This inspection was carried out on 14th November 2005.

The inspector made no statutory requirements on the home as a result of this inspection and there were no outstanding actions from the previous inspection report.

What the care home does well

The staff provide a warm and comfortable clean home for people to live in. Service users said that they are supported to maintain their independence and were treated at all times with respect. Staff were observed to provide appropriate care when supporting service users in maintaining their independence in all daily tasks. Service users made comments such as "Staff are very pleasant, very kind and helpful" All service users were complimentary of the food provided as one service user said, " the food is very good with plenty of variety". There have been no requirements or recommendations made at this inspection.

What has improved since the last inspection?

The home has had a permanent registered manager for short time now. This is reflected in the consistent approach to care provided by the homes staff team. In discussions held with staff it was clear that they felt that they now had direction, guidance and support from the homes management team and that everyone was working in conjunction to ensure that service users receive a good quality service. All of the requirements made from the last inspection had been addressed and implemented. All of the communal areas such as the corridors have been refurbished and recarpeted to a high standard through out the home.

What the care home could do better:

The home should continue to maintain the high standard of care it provides as it continues to meet service users expectations and aspirations.

The home is a detached property that is purpose built which also adjoins a sheltered housing scheme.The accomodation is spaced over two floors and all areas are accessed by a vertical passenger lift. The home is set in its own gounds with a large car park to the front the home. Townend Close is in easy access of local amenities in Crosshills and the towns of Skipton and Keighley are readily accessible. This report relates to an unannounced inspection carried out on the 14th November 2005, which started at 9.30hrs and finished at 15.00hrs. A tour of the home was carried out which included the service users private accommodation, a selection of records were looked at and time was spent observing activity in the home, talking and listening to service users, relatives and staff. The focus of the inspection was a number of key standards, inspecting the care records of four service users in detail to establish if they corresponded with service users experiences in the home. There were also discussions with a visiting community nurse. There were also discussions with staff and the manager of the home. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 1,3 and 6 Prospective service users can be confident before moving into the home that their needs can be met. EVIDENCE: A copy of the new Statement of Purpose was inspected and contained all the required information and is given to all prospective service users. The registered manager or carer carries out a pre-admission assessment and an assessment form is completed prior to admission into the home. A care plan from a placing or funding authority where applicable is also obtained. The information on files was comprehensive and details give good indication of the care needs of prospective service users. All service users are given Terms and Conditions of Residence, which is signed and dated, by service users or their representative and a copy is held on file. The registered manager stated that the home does not provide intermediate care. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 7,8,9 and 10 The health care needs of service users are well met. EVIDENCE: Four service users case files were inspected, including one of the most recent admission into the home which, contained comprehensive plans of care. Risk assessments covering areas of daily living were available and up to date. The health care needs of service users were recorded on their individual care plans. For those service users that are unable to self medicate the home operates a monitored dosage system, which was introduced that day and were inspected. Medication was appropriately stored although the home was waiting delivery for their new medication trolleys and records were accurately maintained. A visiting community nurse also confirmed that the home is always clean and staff are helpful. The health care needs of service users are overseen by one GP practice in Crosshills that has eight doctors. The home is closely situated to Townend Close DS0000007962.V264375.R01.S.doc Version 5.0 Page 10 the local hospital where they use the Accident and Emergency department as and when necessary and appointments for out patients. Staff were observed throughout the day as being polite, courteous and helpful to service users. They were also observed knocking on all doors before being asked into service users flats. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 12,13,14 and 15 Service users’ lifestyles within the home reflects their wishes and service users are encouraged by the staff team to continue with their individual interests EVIDENCE: Visiting arrangements are flexible throughout the day as this was evident by the amount of visitors to the home. It was also confirmed by service users that there relatives were able to visit as and when they liked. The home employs an activities organiser who arranges various activities such as bingo and singalong. A range of activities is available on a daily basis. All service users confirmed that they are able to get up and go to bed as and when they wish. Mealtimes at Townend Court are flexible. Choices are available each mealtime and all service users spoken with commented that the food at the home was very good. Special diets such as diabetics are catered for. Service users in the main lounge confirmed that staff regularly brought round a tea trolley throughout the day. Service users and their relatives and friends are confident that their complaints will be listened to, taken seriously and acted upon. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 16 and 18 Service users are confident that their complaints and concerns are dealt with and are acted upon. EVIDENCE: The home’s complaints procedure is available to service users and visitors. Service users commented that if they had a complaint or a concern that they would speak directly with staff on duty or to the manager. It then would be acted upon. Service users were also confident that the registered manager would always put things right. There has been one complaint received by the home, which has been appropriately investigated but has not yet concluded. The home has received a number of thank you cards and letters from relatives expressing their gratitude to the care that their relatives had received whilst in their care. Service users live in a safe, well-maintained environment. Service users have access to safe and comfortable indoor and outdoor communal facilities. Service users have sufficient and suitable lavatories and washing facilities. Service users have the specialist equipment they require to maximise their independence. Service users’ own rooms suit their needs. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 19,23,24,25 and 26 The environment of the home is maintained to a high standard and provides service users with a clean and homely place in which to live. EVIDENCE: The home is a large building with communal areas on the ground floor. Service users accommodation is over two floors, which is serviced by a passenger lift. There is a large dinning room on the ground floor and a communal lounge that is also on the ground floor. There are three main bathrooms on the first floor and two bathrooms on the ground floor. Service users’ rooms all have en-suite facilities, which have all been personalised with their own furniture and belongings brought from their own homes. Some service users were quite proud of their own rooms. The home was decorated to a high standard. The main hallway and corridors have recently been refurbished and re-carpeted. All areas in the home were very clean and tidy throughout; there were no offensive odours present. All service users commented upon how clean the home was kept. Service users’ needs are met by the numbers and skill mix of staff. Service users are in safe hands at all times. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 27,28,29 and 30 Service users are protected by the home’s vigorous recruitment procedures. EVIDENCE: Staff records showed that the registered manager undertakes thorough checks before staff commence working at the home. CRB checks have been carried out and staff have been checked against the POVA first list. All staff complete application forms. Two written references are obtained. Staff files also contained records of supervision. The staff rota showed that there are six care staff on duty each morning and in the afternoon. This does not also include the management team. Two staff are on waking duty each night with a senior staff member on call. There are nine staff that have obtained either NVQ level 2/3. Five staff have commenced NVQ Level 2 and three have commenced NVQ Level 3. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 31,33,35,and 38 Service users’ health safety and welfare are promoted and protected. EVIDENCE: Throughout the day from discussions held with service users and staff and through observation, Townend Court continues to be managed well with a committed staff team. Service users financial interests are protected by the homes procedures. A new procedure has been introduced and had only been in place for one week. Two signatures are obtained for all financial transactions. Receipts are also obtained for all purchases made on behalf of service users. The organisations health and safety policies and procedures are in place. A number of health and safety records were inspected all of which were up to date and accurately maintained. PAT testing was being completed on the day of inspection on all electrical goods provided by the home.
