The home provides residential care for up to 20 elderly people, including up to ten that have dementia (DE (E) or a Mental Disorder (MD (E). There is a garden and car parking space to the front of the home, with a lawn and patio easily accessed by service users. A conservatory has been built onto the front of the home, which also provides staff with a new office. Accommodation is provided on two floors accessed by a stair lift. There is a large lounge for non-smokers with an adjacent smaller lounge for those wishing to smoke. Meals are prepared in a spacious kitchen and served in the dining room, this adjoins the lounge. Assisted bathing facilities are provided and all rooms have call bells. Suitably qualified care staff provide personal care within a relaxed, friendly atmosphere. The Registered Providers are committed to training their staff and over 50 have achieved the NVQ level 2 or above in Care. The General Practitioner’s surgery is close by and the home maintains a very good rapport with the local Doctors, the District Nurses and Community Psychiatric Nurses. There are opportunities for service users to socialise and visitors are actively encouraged. The Hollies DS0000009041.V273478.R01.S.doc Version 5.0 Page 5 SUMMARY
This is an overview of what the inspector found during the inspection. This was an unannounced inspection that took place on the 10th January 2006, between 09.30 and 14.30 hours. The inspector met with the Registered Provider, the Registered Manager, the assistant manager, service users and a visiting District Nurse. The Hollies is a well managed home that benefits from an established and stable staff team. The home has a proactive approach to the welfare and best interests of service users. Overall, the inspector left with a good impression of this home and the care provided to service users. What the service does well: What has improved since the last inspection? The laundry area is far too small, and better provision could be made with a rearrangement of existing facilities. The intended outcomes for Standards 1 – 6 are: 1. 2. 3. 4. 5. 6. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 1, 3, 4, 5 Service users have the information they need to make informed decisions, and the opportunity to visit the home before making a decision about admission. Care needs assessments are carried out for all prospective service users. EVIDENCE: The Registered Providers have produced a Statement of Purpose and Service User’s guide, which include the required information. The Service Users Guide, a Contract, and a Statement of Services and Fees, are given to all service users. The Statement of Purpose and Service User Guide are due for review this month. Prospective service users have the opportunity for a trial visit prior to being admitted to the care home. The registered provider should include the provision for trial visits in the Statement of Purpose and Service User Guide. The most recent admission to the home was not assessed prior to admission. The registered manager must ensure that all prospective service users have their care needs assessed prior to moving into the home. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 7, 11 Clear, informative care plans are in use. Service users confirmed they are treated with respect and were complimentary about the kindness of the staff. EVIDENCE: Care documentation is clear and informative, and includes the involvement of external health professionals in the delivery of care. A visiting District Nurse was complimentary about the care provided by the home and the communication by the home with visiting health care professionals. As discussed at the time of the inspection, there is room for improvement with involving service users/representatives in the review of care plans. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 12, 13, 14 Service users spoken with expressed satisfaction with the lifestyle afforded by the home, and confirmed that they have the freedom to make choices, maintain contact with others important to them, and determine their own lifestyle. EVIDENCE: Most of the service users have family, and some service users ‘share’ visitors. Five service users actively use the local community facilities. There are regular visitors to the home. There is some recorded evidence of social/recreational activities, but this is not recorded as frequently as it could be (especially as the registered provider and registered manager advised the inspector that there are almost daily activities). Two staff are employed for two hours/week specifically for social activities. The registered manager should encourage better recording of the service user’s daily life in the individual service user’s file. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 21, 22, 25, 26 The home provides safe, comfortable accommodation that is personalised, wherever possible, to reflect individual service user preferences. The home works closely with other health care agencies to ensure specialist equipment is provided as needed. EVIDENCE: There are sufficient toilets and bathing facilities provided, though currently one bathroom is not used. The Registered Provider should consider; converting the toilet opposite room 12 to a walk-in-shower room; converting the unused bathroom on the same floor to a laundry room; converting the existing small laundry area for additional storage. The home works closely with supporting agencies and health care professionals to ensure that specialist equipment is made available to service users as necessary. The home is well maintained, clean, and safe for service users.

The Commission consider all the above are key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 27 Service users are served by well-trained and skilled staff, in sufficient numbers. EVIDENCE: Thirteen out of nineteen care staff (approximately 65 ) have achieved NVQ Level 2 or above. On the day of the inspection there were 18 service users resident with the registered manager and 5 care staff on duty. The duty rota shows consistently high staff to service user ratios. The registered providers are in the home almost on a daily basis (one is a nurse and is actively involved in staff training). Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 31, 38 The Hollies is a well managed, safe home that benefits from an established and stable staff team. The home is run in the best interests of the service users. EVIDENCE: The registered manager has worked within the home for approximately 17 years. The registered manager has achieved the NVQ Levels 2,3 and 4 in care, and has also completed the Registered Manager’s Award. She has the A1 and A2 Assessors qualification and is the Moving and Handling trainer for the home. The Inspector observed clear lines of accountability within the home and with the registered providers who are involved with the managing of the home on a day-to-day basis. An experienced deputy manager supports the registered manager. A recent health and safety inspection carried out in November 2005 shows that the home promotes and protects the health, welfare, and safety of the service users and staff. The home enjoys a good reputation and relations within the local community.

STATUTORY REQUIREMENTS This section sets out the actions, which must be taken so that the registered person/s meets the Care Standards Act 2000, Care Homes Regulations 2001 and the National Minimum Standards. The Registered Provider(s) must comply with the given timescales. No. 1. Standard OP3 Regulation 14, 16 Requirement The registered manager must ensure that all prospective service users have their care needs assessed prior to moving into the home. The registered provider must provide a facsimile facility at the home. Timescale for action 31/01/06 RECOMMENDATIONS These recommendations relate to National Minimum Standards and are seen as good practice for the Registered Provider/s to consider carrying out.
