supported by the manager and that she has had a positive impact on the home since starting there. All felt that the manager was approachable and that she acts on any issues raised. Staff stated that they had every confidence in the manager. There were no apparent health and safety issues observed during this visit. Weekly tests of the emergency lighting and fire alarm systems are recorded.
