Conditions of registration: Date of last inspection 20th October 2004 Brief Description of the Service: 22 and 24 Thornhill Road is owned and managed by St. Anne’s Shelter and Housing Action and is registered to provide nursing and social care for up to seven adults with a learning disability. The home is situated in a residential area in the Rastrick area of Calderdale, close to local community facilities and public transport links. The property is purpose built and the accommodation is provided on one level and is fully accessible for the service users. All bedrooms are for single occupancy and highly personalised to reflect their occupants interests and tastes. Communal areas are comfortable and furnished and fitted to a good standard. This is an overview of what the inspector found during the inspection. This inspection took place on 4th October 2005. The atmosphere within the home was lively with the majority of the service users preparing to go out to attend activities within the community. The staff team were meeting the needs of the service users in a sensitive and caring manner and were observed to be promoting independence and dignity. Prospective users’ individual aspirations and needs are assessed. Prospective service users know that the home that they will choose will meet their needs and aspirations. Prospective service users have an opportunity to visit and to “test drive” the home. Each service user has an individual written contract or statement of terms and conditions with the home. The Commission consider Standard 2 the key standard to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 2 Service users needs are fully assessed and systems are in place to ensure their needs and aspirations are met. EVIDENCE: The organisation uses a person centred planning service agreement which is reviewed on a regular basis, which ensures that the service users current needs are met. The intended outcomes for Standards 6 – 10 are: 6. 7. 8. 9. 10. Service users know their assessed and changing needs and personal goals are reflected in their individual Plan. Service users make decisions about their lives with assistance as needed. Service users are consulted on, and participate in, all aspects of life in the home. Service users are supported to take risks as part of an independent lifestyle. Service users know that information about them is handled appropriately, and that their confidences are kept. The Commission considers Standards 6, 7 and 9 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 6,9 Service users needs are thoroughly assessed and the home has a good approach to promoting the service users health care. EVIDENCE: All seven of the service users case records were examined and were found to contain detailed information. Each service user has an individual plan of care which is reviewed on a monthly basis. Service users likes and dislikes are identified and recorded. A daily record is kept which clearly identifies how the service user has spent their day with outings and activities undertaken recorded, the record is signed and dated. A health action plan is in place for each service user which details any physical and emotional health care needs. Detailed risk assessments are in place which are regularly reviewed. Service users are able to take part in age, peer and culturally appropriate activities. Service users are part of the local community. Service users engage in appropriate leisure activities. Service users have appropriate personal, family and sexual relationships. Service users’ rights are respected and responsibilities recognised in their daily lives. Service users are offered a healthy diet and enjoy their meals and mealtimes. The Commission considers Standards 12, 13, 15, 16 and 17 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 12,13,14 Service users are encouraged to exercise choice and care is provided in a flexible manner to enable residents to follow their preferred lifestyle. EVIDENCE: Service users are encouraged and enabled to participate in a wide range of community social and recreational activities. Recent activities have included going swimming, visiting the pub, pictures, adult educational classes, and attending hydrotherapy. Holidays are enjoyed by the service users, usually twice a year, and this years destinations have included Centre Parcs, and Blackpool. The staff team are supported by five community support staff from the lower valley support services to ensure that the service users are appropriately supported whilst out in the community. Service users retain, administer and control their own medication where appropriate, and are protected by the home’s policies and procedures for dealing with medicines. The ageing, illness and death of a service user are handled with respect and as the individual would wish. The Commission considers Standards 18, 19, and 20 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 18,19 Service users personal and health care support is given in accordance with their wishes. EVIDENCE: Service users personal support needs are assessed and form part of their plan of care. All personal care is given in private. Shared spaces complement and supplement service users’ individual rooms. Service users have the specialist equipment they require to maximise their independence. The home is clean and hygienic. The Commission considers Standards 24, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 24,25,26,27,28,29,30 The home is well maintained, with high standards of cleanliness throughout, providing a pleasant, fully accessible and safe environment for service users. EVIDENCE: 22 and 24 Thornhill Road is located in a residential area of Rastick close to community facilities and public transport links. The home is purpose built and comprises of an interconnecting semi detached property, designed over a single floor. The home is fully accessible and provides wheelchair access and adaptations to meet the mobility and personal care needs of service users. The accommodation offers spacious communal areas and a private, landscaped and accessible garden, where service users are able to spend time in the warmer weather. JUDGEMENT – we looked at outcomes for the following standard(s): 31,32,33,34,35 The establishment is staffed by a well trained and motivated workforce. EVIDENCE: From records examined and following observation and discussion during the inspection, sufficient staff are employed to meet the needs of the service users. There is always a Registered Nurse on duty who works alongside a team of support workers. The staff team were observed to be meeting the needs of the service users in a sensitive and dignified manner, with appropriate use of informality and humour. There is a commitment to staff training within the home with an on-going programme of NVQ training. At the present time four members of staff hold their NVQ III award and a further three are undertaking their award. All new staff receive appropriate induction training. A wide range of training courses are available for all staff to undertake. The organisations recruitment procedure is robust and all new staff are subject to the relevant checks being carried out.
